Treatment of invasive bladder cancer:

A.Partial Cystectomy: removal of 2 Cm margin of normal tissure around the tumor.
هاي تفيدني بحالة الtumor جان
1-Single
2-Away from uretric orifice by at least 1 inch
3-must be in dome of bladder


B. Radical cystectomy with urinary diversion: it's usually tx of choice for invasive bladder ca.
هذا complete cystectomy
ال Urinary diversion عدنا اربع انواع راح احجي عليها بالصور الجوة

1- Uterosigmoidostomy
هنا الفكرة شنو ، راح يربط ال ureters ثنينهم بالsigmoid colon
وبهل حالة both feces and urine are excreted through anus

2- ileal conduit
هنا حيربط ال ureters بجزء من ilum مباشرة ومن الilum لstoma خارجية بالبطن

3- Cutaneous Uretrostomy
هنا عدل وكبل مباشرةً يربط الureters بستوما ليبرة

4- Orthotopic ileal neobladder
هنا مثل راح يسوي bladder جديدة
عن طريق ياخذ سكشن صغير من الileum ويربطه من جهة بال ureters ومن جهة لخ بال urethera

هاي شنو سعيات الجراحة

اذا ماتغير شي بالاعتراضات الم ال١٢٨ واحد ونسوي انقلاب على فرع الجراحة وعلى الممثل علواهس

الفائدة من ال midline incision مال laparotomy:
1- linea alba is avasucular structure
2- good access to the abdomen
لهذا بحالات ال emergency نفتح midline

Liver Tumors

Benign Tumors
1- Haemangioma
No malignant potenial

2- Focal nodular hyperplasia
No malignant potential

3- Hepatic Adenoma
هنا المشكلة
Have malignant potenial,
Seen in women aged between 25-50yrs
And associated with use of oral contraceptive pills
مشكلتها الثانية هي
Liable for rupture --> Hemoperitoneum
