🔵 اثر تحریک جریان متناوب ترانس کرانیال (#tACS) بر بهبود عملکردی در مبتلایان به #سکته_مغزی

#سکته_مغزی یک اختلال نورولوژیک شایع است می تواند باعث #ناتوانی های قابل توجه شود.
تحریک جریان متناوب ترانس کرانیال یک تکنیک #نورومدولاسیون غیرتهاجمی است که اسیلاسیون های #مغز را تنظیم و ريتم های #مغز را مجددا شکل می دهد.

در #tACS، جریان های الکتریکی متناوب سینوزوییدی از طریق الکترودهای اسکالپ در نواحی مشخص #مغز به سر اعمال می شود تا فعالیت #مغز مدوله شود.

مدت تحریک، فرکانس، آمپلیتود، تفاوت فاز، و محل تحریک پارامترهای اصلی در کاربرد #tACS هستند.

شدت جريان متناوب در دامنه ۲-۰/۵ میلی آمپر با استفاده از الکترودهای فلزی یا رابر با سطح تماس پوست-الکترولیت ۳۵-۲۵ سانتی متر مربع اعمال می شود.

جریان الکتریکی بطور متناوب بین دو الکترود (الکترود مثبت به نام آند، و الکترود منفی به نام کاتد) با موج سینوزوییدی جریان می یابد. این جریان های الکتریکی مثبت ثابت و ضعیف بر #نورون های کورتیکال‌اثر کرده و تحریک پذیری کورتیکال را تغییر می دهد.

بعلاوه، #tACS عملکرد #مغز را بوسیله مدوله کردن فعالیت اسیلاتوری اینترینسیک وابسته به فرکانس بهبود می دهد.

این مقاله مروری نشان داد که #tACS در بهبود عملکردی مبتلایان به #سکته_مغزی موثر است.

همچنین کاربرد #tACS با بهبود عملکرد کلی، اختلالات حسی حرکتی، آفازی، و نادیده انگاری (نگلکت) مرتبط است.

با توجه به کاربرد بالقوه کلینیکی #tACS، انجام تحقیقات با کیفیت بالا مبتنی بر شواهد پیشنهاد شده است.

tDCS improves functional gait capacity in patients with MS with a low quality of evidence but not static balance with a very low quality of evidence. Similarly, there were no significant differences between stimulation in the #M1 or #cerebellum. Further studies with parameter #standardization and #individualization of application are needed to increase the success of tDCS therapy.

Transcranial direct current stimulation for #fatigue in neurological conditions: A systematic scoping review

This review will provide insight into the stimulation location that is most frequently used to effectively alleviate fatigue for a range of neurological conditions. Given its substantial benefit in fatigue re-covery, additional primary research may be done to strengthen the current evidence and allow for more appropriate recommendations to be made regarding incorporating tDCS as a treatment for efficient fatigue reduction.

Anodal tDCS of bilateral #S1 or left #DLPFC can significantly reduce #fatigue in patients with MS. Importantly, tDCS relieves fatigue in low physical disability but not high physical disability MS. Furthermore, rTMS relieves MS-associated spasticity and tDCS improves MS-associated #CognitiveFunction, but tDCS has no effects on MS-associated #mood and #pain. No conclusion can be made regarding motor function effects of NIBS due to the variety of protocols. Open questions pertain to the longevity of effects, the differential outcomes following different stimulation intensities and whether specific subtypes of MS benefit more from stimulation treatments than others.

This meta-analysis demonstrated a favorable effect of tDCS on #fatigue, #pain, and #psychiatric symptoms, as well as a favorable effect of TMS on fatigue and #spasticity. These findings support the idea that NIBS could be a promising alternative symptomatic therapy for patients with #MS.

Effects of Transcranial Direct Current Stimulation on pain and pain-related outcomes: an umbrella review

Our findings suggest that tDCS might be effective for #fibromyalgia, #migraine, and #neuropathic_pain associated with spinal cord injury and #stroke. However, further evidence is needed for chronic orofacial pain, multiple sclerosis, knee osteoarthritis, central post-stroke pain, intra-abdominal pain, and phantom limb pain.
