The combination of severe pain, photophobia, reduced vision, contact-lens use, and a focal corneal opacity strongly suggests corneal infection.

Conjunctivitis usually causes discharge and irritation rather than a focal corneal opacity with severe pain and visual loss.

The most likely diagnosis is Otosclerosis.

The combination of gradually progressive hearing loss in a young adult, normal tympanic membrane, absence of infection, and better hearing in noisy environments (paracusis Willisii) strongly suggests otosclerosis.

Chronic otitis media → usually has abnormal tympanic membrane/history of infection.

Presbycusis → occurs mainly in older adults and causes sensorineural hearing loss.

Vestibular schwannoma → typically causes unilateral sensorineural hearing loss ± tinnitus.

The infection can extend beyond the mastoid and cause serious intracranial complications, including meningitis and brain abscess.
