An incident occurred 15 nautical miles northeast of Oman.

The captain of a container ship reported that his vessel was approached by an armed boat belonging to the Iranian Revolutionary Guard Corps (IRGC), without any VHF communication, and then fired upon the ship, causing significant damage to the bridge.

Preliminary reports suggest the tank suffered from a "hang-fire" which a type of "misfire" where the gunner squeezed the trigger but the primer failed to ignite the shell's propellant charge, resulting in a "slow burn/smoldering" of the shells propellant, standard procedure is to wait 5-10 seconds before recovering the malfunctioning shell, although it is believed the ammunition completely ignites after getting into contact with the oxygen rich* crew compartment which result in a violent but short cook off.

Ammunition involved in the accident was a 120mm "JM12A1 HEAT" High Explosive Anti Tank (HEAT) round, a licensed version of the German "DM12A1" HEAT shell produced locally by Daikin, this accident is highly unusual and the first of its kind in Japan.
