Periumbilical pain that migrates to the right lower quadrant is a classic finding.

Pain usually occurs before nausea and vomiting.

πŸ‘€ Watch for:
β€’ RLQ tenderness and guarding
β€’ Anorexia
β€’ Low-grade fever
β€’ N/V

⚠️ A sudden decrease in pain followed by diffuse abdominal pain may indicate perforation.

1. Keep the patient NPO.
2. Monitor vital signs.
3. Administer prescribed medications.
4. Prepare for possible surgery.

🚨 Rigidity, tachycardia, hypotension, or confusion may signal peritonitis or sepsis.

⭐️NCLEX tip:
Worsening or diffuse abdominal pain requires immediate reassessment and provider notification.

This is where so many students get stuckβ€¦πŸ‘‡

NCLEX gives you ONE disorder or medication you’ve never heard of and suddenly you think you can’t answer the question. 🀨

You can.

Don’t panic. Don’t guess. Break down the patient.

🩺 What is happening physiologically?

Look at the vitals, labs, symptoms, mental status, oxygenation, and urine output.

🚨 What can hurt this patient first?
πŸ‘‰πŸ» Think airway, breathing, circulation, neuro, and safety.

πŸ’Š Don’t know the medication?
πŸ‘‰πŸ» Look at WHY they’re getting it and what changed after they received it.

❌ Then eliminate answers that delay care, ignore instability, or could cause harm.

Remember:

Patient first. Diagnosis second.

You do NOT have to know every disorder or medication on NCLEX.

You need to figure out what is happening to the patient and what they need FIRST.

That’s clinical judgment. 🧠

πŸ“Œ Save this because you WILL get a question where you don’t recognize something!

Severe pain is expected in a sickle cell crisis… but THIS changes your priority πŸ‘‡

🩸 Understand the WHY:
Sickled red blood cells block small vessels β†’ less oxygen reaches tissues β†’ ischemia and severe pain.

These cells also break down early β†’ anemia, fatigue, and jaundice.

What you need to know for NCLEX:

β€’ Pain crisis: Give prescribed pain medication promptly. Hydrate as ordered and monitor for fluid overload.
β€’ Oxygen: Assess oxygen saturationβ€”give oxygen for hypoxemia.
β€’ Chest pain + fever + trouble breathing: Think acute chest syndrome. Emergency!
β€’ Fever: Spleen damage increases infection risk. Prompt evaluation is needed.
β€’ Sudden weakness or trouble speaking: Think stroke. Act immediately.
β€’ Hydroxyurea: Helps reduce future crises. Monitor the CBC.

πŸ’‘ Answer tip: New breathing problems take priority over uncomplicated limb pain. Look at the whole clinical picture!
