بيكون معانا على أربع مراحل :
✨1- inspection
✨2- palpation
✨3- percussion
✨4- Auscultation

-و أول شيء ذكرت الدكتوره شروط الـ
local examination
بالنسبة للطبيب
- أنه يكون على يمين المريض .

و ايش الشروط بالنسبة للمريض ؟ وكانت :
-posation
-exposure

🦋في هذا المعمل أخذنا ال inspection و ايش أول حاجة بنشوفها في ال abdomen 👈
✨ -Contour of the abdomen .
✨-Resperation .
✨-Subcostal angle .
✨- Epigastric pulsation .
وشرحت كل هذا

و بعدها أخذنا ال divarication of recti-

و أخذنا كيف نفحص ال umbilicus و ايش ال normal و ايش ال abnormal

و أيضًا تطرقت لموضوع ال hernial orifices .

و أخر شيء فحص ال Skin و ال scars و أنواعها .

⚜تقرير معمل الميكرو الاخير⚜

في البداية راجعت المعمل الاول والي اخذنا فيه بكتريا( Enterobacteriaceae)الي قسمت عن طريق Biochemical test👇
1⃣ Shigella

2⃣ Salmonella

3⃣ Escherichia coli

4⃣ Brucella

وفي هذا المعمل كملت البقية واخذت ٣ انواع الي هن 👇

1⃣ H.pylori

2⃣ vibrio

3⃣ Campylobacter

اولاً بدأت الدكتوره بمراجعه المعمل السابق.
وانتقلتا لموضوع معملنا وكان عن Live Function Test .

✨ prothrombin time &INR .

⭐️ Excretory Function ;
✨Serum Albumin {total , direct, indirect} .
✨ Urine bilirubin.

✨ Urine bile salts والدكتوره هنا قالت نقراء موضوعه بس .

نفس الشيء قالت الدكتوره نقراء او نعدي لان مش مهم.

⚫️ملاحظة : بالنسبة لتفريغ المعمل الاول ميكرو الذي نشرناه كان في بعض التعديلات فيه فلاحد يذاكر منه.. ذاكروا من هذا فقط

منشورنا اليوم عن المعمل الثالث والأخير لمادة البارا🪱.

🔸حيث بدءت الدكتور بشرح المجموعة الأخيرة من ال worms ال cestoda(tape worms) والتي تتضمن :
1⃣Taenia saginata.
2⃣Taenia solium.
3⃣Hymenolepis nana.

وذكرت الدكتورة ان ال T.solium أخطر من ال T.saginata لان ال eggs حقها تكون infective stage وممكن تسبب مرض يسمى ال cysticercosis.

🔴Disease : Taeniasis

🔸الذي علينا ال T.saginata وال H.nana لان T.solium يكون ال intermedate host حقها يكون ال pigs أعزكم الله. لذلك غير منتشرة في البلاد الإسلامية.

🔸نبدء بال
1⃣Taenia saginata ( Beef Tapeworm)
🔹Diagnostic stag : Egg or gravid segment.
🔹intermediate host : cattle.
🔹infective stage : Cysticercous bovis(larva).
🔹Mode of infection : by eating raw or undercooked meat that contain larva.
🔹Habitat : small intestine of human.
🔹Definitive host : human.
🔹sample : stool

⚪️Characters of Egg
🔹Size : 40£ مش علينا*.
🔹shape : rounded with radially striated shell.
🔹color : yellowish brown.
🔹content : hexacanth onchosphere(6-hooked embeyo).

🟠Characters of gravid segment
🔹uterus with lateral branches on each side ( 15 - 30 ) branch.

🔴لأن هذه الدودة highly active ؛ لذلك تذهب الى ال appnedix ويسبب انسداد والتهاب فيها ( appendicitis ).

🔴ال life cycle مش علينا.

3⃣Hymenolepis nana (Dwarf tape worm)
الدودة الشريطية القصيرة(القزمة)
🔸Diagnostic satge : Egg.
🔸Intermediate host : Human.
🔸Infective stage : Cysticercoid larva.
🔸Habitat : Small intestine of human.
🔸Definitive host : Human
🔸sample : stool.

⚪️Characters of Egg
🔸Size : 45×30£ مش علينا🥴.
🔸Shape : rounded with tow polar thickening from each arises 4-8 filaments.
تشبه العشرين الريال🪙🥴.
🔸Color : Trabslucent.
🔸Content : Hexacanth onchosphere (6-hooked embryo).

🔴 life cycle: مش علينا دعمموا😅
🔸Direct(auto infection).
🔸Indirct by flea(البراغيث)

🔴ثم انتقلت الدكتورة تشرح مجموعة أخرى من الطفيليات وهي ال protozoa.

