🔹 Cut section of thyroid showing solitary encapsulated, spherical lesion of size 2x2cm, grey white in colour with focal grey brown haemorrhagic areas.

🔹It is compressing the adiacent normal thyroid parenchyma.

🔹Normal thyroid follicles appear at the lower right.
🔹The follicular adenoma is at the center to upper left.
🔹This adenoma is a well-differentiated neoplasm because it closely resemble normal tissue.
🔹The follicles of the adenoma contain colloid , but there is greater variability in size than normal.

وهذه الـ Capsule تضغط ع normal follicles
لان الورم ما اخترق الـ capsule لذلك كل ما كبر كل ما شد الـ capsule فتضغط ع ما حولها

🔻Follicular carcinoma

🔹well-differentiated tumour .encapsulated solitary exhibiting capsular and vascular invasion ,and it spreads via the blood
stream.
🔹It rarely spreads to the lymph nodes of the neck ,which tends to occur late in the disease

◽️نلاحظ وجود capsule ومثل ما نعرف غالباً ان carcinoma مافيها capsule لكن هنا بتكون استثناء 🥴
▪️ونفرق بين Folliculular carcinoma وبين الـ Adenoma بانه في الـ Carcinoma
يوجد capsular and vascular invasion ,

🔷السهم الازرق -----> capsular and vascular invasion
🔻السهم الاحمر ----> New blood vessels
حيث ان السرطان بيفرز Growth factor تعمل ع تكون اوعية دموية جديدة 😕

1️⃣ الي في المربع الاول تمثّل Adrenal Atrophy مثلا في ال addison's disease ☠️
2️⃣ الي في المربع الثاني تمثّل Normal adrenal gland 🙂
3️⃣ الي في المربع الثالث تمثّل Adrenal Hyperplasia خاصه عند مرضى ال Cushing's syndrome ⚠️
