Non-insulin therapy

• GLP1 & SGLT2 have a mAcrovascular protective effect.
SGLT2 in particular protects against heart failure & diabetic kidney disease.

• TZDs (pioglitazone) increases rates of HF & hepatotoxicity. Rosiglitazone is not used anymore due to these reasons.

• Metformin can cause lactic acidosis especially in RF.

• Sulfonylureas, on the other hand, can cause significant hypoglycemia. They also lose their effect (and cause total B-cell failure) after long-term use "sulfonylurea failure."

• They lead to weight gain too.

• DPP-4 drugs (all end in -liptin) shouldn't be used with GLP1.

• SGLT2 (all end in -flozin) is really fucking good. But use it cautiously with DM-1 as it can precipitate euglycemic DKA.

• SGLT2 & Metformin are euglycemic agents (do not cause hypoglycemia).

• the GLP1 guys all end in -tide
Insulin therapy

• Add zinc/protamine to your regular (short-acting) insulin, and it becomes a long-acting insulin (called isophane or NPH insulin). Add a lot more zinc ions, and it becomes (lente insulin).

• Liver & renal disease delay insulin breakdown in the body.
