🟧 تحت تأثير ال ( FSH) عدد من الخلايا داخل ال Ovary يحصل لهم Growth ..
لكن فقط خلية واحدة تصبح Dominant Follicle? او تسمي Graffian Follicle ?

Why ?

FSH induces the appearance of the LH receptor sites in the granulosa cell.

The FSH-induced increase in LH receptors is essential for preparing the graafian follicle for the pre-ovulatory surge of LH which initiates ovulation.

بعض الملاحظات الي ركزت عليهم الدكتورة عليه اليوم

*بتتطلع أكثر من follicles من ovary حوالي خمس إلى ست فقط واحده تصبح Graafian follicle السبب الي يخلي هذه الوحيده الي الي تصبح mature هو امتلاكها بعدد كبير من LH receptors

*لما تجي واحدة وتعطيها Ovariay indication
ممنوع تعطيها أي NSAID لأنه تعطل prostaglandin الي لها دور في rupture وخروج البويضه وبتالي كأنك ما عملت شي

*عشان ما يحصل نزيف أثناء الدوره في ثلاث آليات تساعد على ذالك 👈🏿
*Platelets by formation plug
* PGf2الفا by VC
* fibrinolytic system *(prostacycline،PGE2)

إذا حصلت مشاكل فيهم يؤدي الى إنه أما يزيد النزيف أو يحصل clotting

لما تكون الدوره غزيره معظم النساء تجي تشتكي من وجود قطع متجلطه كبيرة السبب في ذالك الدم كثير وويكون غالب على fibrinolytic system


كثرت استخدام حبوب الحمل estrogen تؤدي الى endometrial carcinoma

*العلامات الي فيها نستدل إنه حصل ovulation
¥نشوف درجه الحرارة نلاقيها زادت نص وتضل ¥كذا لحد ما تجي المنسيز والمسؤول عن ذا هو progesterone .
¥كمان نقيس progesterone في اليوم الواحد والعشرين من الدوره السابقه والمفروض إذا حاصل تبويض يرتفع progesterone.
¥كمان نقيس LH.
¥برضو cervical mucosa تصبح thin , acellular,positive fern test, stretch to 10cm

*إذا ماحصل ovulation تضل follicle تفرز estrogen وهذا بدوره يؤدي الى endometrial carcinoma
وكمان أحيان تظهر وكأنها cyst فتظن إنها حامل تعمل لها فحص pregnant test يطلع نقتف


*Main hormone during follicular phase is FSH

*Main hormone during ovulation is LH

*Main hormone during luteal phase is progesterone
