Systematic review with meta-analysis: the efficacy and safety of #tenofovir to prevent mother-to-child transmission of hepatitis #B virus

For pregnant women with high hepatitis B virus DNA levels, tenofovir administration in the second or third trimester can prevent mother to child transmission when combined with hepatitis B immunoglobulin and the hepatitis B vaccine. Tenofovir is safe and tolerable for both the mother and foetus.

Genital infection with herpes simplex virus type-2 (HSV-2) is common, and increases risk of HIV transmission and acquisition. Pericoital use of tenofovir (TFV) gel provided protection from HSV-2 acquisition in the CAPRISA 004 Study.

Of 566 participants at risk for HSV-2, 532 (94%) had first quarter plasma TFV and end-of-study HSV-2 serology available. Over follow-up of 501 PY, 92 incident HSV-2 cases occurred: 77 in women with no plasma TFV detected and 15 with plasma TFV (incidence 20.6 (16.2-25.7) vs. 11.9 (6.6-19.6), respectively). Plasma TFV detection was associated with trend towards reduced risk of HSV-2 seroconversion (unadjusted HR 0.59 (0.34-1.02; P=0.060); HR adjusted for site, age, ≥ 2 sex partners, hormonal contraception, anal sex, and HIV status 0.60 (0.33-1.08; P=0.086)).

Conclusions
Detection of plasma TFV among TFV gel users was associated with trend towards reduced risk of HSV-2 acquisition after controlling for sexual behavior and HIV-1 acquisition.
