🛑Abnormal Uterine Bleeding Mangment :

🟡 Treat the cause if present .

🟡 If no identifiable cause ( Dysfunctional Utrine Bleedig ): 👇🏻

🔴Acute bleeding (Medical ,Hormonal , Surgical)

1-medical :✨

🔸Tranexamic acid IV ( 10 mg /kg every 6 hours til the bleeding stops or diminished then chang to oral )

2-Hormonal ✨

🔸OCP cntain ethynil estradiol
One pill every 6 hour till bleeding stop or diminished then
Tapring one pill every 8 hours then every 12 hours

3-Surgical ✨
🔸D&C
🔸Intrauterine Foley Ballon can be inflat as temporary till the drugs action occurs

🔴 Chronic DUB (Regular & irregular )

🔶Regular ( Menorrhagia ) : (medical , Hormonal , Surgical )

1-Medical > Gives for 5 cycle ✨

A- Tranexamic acid (500×3 for first 1-5 days )

🔻Mechanism of action (MOA)
-by inhibition fibrinolysis
🔻contraindications:
- History of DVT ,Pulmonary embolism , DIC

B- Mefenamic acid (500×3 for first 1-5 days )

🔻MOA :
-inhibition of CoX > decrease PGE


2-Hormonal: ✨

A- OCP for 21 day

🔻MOA :
- Atrophy of endometrium
-Also decrease fibrinolysis

🔻Contraindication >> الكتاب

B- LNG -IUS
this most effective method (70% - 97 %) specially in :

-In sever & recurrence case
- Patient not tolerate OCP
- multiparty old woman
-She need contraception

🔻MOA :
-Endometrial atrophy

C- Cyclic Medroxy progesterone acetate
10mg per day for 10 days ( 10-24)

-If she didn't need contaception

🔻MOA :
-Endometrial atrophy


🟢 If all previous method failed : other Hormonal

D- Continues Medroxyprogestron acetate
10 × 3 for 3 cycle

🔻MOA
- induce Amenorrhea

E- GnRH analogous :

🔻MOA :
-Medical induce Menopause


3- Surgical : ✨

A- Endometrial ablation > iatrogenic induce Asherman's syndrome
B- Hysterectomy (last choice bute cure for DUB)



🔶Iregullar (Metrorrhagia )

✨ Should start with Hormonal to induces regullar cycle then treat as sam regular

الفرق بين التوهم والهلاوس والانخداع :

🟢التوهم Delusions : اعتقاد ثابث راسخ خاطئ لا يتزعزع ولا يستجيب للنقاش المنطقي .

🟠الهلاوس Hallucinations : هي ادراك كاذب مع عدم وجود مؤثر خارجي أو داخلي حقيقي .

⭕ بما انو الجو شوب كتير، الكل عم يصير نزق و عصبي. حدا بيعرف ليش ؟

✅ زيادة درجة حرارة الجسم تؤدي إلى زيادة هرمون الكورتيزول (Cortisol) وهذا يؤدي إلى ارتفاع معدل ضربات القلب وضغط الدم ومن ثم إلى النزق و الانزعاج بشكل سريع.

✅ تؤثر درجة الحرارة أيضًا على الدماغ بسبب نقص الأكسجين في مناطق الدماغ التي تتحكم في الاندفاع, وبما أن الجسم يوجه المزيد من الدم إلى سطح الجلد في محاولة للتبريد، ينخفض ​​التفكير الهادئ و وذلك لأن السيروتونين (Serotonin) ينخفض.
