Tales from the paper mill: We installed a new high temperature rated camera housing in the dryer hoods. It's routinely over 200F in there so the cameras have purge air on them and a small cyclone cooler. We got it aimed just right and I looked at my colleague and said "that's not going anywhere" after I gave the ring holding a glass cover on the end. "just an extra 1/4 turn". A few hours later the machine started up and we went home as soon as they were making first grade paper.

So we come in the next morning and the mill manager was grinning at us. He egged me on by saying they had a break last night at that new camera had caught it perfectly. He had printed out a color screen shot of that glass plate falling into the machine. It bounced around 4 or 5 times before finally landing in a nip and breaking the sheet.

Although we had a cable lanyard on the retaining ring, there wasn't a damn thing I could do about the glass window. The machine vibrated the ring off and that was that. At least we had safety tied the camera and that ring to the hand rail.

The Broward Sheriff Fire Rescue said crews responded to a report of an industrial accident in Deerfield Beach, Florida.
Officials said the man got trapped under the hopper arm of a conveyor belt while trying to address an issue with a large production machine.
The man was pronounced dead at the scene, and his identity has not been released, according to NBC Miami.
