This was one of my three groups of factors with internal AFU issues and deficiencies and delays in Western support being the third.
Western underestimation of the Russians during planning goes in the latter group while the former is the main reason why I think more equipment alone wouldn't have drastically changed the outcome.

The key moving forward is learning the right lessons and making the necessary corrections, both in Ukraine and the West.

As more time has passed and more info comes out, I think the most important factor shaping the outcome of the offensive was the Russians, their defensive effort and their resilience to attrition.

OTD in 1994, the Budapest Memorandum was signed.
Under the agreement, the signatories (US, UK and Russia) reaffirmed their obligations to respect independence, sovereignty and territorial integrity of Ukraine in exchange for its adherence to the Treaty on the Non-Proliferation of Nuclear Weapons.
Within the framework of the deal, Kyiv get rid of its Soviet-originated heavy bomber fleet (Tu-22, Tu-22M, Tu-95, Tu-160) and air-launched cruise missile arsenal (Kh-22, Kh-55).
Ukraine also transferred over 1,000 nuclear warheads that remained on its territory after the Soviet Union collapse to Russia for elimination.
