Tales from the paper mill: On a field day we were working on our equipment and lost our supply of compressed air. So we go investigate. The two guys working on a gang lock box had locked out our valve when he meant to lock out the valve next to it for a pneumatic lifting system. I tracked them down and got them to change the lockout. OK.

Next month, same damn thing... they locked out our air supply again. tracked them down and they moved the lock.

3rd months... same thing again. Really. One of htem had a clipboard and was checking boxes and the other had a bunch of locks putting on locks and hasps. Raised hell with them about it hoping they'd rememeber next month.

4th month... you guessed it. You see the problem isn't that they locked out my equipment. The bigger problem is they DIDN"T lockout something that some poor schmuck would work on in 30 minutes. So finally I went to management about it. I don't wanna get these guys in trouble because I've got to maintain a rapport with them. So I asked management if they would have a friendly reminder talk with ALL the lockout teams. The guy brought up a fair point, he said I needed to tell him which team so he could go to the source of the problem and I told him.

Those guys were mad at me for damn near a year.

The bigger point is that they were setting up some other guys for failure. How many other lockouts had they pencil whipped? This mill had to bright fucking idea that if something was more than 2 locks, it was a "complex lockout" and you couldn't do it.. THEY had to have their "qualified" guys do a gang box.
