هاي تصير بيك لما عينك تتعرض لكميات جبيرة من الـ UV light. تلكاها خاصةً عند الحدادين لأن يشتغلون باللحيم. تلكاها همين عند الناس اللي يعيشون بمناطق ثلجية (مثل الإسكيمو) لأن الثلج يعكس أغلب الـ UV اللي يسقط عليه بحيث يشتغل مثل المراية ويعمي العين. بهالحالة يسموها عمى الثلج Snow blindness.
فكرة الـ photokeratitis هي مثل حروق الشمس: بحروق الشمس جلدك رح يحترك، بينما هنا رح تحترك القرنية والملتحمة مال عينك cornea & conjunctiva وتسببلك ألم قوي كلش كأنما حبات رمل خشن وحار فاتت بعينك.

الإسكيمو حلّوها للسالفة بأن صنعوا لنفسهم نظارات خاصة من العظام والجلد والخشب. فكرة النظارات أنها تحتوي فتحات رفيعة مثل اللي بالصورة بحيث ما تدخّل إلا كميات قليلة نسبيًا من الضوة، بس بنفس الوقت ما تؤثر على الرؤية (يكدر يشوف كلشي ومدى الرؤية مالته طبيعي بشكل أُفقي، بس العمودي بيه نقص). النظارات تكون مطلية باللون الأسود من الداخل حتى تقلل الضوة المنعكس بعد أكثر.

There is growing evidence that some relationship may exist between panic disorder and chronic obstructive pulmonary diseases (COPD). There is increased prevalence of panic disorder among patients with COPD. Moreover, patients with panic disorder have an increased childhood prevalence of respiratory diseases, in particular, asthma and bronchitis.

There are several differences between panic and anticipatory anxiety. Panic attacks are seldom accompanied by stimulation of the HPA axis. Cortisol elevation has been commonly reported in fear and phobic anxiety. Panic attacks are characterized by the prominence of respiratory and cardiac symptoms, in particular, breathlessness. This is not the case for fear or phobic or generalized anxiety.

الفرضية تكول أنّ الـ panic disorder يكون عند الافراد اللي يكون عدهم exaggerated respnose to suffocation.
يعني يحسون بالاختناق بسهولة أكبر من باقي الناس وبالنتيجة فالـ panic attack هي ردة فعل حتى الجسم يقلل من الشعور بالإختناق
Which is why both breathlessness and hyperventilation are characteristic of this disorder

This also reminds me of James-Lange theory & Somatic-marker hypothesis

Heard a patient once describing her MI as panic-attack like symptoms, and "عبالك مي حار ونزل على گلبي"

Also, had a patient with lumbar disc herniation describing his foot weakness as "رجلي ما تلزم الكاع"

This is the sort of things that cannot be learned except by experience and interacting with patients...
This experience is very essential so that you can translate the patient's description into the scientific, English nomenclature that we, as med students, are used to.
