AB in dog bite
There is prophylaxis and therapeutic AB in this case :

▫️prophylaxis AB :
If it was dirty wound or if it was clean wound but it was (deep,crush, ,in face_hand_feet_gentila)

🕸1st line:
Augmentin(orally) 3-5d 
الdose راح تكون حسب نوع الaugmentin
 
If :
7:1 ➡️ 45 mg/kg/day ÷2
4:1➡️ 30mg/kg/day÷3

🕸2nd line if there is hypersensitivity to first line :
Dual AB
حتى نغطي anaerob والpasteurella
لل anaerob راح نستخدم :
Flagyl 500mg 1×3 (adult)// 1omg/kg 1×3 (pediatric)
اذا جان اكو CI مثل يشرب كحول راح نحول على clindamycin
300mg 1×3

للpasteurella نستخدم :
Ciprofloxacin 500mg 1×2

▫️Therapeutic AB :
If there is :
🔹️ifected wound (with systemic maifestation like fever ,headace)
🔹️near to joint (risk of arthritis)

Give:
Ampicillin +flagyl
The ampicillin :
IV (3g 1x4) for 2_3 d
وما يصير ننطيه ويه الaminoglycoside

من يصير حريق
من أكثر الأشياء الي نخاف منها هن :
cyanide toxicity and co toxicity

ف هسه نجي نعرف لازم شلون نفرق بينهم :
بالعادة الcyanide toxicity يكون مصحوب بCNC mainfestation
altered mental status or seizure
ويكون وجه الشخص ملطخ بsoot تحديدًا داخل حلگه؛ يكون ضغطه منخفض hypotension ومن نسوي ABG راح نلكه اكو anion gab metabolic acidosis ف تكون اكو زيادة بالlactic acid more than 10 mmol
هسه نجي للنقطة الأهم شلون أعالجه؟
بكل بساطه أنطيهhydoxycobalamine
الي راح يتحد ويه الcyanide ويكون Cyanocobalamine الي راح ينطرح عن طريق الكلية ويكون لون الأدرار هنا أحمر
العلاج الثاني أنطي nitrites مثل :
Amyl nitrate
Sodium nitrite
النايتريت راح يسوي اوكساديز للهيموگلوبين ويحوله الى methemoglobin الي راح يتحد ويه الcyanide ويكون cyanomethemoglobin
العلاج الثالث انطي :
Sodium thiosulfat

هذا المريض ننطيه 100% o2 او hyperbaric oxygen therapy الي اكيد ما موجودة بالطوارئ العراقية لذلك نكتفي بالأوكسجين العادي

يعني بأختصار
المريض اللي وجهه احمر CO poisoning
المريض اللي أدراره احمر بعد ما ننطيه hydroxycobalamine
هو cyanide toxicity وغالباً وجهه مليان soot

شوكت تفكر بالPE؟
WHEN THERE IS :

Sinus tachycardia 
Right ventricular strain pattern 
 T wave inversions in the right precordial leads (V1-4) ± the inferior leads (II, III, aVF). This pattern is associated with high pulmonary artery pressure

Simultaneous T wave inversions in the inferior (II, III, aVF) and right precordial leads (V1-4) is the most specific finding in favour of PE,

SI Q3 T3 pattern :
deep S wave in lead I, Q wave in III, inverted T wave in III

Clockwise rotation – shift of the R/S transition point towards V6 with a persistent S wave in V6 (pulmonary disease pattern), implying rotation of the heart due to right ventricular dilatation

من يكون عندي مريض hypoglycemia ليش من أنطيه شيء بيه suger يأكله أو يشربه،المريض راح يصير بيه تحسن لفترة قليلة ويرجع يتخربط على عكس اذا انطيته iv glucose ؟
أكو هرمونات أسمهن :
(Glucagon-like peptide-1 (GLP-1) and glucose dependent insulininotropuc polypeptide (GIP) )
are released from gastrointestinal L cells and K cells, respectively following an oral suger and act via receptors on the pancreatic β cells to increase insulin secretion.
Thus, for a given glucose stimulus to the β cell, there is greater insulin secretion with oral glucose administration (where the gut peptides are released) compared to intravenous glucose administration (which does not stimulate gut peptide release).
This enhanced insulin secretion following oral administration of glucose is termed the "incretin" effect, and GLP-1 and GIP are known as incretin hormones.

يعني بالمختصر السكريات من تنوخذ عن طريق الفم راح تزود أفراز الأنسولين الي راح يزيد الhypoglycemia وما يقدملي الفائدة المرجوة ،
او يقال ربما نكدر نتبع قاعدة ال15 mg يعني كل ١٥ دقيقة ابقى انطيه للمريض 15 mg من السكريات وبعد ٣ محاولات اذا ما فاد أحول للiv glucose

In metabolic acidosis, there is ↖️in H+ so, it leads to:

1️⃣ Reduced myocardial contractility, thus decreasing stroke volume, cardiac output and MAP ultimately leading to hypotension.

2️⃣ vasodilation, which leads to a decrease in total peripheral resistance which lead to more hypotension.

3️⃣It may lead to arrhythmia speically VT.

وهذا أحد الأسباب الي يفسر حدوث الhypotension in DKA patient أضافة الى الdehydration الي حاصل عنده

In the context of IBD, especially when accompanied by bloody stools, loperamide can mask symptoms of more severe complications, such as toxic megacolon, potentially leading to delayed diagnosis and treatment.

وبالمناسبة ال loperamide ميصير ننطيه لل diarrhoea cause by bacteria لذلك لازم نتأكد انه الsource هو virus
