Multimodal analgesia: استخدام عدة وسائل لتسكين الألم (باراسيتامول، NSAIDs، التخدير الموضعي...) لتقليل الحاجة إلى الأفيونات (Opioids).
في عمليات فتح البطن (Laparotomy) يفضل استخدام Epidural analgesia لتسكين ممتاز وتحسين التنفس والحركة.
استخدام TAP block (Transversus Abdominis Plane block) لتخدير أعصاب جدار البطن وتقليل الألم.
إزالة القسطرة البولية مبكرًا لتقليل التهابات المسالك البولية.
الحركة المبكرة (Early mobilization) خلال أول 24 ساعة إن أمكن، لتقليل خطر الجلطات الرئوية والوريدية وتحسين وظيفة الرئة والأمعاء.

Pre-op: Education + Optimize comorbidities + Stop smoking + Nutrition + Correct anemia.
Intra-op: Less fluids + Keep warm + Normal glucose + Laparoscopy + No routine drains.
Post-op: No routine NG tube + Multimodal analgesia + Epidural/TAP block + Remove catheter early + Early mobilization.
الفائدة الرئيسية:
يقلل من المضاعفات العامة (Overall complications).
يقلل مدة البقاء في المستشفى.
يسرّع العودة للنشاط الطبيعي.
لا يغيّر بشكل واضح معدل المضاعفات الجراحية نفسها (مثل تسرب المفاغرة أو النزيف) أو معدل الوفيات.
A. التحضير قبل العملية (Preoperative optimization)
يهدف إلى تجهيز المريض بأفضل حالة ممكنة قبل الجراحة.
يشمل:
تثقيف المريض (Patient education): شرح العملية، الألم المتوقع، وخطة السيطرة على الألم بعد الجراحة.
ضبط الأمراض المزمنة: مثل السكري، ارتفاع الضغط، وأمراض القلب والرئة.
الإقلاع عن التدخين: لأنه يقلل التهابات الجروح ومضاعفات الرئة.
Prehabilitation: تحسين اللياقة البدنية بالتغذية والتمارين قبل العملية.
العناية الغذائية: علاج سوء التغذية إذا وجد.
تصحيح فقر الدم (Anemia): لأن الأنيميا تزيد الحاجة لنقل الدم وتؤخر الشفاء.
المريض المثالي لبرنامج ERAS
ASA I أو ASA II.
قادر على الحركة (Ambulatory).
حالته الغذائية جيدة.
موانع تطبيق ERAS
الجراحة الطارئة (Urgent/Emergency surgery).
ASA IV–VI (مرضى ذوو خطورة عالية).
سوء تغذية شديد.
عدم القدرة على الحركة (Immobile).
B. أثناء العملية (Intraoperative management)
الهدف هو تقليل الإجهاد الفسيولوجي الناتج عن الجراحة.
اتباع بروتوكول تخدير موحد.
تقليل إعطاء السوائل إلى الكمية المناسبة لتجنب احتباس السوائل ووذمة الأنسجة.
منع انخفاض حرارة الجسم (Hypothermia) باستخدام وسائل تدفئة مثل Bair Hugger.
الحفاظ على سكر الدم ضمن المعدل الطبيعي.
استخدام الجراحة طفيفة التوغل (Minimally invasive surgery) مثل المنظار عند الإمكان.
عدم وضع الدرينات (Drains) بشكل روتيني إلا إذا كانت هناك حاجة واضحة.
C. العناية بعد العملية (Postoperative care)
