BLEEDING DISORDER:

Coagulation factors disorder
Platelet
Blood vessels disorder
ABO incompatibility (mechanism)

The most common cause is vit.k def.
Why? Due to decrease vit.k in breast milk ,her mother use antibiotics for long time, antiepileptic, warfarin.

*So both PT,PTT are prolonged*. So that give them vit.k direct after delivery.

Transient thrombocytopenia due to IgG that lead to fragmentation of platelets, So that we need to do CS to avoid intracranial hemorrhage during normal delivery,after birth give mother and her baby steroid and platelets , these happened during first month just then disappear.

أهم SE نخاف منه وما عاد نقدر نتفاده لل steroid هو collapse of vertebral growth

DIC (decrease in factors2,5,8) due to infection
وتحصل عند الأطفال الي يأكلوهم سمن الي بدوره يؤدي إلى necrotising enterocolitis

1mg of Hb produce 35mg of bilirubin.

عشان نحسب الدم كم (85cc×wigh)

-Bleeding disorders affect about 1-2% of newborn .

- vit k is recommended for all newborns 1mg lM immediately after delivery and dose should be increased to 2 or 3 mg in risk baby's

-also vit D3 is recommended dialy from the first day 400IU

-phototherapy for jaundiced baby should bestarted when bilirubin levels is more than 12mg/dl


-exchange transfusions started when bilirubin levels reach 20 mg/dl.

لكن فقط خلية واحدة تصبح Dominant Follicle? او تسمي Graffian Follicle ?

Why ?

FSH induces the appearance of the LH receptor sites in the granulosa cell.

The FSH-induced increase in LH receptors is essential for preparing the graafian follicle for the pre-ovulatory surge of LH which initiates ovulation.

بعض الملاحظات الي ركزت عليهم الدكتورة عليه اليوم

*بتتطلع أكثر من follicles من ovary حوالي خمس إلى ست فقط واحده تصبح Graafian follicle السبب الي يخلي هذه الوحيده الي الي تصبح mature هو امتلاكها بعدد كبير من LH receptors

*لما تجي واحدة وتعطيها Ovariay indication
ممنوع تعطيها أي NSAID لأنه تعطل prostaglandin الي لها دور في rupture وخروج البويضه وبتالي كأنك ما عملت شي

*عشان ما يحصل نزيف أثناء الدوره في ثلاث آليات تساعد على ذالك 👈🏿
*Platelets by formation plug
* PGf2الفا by VC
* fibrinolytic system *(prostacycline،PGE2)

إذا حصلت مشاكل فيهم يؤدي الى إنه أما يزيد النزيف أو يحصل clotting

لما تكون الدوره غزيره معظم النساء تجي تشتكي من وجود قطع متجلطه كبيرة السبب في ذالك الدم كثير وويكون غالب على fibrinolytic system


كثرت استخدام حبوب الحمل estrogen تؤدي الى endometrial carcinoma

*العلامات الي فيها نستدل إنه حصل ovulation
¥نشوف درجه الحرارة نلاقيها زادت نص وتضل ¥كذا لحد ما تجي المنسيز والمسؤول عن ذا هو progesterone .
¥كمان نقيس progesterone في اليوم الواحد والعشرين من الدوره السابقه والمفروض إذا حاصل تبويض يرتفع progesterone.
¥كمان نقيس LH.
¥برضو cervical mucosa تصبح thin , acellular,positive fern test, stretch to 10cm

*إذا ماحصل ovulation تضل follicle تفرز estrogen وهذا بدوره يؤدي الى endometrial carcinoma
وكمان أحيان تظهر وكأنها cyst فتظن إنها حامل تعمل لها فحص pregnant test يطلع نقتف


*Main hormone during follicular phase is FSH
