✨Anatomy of the hand ✨

🖇The hand is the manual part of the upper limb distal to the forearm .
طيب وظيفتها شنو؟
✨The human hand is designed for:
▪️grasping
▪️precise movement
▪️serving as tactile organ
🔥muscle of the hand 🔥
🖇 اول حاجه ح نقسمهم ل
: 3 groups
♻️ Muscle of lateral compartment ( Thenar mucle) :
1⃣ Abductor pollicis brevis:
▪️: from common origin of the thenar m
يلي هو: - lateral part of flex retinaculum
- tubercle of scaphoid
- crest of trapezium
▪️I : base of proximal phalanx of the thumb
الأعصاب البتغذيها هي:
median n
▪️البتعمل Abducts the thumb.
2⃣ Flexor pollicis brevis:
نجده في
▪️ : base of proximal phalanx.
▪️الأعصاب الداعمه له: Median .N
طيب وظيفهالجزءده شنوووو:🤔 flexes the proximal phalanx of the thumb .
3⃣ Opponen pollicis:
▪️I : ant surface of the 1st metacarpal bone.
▪️ N/S : median n .
▪️A ction.: opposes the thumb against other finger.
4⃣ Adductor pollicis: في ناس بعتبروها تبع ال intermediate group
▪️Origin : 🔹 oblique head from base of the 1st &2nd metacarpal and capitate bone
🔹 transverse head: from the front of the 3rd metacarpal
▪️ بتنتهي في
: base of proximal phalanx of the thumb.
▪️N/ S : deep branch ofulnar n
▪️Action: adducts the thumb.
♻️ نشوف ال hypothenar group: Muscle of medial compartment
1⃣ Abductor digiti minimi
2⃣ flexor digiti minimi brevis
3⃣ opponens digiti minimi
all supplied by deep branch of ulnar n
♻️ muscle of the intermediate compartment:
تنقسم لمجموعتين
1⃣ lumbrical muscle
2⃣ interosseous muscle وتنقسم إلي palmar and dorsal
▪️palmaris brevis m.
🖇ودي بتطلع من الflexor retinaculum وال palmar aponeurosis
وبتعمل insertion في ال. skin of the ulnar border of the hand
N supply: superficial br of ulnar n.
وبتعمل cupping of the hand.
✨اما بالنسبه ل Deep fascia:
بنلقاها Thin over the thenar and
hypothenar but thick in the centeral part ( palmar aponeurosis)
🖇نتكلم شويه عن ال palmar aponeurosis:
هو. thicken of deep fascia triangular in shape occupied central area of the hand .
♻️ the apex attach to the distal borader of flexor retinacullum .
♻️ the base divided into 4 slips at the base of the fingers
وظيفتو شنو؟
protect the underlying structure and give firm attachment to the skin.
🖇 طيب ال retinaculum دا شنو؟
عباره عن strong band of deep fascia
عندنا flexor and extensor retinaculum
▪️ flexor retinaculum stretches across the ant suface of carpal bones covering the concavity of the carpus into atunnel for the passage of flexor tendons from the forearm to the hand.
▪️ Extensor retinaculum:
lies obliquely across the extensor tendons at the junction of the back of forearm & the hand.
longer ,thinner, & higher than flexor retinaculum.

▪️carpal bones:
وديل 8 مرصوصين علي صفين
1- proximal row from lateral to medial ➡️ scaphoid, lunate, triquetrial & pisiform
2- distal row ➡️ trapezium, trapezoid, capitate & hamate
تحفظوهم كيف؟ : 😅
سلمى ليلي تلعب بوكر تكسب تخسر كلو هظار
اهم حاجه تعرفوها عنهم انو ال. pisiform عباره عن sesamoid bone inside the tendon of flexor carpi ulnaris
وانو اكبر واحد فيهم هو ال capitate
وال hamate عندو hook وهو related to the deep br of ulnar n .
▪️ تاني عندنا ال metacarpal bones:
هم 5 وعبارة عن long bones بتكونوا من shaft & 2 heads
▪️اما بالنسبه لل phalanges:
14 long bones
في ال thumb عندنا 2 : proximal & distal
وال 4 الباقيين فيهم 3 : proximal, middle & distal.
🔥عندنا حاجه مهمه اسمها Carpal tunnel : it is an osseofibrous tunnel formed by carpal groove and flexor retinaculum.
♻️ transmit: - median n,flexor digitorum superficialis ,flexor digitorum profundus & flexor pollicis longus .
🔥 Carpal tunnel syndrome :also called median nerve compression, is a condition that causes numbness, tingling, or weakness in the hand at thet distribution of median nerve. also it contain motor symptoms .

