انو HBeAg
Hepatitis e core antigen
وجوده وارتفاعه يدل على زيادة ال viral replication
لكن قد يحدث للجين اللي يكونه mutation
بالتالي
عيكون عندي زيادة HBV DNA من غير ال HBeAg
ونعتبرها Active مش carrier
حتى لو HBeAg Negative

🌹 الشخص اللي المفروض كان Vaccinated :
بس في وقت من الاوقات جا لنا وكان نتيجة فحصه
HBsAg ( positive +)
Anti-HBsAg ( negative)
على الرغم انو اخذ اللقاح

قد يكون:
1- بحاجة الى إعادة الجرعة ( كان غير كافي) ..
2- ما واصلش واخذ كل الجرعات .. ممكن يكون اصيب وهو ياخذها
3- او انه اخذ جرعة الفاكسينيشن بعد ما عرف انه مصاب >> طبعا في هذي الحالة بياخذها مع الاميون قلوبيولين.. HBIG

اذا قد حصل عندي sensitization وال IgE صارت مرتبطة على سطح ال mast cell
الدكتور قال عشان يصير degranulation
لل mast cell لازم يصير عندي🌹 cross link🌹 بين ال Allergn مع 2 IgE

🌺 لو في واحد دعس مسمار ..
بيعطوه Anti- tetanus ؟
هو عبارة عن ايش ؟ عن Antibodies من مصدر حيواني .... لذلك لما تحقنها بالجسم .. ممكن يكون الانتي بدي هذا معاه انتيجين يعتبر ك foreign للجسم وفي بعض الحالات ممكن يتسبب في Hypersensitivity reaction type 3 ..
يقوم الجسم يطلع له Antibody
الان كمية الانتيجين اللي دخلت الجسم كبيرة excess ..
فبالنهاية يتكون عندي immune complexes وتؤدي الى ظهور الاعراض ...
وهذا الخبر اسمه serum sickness

ويعتبر🌷 self limited🌷 لانو بنعطيه single dose و بيكمل الانتيجين بعد شوية

لانو ال antigen اللي احنا نتكلم عنه بتنتهي من الجسم .. قبل ما ياخذ الجهاز المناعي وقته وتطلع ال Antibody ..فما تطلع الا بعد نقصان الانتيجين وما يحصل تراكم لل Immune complexes ... فما يصير عندي الserum sickness

.واللي عبارة عن Superinfection- resistance
بمعنى انو اذا دخل الفايرس للخلية يصير Down regulation of receptor
بالتالي تكون الخلية ماعاد تتأثر بفايرس اخر اذا كانت مصابة بالفايرس

بس فيه حالات يكون ال Patient متعرض
لل HBV & HCV في نفس الوقت ؟

مالسبب ؟

السبب انو الكبد يتكون من الالاف الخلايا
فالفايرس B يصيب خلية و الفايرس C يصيب خلية اخرى غير مصابة ..

🌷 There are 6 genotypes of hepatitis C virus ..

🌷 the genotype in middle-east including Arabic country : Yemen , Egypt and so on Is Genotype 4⃣

🌷 we can't develop a vaccination for Hepatitis C virus due to presence of variable Quasispecies .. which change it's antigen خيرااات

🌷The presence of Anti- HCV IgM is not useful to determine if it is acute, chronic or either cured infection ( due to Quasispecies)

🌷 We must do RT-PCR to confirm the results

🌷 Both Acute and Chronic infection present HCV DNA .. But note
🍃 Acute infection >> rised ALT
🍃 chronic infection >> fluctuating ALT

على عكس ما يبدو ان ال Naked more tolerant

Why !!?

Enveloped viruses mean they have outer lipid layer of glycoprotein and lipoproteins (envelop) that can be neutralized easily by various chemical and physical agents .

والعكس بالنسبة لل naked ..

Despite the lack of a lipid envelope, these organisms have a very resistant viral capsid 

بنلاحظ هذا عملياً بمثال !؟

انو وقت ما يكون معايا Naked virus بيكون مقاوم لل PH of stomach بالتالي لو اخذناه عن طريق ال Fecal-oral route بيسبب العدوى. .
ولو كان enveloped بيتدمر الانفلوب وال PH بتقضي على الفايرس
