HARD COPY OF F137/SF10/STUDENT PERMANENT RECORD
Reminders before sending or submitting:

● Form 137/SF10/Student Permanent Record must have the wet signature over printed name and designation of the School Official (must be included in the signatories in the Form 2A portal)
● It must be a certified copy (CTC) and not the original copy of the document
● The copy must be in a sealed envelope with wet signature on the flap of the envelope
● Label format for the envelope:

Full Name of Applicant (Last, First, Middle, Suffix):
Senior High School Name:

Note if the applicant will send the hard copies::

1. Put a separate paper inside the envelope that includes the following information:
● Application Number (123-XXX-XXXX):
● Learner Reference Number (LRN):
● Full Name of Applicant (Last, First, Middle, Suffix):
● Senior High School Name:
● Senior High School Physical Address (Location):

2. If the envelope is already sealed by the school official without the information (Application number, LRN, full name, school name and address), the applicant can include a separate paper with those details. Then put the sealed envelope with Form 137 and the separate paper in another envelope. The outer envelope DOES NOT NEED to have a signature of the school official, only the sealed envelope with the F137
inside.

DOST Undergraduate Scholarship Application:

1.) Head over to DOST's announcement regarding the opening of their Scholarship:

2.) Read the post and the attached pictures thoroughly. It contains the necessary guidelines and qualifications you need to know before applying for the scholarship.

3.) Click the link in the post. It will direct you to the application portal (or you can click this link, as well:)

4.) Click Undergraduate Scholarship.

5. Read and understand EVERYTHING.
Failure to read and abide by the rules/terms/conditions speak a lot about you - whether you are deserving of the scholarship or not.

6.) Look for the link that says "download scholarship forms C-J". Have these forms signed/accomplished.

7.) Look for the link that says "DOST-SEI Undergraduate Scholarship E-Application System"

8.) Register/create an account. Be sure to use a working email and take note of your login credentials. Multiple accounts are strictly prohibited.

9.) Once you have an account, you can start uploading the forms and other requirements. Your progress can be saved - meaning, you can return to it from time to time, as long as you do not click Submit.

10.) Before submitting, REVIEW, REVIEW, REVIEW. You cannot anymore revise/edit your application once you click Submit. It is your utmost priority and responsibility to be vigilant of your application. You won't be given any chance to edit your application regardless of how grave you think your reason is. So be sure to REVIEW. Triple check.

11.) Once you are ready to submit, click Submit. Doing so will submit all your entries including your credentials - this is YOUR APPLICATION; YOUR TICKET. Meaning, submitting another application with the same credentials is a ground for disqualification.

12.) Wait for further announcements/updates regarding your application. Regularly check your email; occasionally, DOST's emails are stored in the SPAM folder.
