إيه دور propranolol في علاج Hemangioma؟

💊 Propranolol is a medication commonly used to treat high blood pressure and other heart conditions.
However, in recent years, it has also been found to be an effective treatment for infantile hemangioma, a type of benign tumor that can appear on a baby's skin.

📋 What are hemangiomas?

Hemangiomas are benign vascular tumors that most commonly appear on a baby's skin within the first few weeks of life.
They are typically bright red and raised, and can grow rapidly in the first few months.
However, most hemangiomas eventually stop growing and involute (shrink) on their own, usually by the age of 5-10 years.

💼 How does propranolol work for hemangioma?

Propranolol is a beta-blocker, which means it blocks the effects of the hormone adrenaline.
Adrenaline can cause blood vessels to widen, which can lead to the growth of hemangiomas.
By blocking the effects of adrenaline, propranolol can help to slow or stop the growth of hemangiomas, and in some cases, can even shrink them.

⏲️ When is propranolol used for hemangioma?

Propranolol is not used to treat all hemangiomas. It is typically only used to treat hemangiomas that are:
Causing functional problems, such as blocking vision or breathing
Ulcerated or bleeding
Very large or rapidly growing
Located in a cosmetically sensitive area, such as the face

🥼 How is propranolol given for hemangioma?

Propranolol is typically given to babies by mouth, twice a day. The dosage is based on the baby's weight and the severity of the hemangioma.

هل كل الـ calcium channel blockers ليهم نفس الـ mechanism of action؟

📋 There are three main classes of CCBs, and they differ in their selectivity for calcium channels and their effects on vascular smooth muscle and cardiac tissue:

🔵 Dihydropyridines (e.g., amlodipine, nifedipine):
- Target L-type calcium channels primarily in vascular smooth muscle cells.
- Cause vasodilation and reduce peripheral vascular resistance.
- Minimal effects on cardiac tissue.

🟣 Phenylalkylamines (e.g., verapamil):
- Act on both cardiac and vascular smooth muscle cells by blocking L-type calcium channels.
- Have more pronounced effects on cardiac tissue, reducing heart rate and myocardial contractility.

🟠 Benzothiazepines (e.g., diltiazem):
- Also block L-type calcium channels in both cardiac and smooth muscle cells.
- Have moderate effects on cardiac tissue, reducing heart rate and myocardial contractility.
- Less potent than verapamil in affecting cardiac tissue.
