What is the difference between the Market-based approach, the Shared-responsibility approach, and the Government-financed approach?

Market-based approach

- government allows the market to determine the price setting and provision of healthcare services without government interference
- In America, the government does not provide healthcare (i.e. it does not build and run hospitals and clinics); it's focus is to help Americans afford healthcare.
- American citizens have to cope with the costs of healthcare largely on their own by buying medical insurance. However, not everyone can pay for the medical
insurance as the market-based system caused the insurance companies to charge higher for insurance premiums to make profit. Thus, lower-income Americans may not visit a doctor when sick.

- Several policies exist in the USA to address the healthcare challenges faced by the lower socioeconomic status groups.

β€’ Medicare is a healthcare insurance provided for those who are 65 years or older, but also covers those under the age of 65 who have disabilities and permanent kidney failure.

β€’ Medicaid is a form of health insurance available to people and families who have limited income or have disabilities.

β€’ There is also legislation on Emergency Medical Treatment. The legislation on Emergency Medical Treatment makes it compulsory for all hospitals to provide care to anyone needing emergency treatment and prevents private hospitals from transferring patients to the public hospitals due to the fear they would not be able to pay for it.

- The market-based approach to the healthcare system provides a range of healthcare services which allows individuals to decide how much healthcare they need and at what price. With the limited government interference, citizens
consider how much to pay and which service provider to choose from, depending on their socio-economic status, in meeting their healthcare needs.

Shared-response approach

- government intervenes by providing basic healthcare services and shares. the costs of some parts of healthcare. Singapore's healthcare system follows this approach.
- Healthcare in Singapore is financed by a combination of personal and collective responsibility, and government provision. Healthcare costs are kept affordable by the provision of heavy government subsidies (Government), collective responsibility by society through risk-pooling (Community), and individual and family responsibility (Individual).

β€’ Healthcare subsidies are available in polyclinics, public hospitals, community hospitals and nursing homes. Lower to middle income groups can also receive additional means tested subsidies at public hospital outpatient clinics and selected private GP and dental clinics under the Community Health Assist Scheme (CHAS).

β€’ Medisave is a compulsory individual medical savings account scheme. It helps Singaporeans to pay for medical treatment without financial difficulty. Working Singaporeans and their employers contribute a part of their monthly wage into the account to save for future medical needs.

β€’ MediShield Life is a health insurance plan for all Singaporeans to help pay for large hospitalisation bills, for life. It allows all Singaporeans to share the responsibility of paying for large bills by risk pooling the financial risks of major illnesses. A deductible is applicable to help focus insurance coverage on large bills.

β€’ Medifund is an endowment fund run by the government that acts as a safety net for needy Singaporeans who cannot afford to pay their medical bills even after MediShield Life and Medisave.

β€’ Other new schemes have also been introduced to relieve rising
healthcare costs for the elderly and lower income groups. Higher
outpatient subsidies for specialist care and medicine have been launched to lower healthcare costs for lower to middle income families. The Pioneer Generation also enjoys special subsidies and Medisave top-ups.

- The shared responsibility approach ensures that the government makes healthcare affordable for all citizens regardless of their income levels. At the same time, citizens play an active role as they develop a sense of ownership and responsibility for their own health because they would have to co-pay for the cost of the healthcare services. This then enables resources to be allocated efficiently by the government to continuously ensure that quality and affordable healthcare is constantly available to all Singaporeans

Government-financed approach

- government provides substantial subsidies and services for most of the healthcare needs of its citizens, which could make some healthcare services free. Sweden follows this approach.
- Sweden provides universal access to healthcare for its people regardless of social class, income or place of residence. The Swedish healthcare system is
publicly governed. The people pay a nominal sum of money for healthcare services as part of co-payment. There is a Voluntary Health Insurance (VHI) which can be bought to complement the publicly financed healthcare. This provides faster access to medical services at GPs and hospitals.
- The average tax rates are higher in Sweden and the governnient finances the healthcare system from the revenue it collects from the tax.
- The government-financed approach to healthcare has a strong intervention from the government in providing comprehensive assistance in terms of healthcare subsidies. This is usually financed from heavy personal taxes on the people. This ensures that all citizens will have access to basic healthcare services regardless of their socio-economic status.
