That's fake news: the certification process for consoles can't begin years before the release even for the biggest projects. It starts several months prior.
Instead, the most promising projects like GTA or Call of Duty have allocated QA teams within Sony and Microsoft, who ran extra tests to polish games.
Failing QA tests is expected and even desirable (to find, track, and fix issues).