▪️causes: - arthritis of the wrist and carpal joint
- dislocation or fracture
- tendosynovitis
- thickening of the synovial sheath .

الليلة حنتكلم عن
"Breast anatomy"

●طيييب هو شنووو؟🤔
It's a glandular organ located in the chest. Which has fats,connective tissue and skin

● و فيهو mammary glands  واللي هي تعتبر modified sweat glands  ودي هسي المسؤولة عن إفراز الmilk

🔶محلو ويين بالضبط؟🤔
It's extends from 2nd &6th ribs vertically 🙃
وبرضو transversely from the latral border of the sternum to the midaxillary line 😐
💢طبعا دا كلو وهو قاعد في الsuperfascial facia of the pectoral region ...  والبفصل بينو وبين الpectoral fascia
Loos connective tissue called "Retro_mammary space"


+عندو امتداد صغير كدا بمشي على جهة الaxilla  وعشان كدا بنسميهو axillary tail 🤓

🔶ارح نشوف تركيبو

●طيب أول حاجة عندنا الnipple ودا بتتجمع عندو الluctiferous duct ...
طبعا الnipple  بتحيط بيهو dark colored area called areola   
واللي هي نفسها بتحيط بيها غدد اسمها mantgomery glands 🤯  واللي بتعتبر بدورها modified sebaceous gland  بتقوم بإفراز مواد زيتية بتحافظ على الareola

●وكمان الbreast بتكون من 15_20 lobes which radiat out from the nipple
.. ولكن أهم قناة في الlobes ديل هي البتفتح في الnipple وبكون اسمها ampulla

●وفي حاجة طبعا بتفصل لينا بين ال lobes ديل وهي عبارة عن fibrous septa ممكن نسميهو suspensory ligaments 🫥

🔶Blood supply:
بكون عن طريق :
1.axillary artery
ودا بكون عن بواسطة الlatral thoracic &acromiothoracic branch

2.internal thoracic
أما دا فبكون بالperforating branches

3.Intercostal arteries
ودا فيهو الanterior &posterior

💢اما الvenous drainage بكون ماشي للaxillary vein 🫡

🔶Lymphatic drainage

بكون عن طريق :
1. Axillary lymph nodes
2.parasternal nodes
3.posterior intercostal
4.sub diaphragmatic

•ونسبة للعدد الكبير دا فعشان كدا ال breast cancer بكون انتشارو سريع

تكملة :
ناخد معلومات خفيفة كدا عن ال breast ممكن تساعدنا .
🔹glandular structure that produces milk for the baby consists of 10_15 loops each loop contains multiple lobules

🔹Modified sweat gland embryologicaly derived from the skin
لأنه ال sweat gland موجودة في ال skin

🔹Breast tissue can develop anywhere along the milk line
طيب ال milk line دي شنو؟🤔
دا بوصف لينا الاماكن الممكن تنمو فيها breast tissues وكل امراض ال breast بتحصل فيها اللي هو بادي من ال axilla to groin

🔹the terminal duct lobular unit is the functional unit of the breast
طيب دي شنو؟🤔
دي عبارة عن lobule كبيرة جواها glandular spaces صغار ديل بيصنعوا ال milk اللي هو بعد يتصنع ح يطلع للبيبي ف ح يحاج duct يطلع بيها
يعني بإختصار
It consists of lobule that contain glandular spaces that drains via duct in nipples
It has two ducts
🔪extralobular terminal duct
دي الكبيرة المتصلة مع ال lobule
🔪intralobular terminal duct
دي الجاية من ال glands الصغيرة


🔹lobules and small ducts form and are present in highest density in the upper outer quadrant.

