Infection
Ischaemia (cardiac, mesenteric)

Ignorance (poor control)
Intoxication (alcohol)


Hepatic encephalopathy: precipitating factors are ABCDEFI:
Alcohol withdrawal
Bleeding (GI)
Constipation
Drugs: withdraw any sedatives/narcotics
Electrolyte imbalances
Fluid depletion: stop diuretics
Infections: treat vigorously



Cardiovascular risk factors are FLASH BODIES:
Family history
Lipids
Age
Sex
Homocystinaemia
Blood pressure
Obesity
Diabetes mellitus
Inflammation (raised CRP)/ Increased thrombosis
Exercise
Smoking


Pulmonary embolism: risk factors 7 H's:
Hereditary (eg factor V Leyden, protein C or S deficiency)
History (previous DVT or PE)
Hypomobility (fracture, CVA, severe illness, obesity, long trip)
Hypovolaemia (nephrotic syndrome, dehydration)
Hypercoagulability (smoking, malignancy)
Hormones (oestrogens [esp. in OCP], puerperium)
Hyperhomocysteinaemia



Goitre: differential diagnosis is GOITRE:
Goitrogens
Onset of puberty
Iodine deficiency
Thyrotoxicosis/ Tumor/ Thyroiditis [Hashimoto's]
Reproduction [pregnancy]
Enzyme deficiencies



Gallstones/cholecystitis: risk factors are 5 F's:
Fat
Female
Family history
Fertility
Forty years old

Such as bacterial endocarditis or cyanotic congenital heart disease

Causing neurovascular compression in the upper limb

Such as cystic fibrosis, empyema, bronchiectasis, tuberculosis (with extensive
fibrosis), fibrosing alveolitis or abscess of lung
5/Circulation Such as atrioventricular (AV) fistula in arm (kidney dialysis patients)
6/Cirrhosis
Of the liver (check for other signs of liver disease)
7/Colonic
Such as Crohn’s, ulcerative colitis and coeliac disease
8/Congenital

D - Deficient Immune System AIDS or
preAIDS lymphadenopathy
E - Endocrine - Addisons, Hypothyroid
S - SLE/Rheumatoid arthritis
- Serum sickness
- Sarcoid

I – Iatrogenic from Medications
N – Nutritional (Not eating, hypoglycemia)

✅ متلازمة ستوكهولم Stockholm syndrome وهي ظاهرة نفسية تحدث نتيجة التعرض لصدمة كحالة الخطف تتظاهر بتعاطف الضحية مع خاطفها والاعجاب به وتأييد سلوكه وتصل لدرجة الدفاع عنه والتضامن معه (ده تاعبني وبرضه عاجبني😂)

✅ أُطلق على هذه الحالة اسم متلازمة ستوكهولم نسبة الى حادثة في نفس المدينة في السويد حيث سطا مجموعة من اللصوص على بنك Kreditbanken عام 1973 لستة أيام، حيث بدأ الرهائن يرتبطون عاطفيا مع الجناة ويرفضون مساندة الشرطة لاطلاق سراحهم

✅ ما تزال أسباب ظهور المتلازمة على أشخاص دون غيرهم قيد الجدل، إلا أنه يمكن اعتبار هذه المتلازمة كنوع من الدفاع عن الذات له علاقة بالصدمة survival mechanism

✅ حيث يعاني أغلب هؤلاء من اضطراب ما بعد الصدمة Posttraumatic stress disorder حيث تظهر عند 8% من الرهائن

✅ وتشمل الأعراض الأخرى الكوابيس Nightmares, الأرق Insomnia, الارتباك Confusion, والقلق Anxiety

Some notes :

Bleeding disorders in newborn:
The most common cause is vit.k def.
Why? Due to decrease vit.k in breast milk ,her mother use antibiotics for long time, antiepileptic, warfarin.

*So both PT,PTT are prolonged*. So that give them vit.k direct after delivery.

Transient thrombocytopenia due to IgG that lead to fragmentation of platelets, So that we need to do CS to avoid intracranial hemorrhage during normal delivery,after birth give mother and her baby steroid and platelets , these happened during first month just then disappear.

أهم SE نخاف منه وما عاد نقدر نتفاده لل steroid هو collapse of vertebral growth

DIC (decrease in factors2,5,8) due to infection
وتحصل عند الأطفال الي يأكلوهم سمن الي بدوره يؤدي إلى necrotising enterocolitis

1mg of Hb produce 35mg of bilirubin.

عشان نحسب الدم كم (85cc×wight)

-Bleeding disorders affect about 1-2% of newborn .

- vit k is recommended for all newborns 1mg lM immediately after delivery and dose should be increased to 2 or 3 mg in risk baby's

-also vit D3 is recommended dialy from the first day 400IU

-phototherapy for jaundiced baby should bestarted when bilirubin levels is more than 12mg/dl


-exchange transfusions started when bilirubin levels reach 20 mg/dl.