🔵Protozoa:
are unicellular organisms, made up of a single cell capable of performing all functions of life.
كائنات وحيدة الخلية يعني خلية واحدة ولها القدرة على أداء كل الوظائف الحيوية😳.

🔵Classification of protozoa according to habitat :
1⃣.Intestinal protozoa :
( 1.Entamoeba histolytica & 2. Giardia lamblia).
2⃣.Blood protozoa (Plasmodium)
3⃣.Blood and tissue protozoa(Leishmania).

🔴أخذنا فقط ال Intestinal protozoa الذي هن :

1⃣Entamoeba histolytica(pathogenic).
🔹Habitat : large intestine(colon).
🔹Disease : Amoebiasis , Amoebic dysentry , Amoebic hepatitis.

⚪️Morphology
🔸Trophozoites : (Pathogenic stage)
أي هي التي باتسبب المشاكل في الجسم
🔹Shape : Amoeboid.
🔹Movement : directional by pseudopodia.(Motile).
🔹Content : may contain RBCs so called (invasive satge).
لانها بتتغذى على خلايا الدم الحمراء🩸.
🔹Multiple : by binary fission
بالأنشطار الثنائي.
🔹Smaple: Diarrhea.
خلال نص ساعة تؤخذ العينة.

🔵Cyst (I.S & D.S)
🔸Immature cyst :
🔹Shape : round or oval.
🔹content : contain nucles , Glycogen vacuoles , 2 chromatoid bodies (rod- like).

🔸Mature cyst :
🔹Shape : round or oval
Content : contain 2 - 4 nucleus.
(Resistant to gastric acid & desiccation).

🔸Diagnostic stage (D.S) :
🔹Immature cyst in fluid or semi - formed stool( 1hour )
🔹Mature cyst in formed stool.
🔹Trophozoites : diarrhea stool.

🔸Infective stage(I.S) :
🔹Mature cyst

🔸Definitive host(D.H) :
🔹human only.

🔸Habitat :
🔹Caecum and simoid rectal in large intestine.

🔸Mode of infection :
1.Autoinfection.
2.Ingestion of contamined food or drink with infected feces.
3.Food contaminated flies or hands of infected food handlers.
4.Direct transmission by cyst carrier(asymptomatic patient).
شخص حامل للمرض ويكون معدي للآخرين بدون ظهور عليه أعراض المرض😈.

🔴ذكرت الدكتور بعض الفروق بين هذا الطفيل العدائي E.histolytica وبين الطفيل المسالم E.coli.
1⃣.E. coli larger than E.histolytica.
2⃣.E.coli contain 1 - 8 nuclei.
3⃣.E.coli don't eat RBCs. it eat other microbes in intestine and food remnants.
لاتتغذى على كريات الدم الحمراء🩸 وإنما تتغذى على البكثيريا الموجودة في الأمعاء وبقايا الأكل.

🔴ثاني نوع من ال intestinal protozoa

2⃣Giardia lamblia (Intestinal flagellates)

🔸Disease: Giardiasis

🔸Habitat: Small intestine especially duodenum may migrate up to bile duct, infection extened to gall bladder.

🔸 It is more common in children.

🔸Mode of infection :
1⃣. Autoinfection (hand to mouth by fecal - oral route).

2⃣. Ingestion of cysts in contaminated food or drink, food handlers, flies

🔸Trophozoite
🔹Spape :  pear like-shaped broad rounded anteriorly and tapering posteriorly.
🔸ايضاً ذكرت الدكتورة أنه يشبه البومه وايضاً يشبه الوجه look like face.
🔹content : Has 4 pairs of flagella,anterior, lateral, ventral & posterior with 2 large sucking disk at the anterior portion and has 2median bodies with axostyle.

🔹Movement :  Highly motile due to they have flagella.

🔹Multiple : by binary fission.

⚪️Cyst (I.S & D.S):

🔸Mature cyst:
🔹shape : It is round or oval shape and thick shelled
🔹content : contain 2-4 nucleus grouped at one pole.

🔹There are axoneme and fibrils..


🔸Diagnost stage (D.S)
🔹motile trophozoite in fluid or semi-formed stool.
🔹cyst in formed stool.

🔸Infective stage (I.S):
🔹Mature cyst.

🔸Mode of infection:
نفس ال E.histolytica.
1⃣. Autoinfection (hand to mouth by fecal - oral route).

2⃣. Ingestion of contaminated food or drink with Infected feces.

3⃣. Food contaminated flies or hands of infected food handlers.