🔹they are lined by two epithelial layer:

1\Luminal cell layer
دي الداخليه ووظيفتها صنع ال milk

2\Myoepithelial cell layer
من اسمها واضح اهه فيها عضلات يعني contractile function عشان تضغط اللبن في يوصل ال nipples
و بتستجيب لل oxytocin

🔹Breast are hormone sensitive.

يعني انه حجمها ووظيفتها mainly بتتأثر بالهرومونات اللي هي estrogen وال progesterone و prolactin عشان كدا بزيد حجمهم في حالات زيادة الهرمونات زي:
🔸after puberty
🔸during menstrual cycle
🔸during pregnancy
وهنا الهرمونات بتزيد بسبب ال corpus luteum (مرحلة من مراحل تكون البويضة لو تتذكروه في صورة ليه تحت 🌚 )في first trimester
وبسبب ال fetus, and placenta in late pregnancy

🔹زي ما انها بتزيد بسبب الهرمونات فمعناها في حالة عدم وجود هرمونات ما بتنمو(before puberty) او بحصل ليها atrophy زي في حالة
(After menopause)

♨️ Breast and milk production during pregnancy:
طيب زي ما قلنا بسبب تأثير الهرمونات ال breast tissues بتزيد لكن ما يكون في milk لأنه لسة مافي baby ف البحصل كالاتي
Milk production inhibited by estrogen and progesterone
اول ما ال baby يتولد ببدا انتاج ال milk
طيب ممكن هنا نسأل نفسنا كيف نحافظ على الانتاج دا فترة طويلة بما انه الرضاعة سنتين؟؟
الاجابة انه عن طريق
nipples simulation by the baby during breast
feeding
الحاجة دي بتزيد ال prolactin اللي هو بوقف ال menstrual cycle
و ال oxytocin اللي هو بيعمل
Duct contraction

⭕️Galactorrhea
زي ما عارفين انه ال milk production بحصل بعد الولادة
لكن اذا حصل في فترة ما فيها رضاعة ولا المرأة كانت والدة دا بنسميه glactorrhea وبتحصل بعدة اسباب طبيعية ومرضية منها:
♦️nipples simulation ( دا ال commonest physiological cause )
♦️prolactinoma of the anterior pituitary (common pathologic cause)
♦️drugs like dopamine antagonist
لأنه ال prolactin بزيد ال milk production زي ما عارفين وال dopamine بيعمل
Inhibition for prolactin
ف نقصان الدوبامين بزيد البرولاكتين😁

⭕️NOTE GLACTORRHEA DOESN'T PRESENT WITH BREAST CANCER⭕️

⭕️Gynecomastia:
هي حالة بكون فيها breast tissues in male
بسبب خلل في ال estrogen to androgene levels
ممكن تكون اسباب طبيعية زي:
🔹new born male baby
بسبب انه الاستورجين جا للبيبي عن طريق المشيمة وبتروح مع الزمن
🔹puberty in males
زي ما عارفين انه during puberty بزيد ال androgene بس المشكلة هنا انه بيتحول لي استروجين عن طريق ال fat يعنس الحاجة دي بتحصل عن الاولاد الوزنهم زايد شديد
🔹older in male
لانه مستوى التستوستيرون بقل مع العمر

وممكن تكون بأسباب مرضية زي:

🔸Liver Cirrhosis :

السبب في كدا انه ال metabolism حق الاستروجين في الكبد بقل عشان كدا بزيد عندنا
🔸klinefelter syndrome :
Cases hypogonadism

زي ما عارفين ال synovial ،joint بتسمح لل upper limb بالحرركة🦿

بنلقى كل من ال shoulder و ال elbow regions بتتكون من ثلاثة joint
و الwrist بلص ال hand متعددة ال joints🙄

