Since the start of the year, I have had a growing number of faces flash before my eyes. Each was a patient whom I’ve sat across from and told that they have cancer. No matter how many times you deliver the news, it never gets easier, even for an oncologist like me. You watch as the patient’s world shatters, their conceptions of self, time, safety and legacy coming into focus in one terrifying moment.

The saving grace of these conversations, if there is one, is that the patients find immense hope in the treatment plan you lay out for them. But as of Jan. 1, the enhanced premium tax credits for Affordable Care Act Marketplace plans expired. Due to Congress’ inaction, millions of Americans will lose access to quality, comprehensive health insurance and find themselves uninsured, stuck in their fear and facing added costs as a barrier to a path forward. What can I say to the patients who will no longer be able to afford health coverage and lifesaving care?

Health insurance is a key indicator of whether someone will survive their cancer diagnosis, and the enhanced health care tax credits for Marketplace plans — called New York State of Health here at home — place comprehensive, affordable health insurance within reach for millions. They do so for small-business owners and their employees, farmers, service workers and so many more Americans who cannot access health insurance through their employers and do not qualify for Medicare or Medicaid. These are members of our communities who contribute to our economy, work hard to achieve their American Dream and do everything “right,” yet their continued ability to access health insurance is in question. 

The hundreds of thousands of New Yorkers who have relied on Marketplace coverage deserve affordable access to annual check-ups, routine cancer screenings, everyday prescriptions – and visits with oncologists. Open enrollment has ended, and people in every corner of the state are pondering what to do. Some already lost their coverage on Jan. 1 because they couldn’t afford their new premiums, which skyrocketed without the enhanced tax credits. Many others are struggling with costs and may not be able to continue paying their premiums as the year goes on. On top of increased deductibles and out-of-pocket maximums, this could bankrupt them.

Continued delays and inaction will meaningfully hurt patients who are in active treatment with a serious diagnosis. Treating this issue as a political bargaining chip does not give proper consideration to the millions of cancer survivors, along with a multitude of others, who currently rely on the health care tax credits for both the treatment of disease and the prevention of it.

As health care costs continue to skyrocket, patients should not be forced to choose between lifesaving medications and groceries, chemotherapy or radiation treatments and rent. This will devastate communities, bury families in debt and burden emergency rooms and health systems with uncompensated care.

We are grateful to our New York members of the House of Representatives who voted to extend the enhanced premium tax credits, and we are urging our entire congressional delegation to work together to find a bipartisan path forward to extend these lifesaving tax credits. On behalf of New Yorkers who are being priced out of their health insurance and will also hear the news, “You have cancer,” I urge Congress to make their life-altering diagnosis a little less scary.
