All students whose courses got auto-dropped once more need to:
→ undergo the reinstatement process
→ confirm their enrollment afterward on MyLaSalle (MLS) Portal
Duration of the Reinstatement Process:
📆 May 12 – May 17, 2025

→ The student must pay a fee of Php 2,000.00 and pay to the campus cashier with this assessment form. This is a requirement in the Reinstatement Request Form.

The Health Services Office will conduct the on-site Periodic Health Examination (PHEx) for Undergraduate Students in DLSU Manila from May 26 to July 21, 2025. Students must book an appointment via My.LaSalle (MLS) starting May 23, 2025 at 1:00 PM by logging in at https://my.dlsu.edu.ph, and may only book within the dates assigned to their ID numbers.
📆 PHEx Schedule per ID Number:
- ID 123: May 26 – June 5
- ID 124: June 6 – 19
- ID 122: June 20 – July 3
- ID 121: July 4 – 17
- ID 120 & below: July 18 – 21
The PHEx will take place at the Waldo Perfecto Seminar Room and Br. Connon Hall Clinic Extension. Students who completed an annual PHEx off-site within the last 6 months (from November 2024 onwards) may submit results from July 22 to August 5, 2025.