اولا نشووف ال
🔰The shoulder joints 💪

بنلقى فيها
🌀 Sternoclavicular joint
💢ال articulation
بتكون بين ال sternal end of the clavical
( The manubrium sterni)
و ال first costal cartilage
💢ملاحظة
ال manubrium sterni دا ال site البرتبط فيه ال
medial end of the clavical

💢نوع ال joint
عبارة عن synovial double- plane joint

💢ال capsule
بتحيط ال joint و بتعمل attachment لل margins of the articular surface

💢ال ligaments
عبارة عن capsule بيدعم الجانب الامامي لل joint عن طريق strong Sternoclavicular ligaments

💢ال articular disc
عبارة عن flat fibrocartilaginous disc بتكون داخل ال joint وظيفتها شنو بتقسم لينا ال joint's anterior الى two compartments
medial و lateral

💢ال accessory ligaments
هي ال costoclavicular ligaments
عابرة عن strong ligaments بتمتد من ال first rib مع ال first costal cartilage الى ال inferior surface of the sternal end of the clavical

💢ال synovial membrane
موجوده في ال joint capsule و بتعمل attached لل margins of the cartilage

💢 ال nerve supply
عندنا ال supraclavicular nerve and the nerve to subclavius muscle
💢ال movements

بتحرك forward و backward عن طريق ال clavical في ال medial joint compartment
و ايضا بتتحرك Elevation و depression عن طريق ال clavical في ال lateral joint compartments

🌀 Glenohumeral joint 💪🦾
الاسم التاني ليهوا shoulder joint

💢ال Articulation
بتحدث بين ال rounded head of the humerus و ال shallow, pear- shaped glenoid cavity of the scapula
عندنا الHyaline cartilage بتخطي ال articular surface
و في عمق ال glenoid cavity بنلقى ال fibrocartilaginous rim

💢ملاحظة
ال fibrocartilaginous rim
يعرف بال glenoid labrum

💢نوع ال joint
عبارة synovial ball- socket joint

💢ال capsule
يحيط بال joint و بتعمل attachment medially to the margin of the glenoid cavity
من خارج ال labrum
و attachment laterally مع ال anatomic neck of the humerus
بيسمح بي wide range of movement
و ال fibrous slips from the tendons of thr the rotator cuff muscles strengthened the capsule

هي subscapularis
Supraspinatus
Infraspinatus
Teres minor muscles
و يعتبر اهم عضلات ال shoulder region

عندنا تلاتة ligaments
يعرف بي ال glenohumeral ligaments
و transverse humeral ligament
و coracohumeral ligament

💢ال Accessory ligament
هي ال coracoacromial ligament
بتمتد من ال coracoid process الى acromial
وظيفتها بتحمي ال superior aspect of the joint

💢ال synovial membrane
بتكون في ال capsule و بحصل ليها attached مع ال margin of cartilage المخطي ال articular surface
بكون لينا tubular sheath حول ال tendon of long the long head of the biceps brachii

💢 ال Nerve supply
Axillary and suprascapular nerves

💢ال movements
ال shoulder joint has a wide range of movement

الطبيعي 90
بيحدث عن طريق ال anterior fibers of
⚜ Deltoid muscles
⚜ Pectoralis major
⚜ Biceps
⚜ Coracobrachialis muscles

♏️ Extension
و ال normal 45
و بتحدث عن طريق ال posterior fibers of the
⚜ Deltoid
⚜ Latissmus dorsi
⚜ Teres major muscles

♏️ Abduction

Abduction of upper limb بيحدث
في ال shoulder joint و بين ال scapula و thoracic wall
⚜ Supraspinatus ( initiate)
⚜ Deltoid muscles (middle fibers)

♏️ Adduction
ال normal ٤٥ swung 45 across the front of the chest
⚜ Pectoralis major
⚜ Latissmus dorsi
⚜ Teres major
⚜ Teres minor

♏️ Lateral rotation
ال normal lateral rotation is 40 to 45
⚜ Infraspinatus
⚜ Teres minor
⚜ Deltoid ( posterior fibers)

