Concomitant, #bismuth quadruple, and 14-day triple therapy in the first-line treatment of #Helicobacter pylori: a multicentre, open-label, randomised trial

http://thelancet.com/journals/lancet/article/PIIS0140-6736(16)31409-X/fulltext

Interpretation
Bismuth quadruple therapy is preferable to 14-day triple therapy in the first-line treatment in the face of rising prevalence of clarithromycin resistance. Concomitant therapy given for 10 days might not be optimum and a longer treatment length should be considered.

#Helicobacter pylori and the risk of #dementia: A population-based study

https://www.alzheimersanddementia.com/article/S1552-5260(18)30159-6/fulltext?rss=yes

We determined the association between H. pylori at baseline and dementia incidence until 2015, per natural log (U/mL) increase in titer, and for seropositive/seronegative, using Cox models adjusting for cohort, sex, age, education, and cardiovascular risk factors.

Results
During a median follow-up of 13.3 years, 529 participants developed dementia, of which 463 had Alzheimer's disease. H. pylori was not associated with risk of dementia (hazard ratio [95% confidence interval] for antibody titer: 1.04 [0.90–1.21]; for seropositivity 1.03 [0.86–1.22]), or Alzheimer's disease.

Discussion
In this community-dwelling population, H. pylori was not associated with dementia risk

Effects of #Helicobacter pylori treatment and #vitamin and #garlic supplementation on gastric #cancer incidence and mortality: follow-up of a randomized intervention trial

https://www.bmj.com/content/366/bmj.l5016

Interventions H pylori treatment with amoxicillin and omeprazole for two weeks; vitamin (C, E, and selenium) and garlic (extract and oil) supplementation for 7.3 years (1995-2003).

.. Effects of H pylori treatment on both gastric cancer incidence and mortality and of vitamin supplementation on gastric cancer mortality appeared early, but the effects of vitamin supplementation on gastric cancer incidence and of garlic supplementation only appeared later. No statistically significant associations were found between interventions and other cancers or cardiovascular disease.

Conclusions H pylori treatment for two weeks and vitamin or garlic supplementation for seven years were associated with a statistically significant reduced risk of death due to gastric cancer for more than 22 years. H pylori treatment and vitamin supplementation were also associated with a statistically significantly reduced incidence of gastric cancer.

#Atorvastatin in combination with conventional antimicrobial treatment of #Helicobacter pylori eradication: A randomized controlled clinical trial

..Considering the anti‐inflammatory and antibacterial effect of atorvastatin, the present study aimed at adding this agent to a four‐drug regimen in order to eradicate H. pylori.

In the current study, 110 patients in the control group received a 14‐day regimen of amoxicillin, clarithromycin, bismuth, and esomeprazole, and 110 patients in the intervention group received 40 mg of atorvastatin daily plus the antibiotic regimen for 14 weeks.

Helicobacter pylori eradication rate in the intervention and control groups was 78.18% and 65.45%, respectively (P = 0.025), and there was a significant difference in terms of non‐ulcer dyspepsia between the groups (P = 0.049), but there was no significant difference in age, gender, and body mass index between the two groups (P < 0.05).

Conclusion
The present study results showed that adding atorvastatin to the four‐drug regimen of omeprazole, clarithromycin, bismuth, and amoxicillin is effective in the eradication of H. pylori. Also, the addition of atorvastatin to H. pylori eradication therapy is more effective in patients with non‐ulcer dyspepsia.

Associations between #Helicobacter pylori with nonalcoholic fatty #liver disease and other metabolic conditions in Guatemala
https://2medical.news/2020/11/13/associations-between-helicobacter-pylori-with-nonalcoholic-fatty-liver-disease-and-other-metabolic-conditions-in-guatemala/

Previous studies have suggested an association between Helicobacter pylori (H pylori) and nonalcoholic fatty liver disease (NAFLD). The aim of the current study was to examine the association in Guatemala, a region with elevated prevalences of both H pylori and NAFLD. Associations between H pylori and other metabolic conditions were also examined, as were associations between H hepaticus and H bilis and the metabolic conditions. …
Comparative Effectiveness of Multiple Different First-Line Treatment Regimens for #Helicobacter pylori Infection: a Network Meta-Analysis.
https://2medical.news/2021/04/11/comparative-effectiveness-of-multiple-different-first-line-treatment-regimens-for-helicobacter-pylori-infection-a-network-meta-analysis/

A number of double, triple and quadruple therapies have been proposed as first line H. pylori infection empiric treatments. However, knowledge of their worldwide and regional comparative efficacy is lacking. We examined the comparative effectiveness of all empirically used first line regimens tested against standard triple treatment, using a network meta-analysis (NWM) of published RCTs. METHODS Data extracted from eligible RCTs were entered into a …
