From the two sources I've found, there were only 5 incidents over these years, 3 involving the US Navy and 2 with the South Vietnamese Navy.

Two of the incidents involving the USN were minor collisions with the AGI USS Banner.
In September 1967 with the Soviet Anemometer (not sure class) and in 1968 with the Izmeritel (Type Tuntselov).
No serious damage in either of these incidents.

A more interesting episode:
On April 22nd 1970, USN A-4 Skyhawks, according to these sources, dropped bombs 1-2 cables off the bow and stern of the Pamir-class AGI Peleng, to force it to alter course.
Would love to see any US sources or confirmation of this story.

Those involving the South Vietnamese Navy was also notable.
The first:
On October 19th 1969, off Da Nang, a South Vietnamese patrol boat fired on the Okean-class AGI Gidrofon, starting a fire onboard the latter.

Also an interesting story on deconfliction.
Following the North Korea attack on, and capture of, the US AGI USS Pueblo on January 28th 1968, Soviet AGI were for a time ordered to stop close quarters tracking of US carriers and to remain 30 - 35 nautical miles off the coast.

On Friday, Slovak government unanimously decided to deliver 13 MiG-29 fighter jets to Ukraine.
Slovakia will also provide Kyiv with Soviet-originated 2K12 Kub medium-range air defense systems.
