[DRUG TEST CONSENT FORM SUBMISSION]
Good day, Lasallians!
All undergraduate students are expected to submit the Drug Test Consent Form (DTCF) to the Student Discipline Formation Office (SDFO) prior to their scheduled drug testing in accordance with the HDA provided by the Health Services Office on the UG Periodic Health Examination (PHEx) and Drug Test.
Students must print the Drug Test Consent Form, read it, and fill in all required information. The required paperwork must be signed by both the student and their parent or guardian.
The DTCF will be collected at the Br. Andrew Gonzalez Hall is open Monday to Saturday, 8:00 a.m. to 5:00 p.m., from May 29 to June 10.
