🔹 (قصيدة المساء) 🔹


شَفَقٌ تَشَّكَلَ في الغروبِ كما الندى
فَاقَ العقيقَ نَضَارةً والعَسجَدا
والشمسُ في خَجَلٍ تُكَفكِفُ نورها
مِثلُ الجُفُونِ على اللِحَاظِ تَمَرّدا
والليلُ يُرخِي فى المساء سُدُولَهُ
والبحرُ من حُلَل العقيق تَوَّرَدَا
أُوَجُ الجَِبالِ تَمَيَسَت آرامها
لَمّا ثَرى التَبرِ النَفِيسِ تَلَبَّدا
والشَمسُ والشَفَقَ الرخيمَ يَحُفُهَا
طَرفٌ تَكَّحَلَ فِضَةً لا إِثُمدَا
والصَّبُ في كَلف يُكَابِدُ لَوعَةً
فالقلبُ خَفَّاقٌ يرفرف هُدهُدَا
واليَّمُ كَالرّاحِ الخضيب مزخرفٌ
لَمّا بِهِ شَفَقُ المساء تعامدا
شَفَقٌ تَضَّوَّعَتِ الدِيَارُ عَبيرَهُ
ولَوَاعِجُ الافنانِ زِدنَ تَوَرُدَا
والبحرُ قد زاد الغروبَ أناقةً
شَفَقٌ غَدا عَذبُ الفُراتِ لَهُ صدى
والنَّجمُ في شَبَقٍ يُنَاظِرُ وَقَتَه
والبَدرُ في شَغَفِ لِيَعتَلِي المدى
إنّي أرى الشَفَقَ الرخيم رصاصةً
صَادت شِغَافَ اليَوم إن أَزِفَ الردى
مِن زَهَوةِ الأفلاكِ رَونَقُ لونُها
تَطّرَزَتَ شتى البِقاعِ زُمُرُدا
حَبَكَت أيادي الليلِ أزهى بُردَةً
مِن أَنجُمٍ زُهرٍ تُضاهي الفَرقَدا
مِن سُندُسٍ خُضُرٍ مع إِستَبرقٍ
مِن حُليَةَ الياقوتِ خيرِِ زَّبَرْجَدا
فَتَوَشَحَت أبهى سَماءَ إِزَارَها
والبَدرُ في عرشِ السماءِ تَسَيَّدَا
فَغَدت لِمَن عَازَ اللِحَافَ نَمارِقَاً
وغدت لِأَهل الغُبنِ أقدسُ مَعَبدا
الليّلُ يُضرِمُ في المُتَيَّمِ جَذَوَةً
فَتصطَلي الأَشوَاظُ قَلبَاً أُوُعِدَا
في ساعة الإغساقِ لَحنُ قَصيدَةٍ
عزفَ الغروبُ الحُبَّ لحناً خَالِدا

Notes on X-ray from physician point of view

🛑 How to Differentiat between PA and AP films of X-ray ❓❓
◽️ In PA film the clavicles form a V shape in the X-Ray image

🛑 How to determine whether the X-ray dose was adequate or un proper dose ❓❓
◽️ In Adequate dose we can only see the lining of vertebrae in the upper mediastinum
◽️In High dose we see the lining of vertebrae along the whole mediastinum " whole X-ray image" ➡️ it's used for vertebral column imaging
◽️In lose dose the vertebral lining is not seen

🔻Clinically
1⃣In high dose image ➡️ soft tissue is not well seen ➡️we could miss any pathology on lung OR we could wrongly diagnosed the patient as Pneumothorax

2⃣In low dose image ➡️ soft tissue is more prominent ➡️ we mistake a bilateral infeltration pneumonia

🛑 How to Differentiat between inspiratory and expiratory films ❓❓
◽️Usually the 5th ant. rib crossing the mid diaphragm in the inspiratory film while it's above the diaphragm in expiratory film

🔻Clinically
1⃣ The expiratory films may give a false appearance of cardiomegaly

🛑 Others note

1⃣ Prominent aorta in x-ray may indecate that the patient is almost more than 33

2⃣ Mitralization sign ▶️ sign of Mitral stenosis " not specific may indecate others Mitral lesion" ▶️ straightening of the left heart border in a chest x- ray due to dilatetion of left atrium and Pulmonary artery

3⃣ Left cardiophrenic angle is almost 60' degree
If it's Markedly acute ▶️ Right ventricle enlargement
If it's obtuse ▶️ left ventricular enlargement

4⃣ The right border of the heart consists of( Right ATRIUM ) , ascending aorta and SVC

🛑 Respiratory causes of haemoptysis

1⃣ Infective : bronchitis - TB - pneumonia - COPD others fungal & viral infections of the lung
2⃣ Neoplastic : either primary or secondary
3⃣ Vascular : lung infarction - vasculitis ( ANCA-associated, SLE, Rheumatoid arthritis )
4⃣ Parenchymal : Diffuse Interstitial fibrosis, sarcoidosis, haemosiderosis, cystic fibrosis
5⃣ Pulmonary Hypertension
6⃣ Trauma or foreign body in the Airway

🛑 Massive haemoptysis causes?
1⃣ TB
2⃣ Bronchiectasis
3⃣ Bronchogenic carcinoma
4⃣ Trauma in the Airway

➡️ Because the lesion is supplied by bronchial artery ( high pressure )

🛑 Ortner's syndrome
Mitral stenosis ➡️ dilatetion of the lt. Atrium
Leading to :
1⃣ Compression of Left laryngeal nerve ➡️ Hoarseness of voice
2⃣ Compression of the esophagus ➡️ dysphagia
3⃣ Compression of the trachea ➡️ shortness of breath, risk of aspiration cough and hemoptysis

🛑 Hemoptysis associated with Hematuria

➡️ We must consider Goodpasture syndrome as possible cause.

Goodpasture syndrome most often manifests as diffuse alveolar hemorrhage and glomerulonephritis together.

🛑 Hemoptysis only during menstruation in female patients??

Catamenial Hemoptysis
intrabronchial or parenchymal endometrial tissue deposits, and is considered a subset of extrapelvic endometriosis

🛑 Peptic ulcer is more frequent in patients with liver cirrhosis .. Why ?

The liver is responsible for the metabolism and degradation of the Gastrin hormone
In case of liver cirrhosis.. The Gastren is not degradated in the liver ➡️ increase the level of Gastrin hormone ➡️ increase secretion of the pepsinogen and HCl from the stomach ➡️ Ulceration