♏️ Medial rotation
Normal rotation is 55

⚜ Subscapularis
⚜ Latissmus dorsi
⚜ Teres major
⚜ Deltoid ( anterior fibers)

♏️ Circumduction
عبارة عن
Combination of the above movement

1_subclavin artery 🩸🩸
ودا جزء من aorta
البطلع من heart مباشرة
عموما عندنا
Branches of aortic :
A_left subclavin
B_left common cartiod
C_brachiocephalic
ودا برضو فيه منو قسمين
1_right subclavin
2_right common cartiod

بالنسبة لل subclavin artery
بنقسم قسمين 🧐
1_The right artery originates from the brachiocephalic artery.

2_The left artery originates from the arch of the aorta

وكدا انتهينا من اول artery 💪🔥

2_Axillary artery 🩸🩸
ودا طبعا سمعنا عنو قبل مرة دي 🤔
في content of axilla

على فكرة دا نفسوا subclavian artery
بس عشان حقوق الملكية 😉
دخل axilla يبقي اسمو axillary artery
نشوف ببدا وين وبنتهي وين 💪
📍Begins at the lateral border of the 1st rib as continuation of the subclavian artery.

📍End at lower border of teres major

📍Is crossed anteriorly by the pectoralis minor muscle, and is divided into three parts; 1st, 2nd
& 3rd.


نبدأ بتقسيمات axillary artery 🩸 🩸
📍1_The 1st part of the axillary artery
نشوف بداية وين ونهايته
Extends from the lateral border of 1st rib to upper border of the pectoralis minor
بتفرع تفرع واحد بس
one branch: Highest thoracic artery

وبكدا خلصنا اول جزء قربنا نخلص، 💪💪

📍The 2nd part of the axillary artery
نشوف موجود وين
behind the pectoralis minor muscle.
ودا بتفرع تفرعين
Two branches: Thoracoacromial
Lateral thoracic.

باقي لينا اخر تفرع 😅
📍The 3rd part of the axillary artery
نشوف بداية ونهاية وين
Extends from the lower border of pectoralis minor muscle to the lower border teres major muscle.
ودا بتفرع الي ثلاثة
Gives three Branches: 1_Subscabular

2_Anterior circumflex humeral
3_Posterior circumflex humeral.

A_Brachial artery
اول شي نمسك اصل الشريان
📍 Origin of the brachial artery :
Continuation of axillary artery ( from subclavian artery )

وبعدين نمسك مسار الشريان
📍Course of the artery
A_ From Lower border of teres major

B_into laterally posterior to median nerve in front of arm

C_end their at the level of neck of radius

D_ Terminates by dividing in to 2 >>> Radial and ulnar arteries

نمسك اخر شي فيه وتفرعات🤠
📍Branches :
A_ Muscular branches : B_profunda brachii artery
C_ superior ulnar collateral artery
D_ nutrient humeral artery
E_ inferior ulnar collateral artery
F_ radial artery
G_ ulnar artery
**Anastamoses : around elbow joint
فيه ملحوظة مهمة دا شريان بنقيس منو 😁
Blood pressure

وكدا خلصنا الشريان الابوي لل upper limb🤝🤝

نبدأ بالاولاد شريان الابوي
4_ulna artery🩸🩸
بنفس الطريقة الي اتبعناها فوق👆 نفس الشي هنتبعها هنا
📍Origin of the artery
From brachial artery (larger branch )

📍Course of the artery
A_From cubital fossa
B_Runs oblique downwards and medially ( in upper 1/3 of forearm)
C_Runs vertically downwards ( in lower 2/3 of forearm )
D_ Runs superficial to flexor retinaculum(Carpal tunnal)
E_ finally Enters palm f_Terminates by dividing in to 2 >>> Superficial and deep palmar branches

📍Branches of ulner artery :
A_Superficial palmar branch : forms superficial palmar arch
ودي عبارة عن
channel between ulnar & radial artery

B_ Deep palmar branch : forms deep palmar arch

والحمد لله انتهينا من اول ولد لشريان الابوي 😉

📍Origin of the artery From brachial artery ( smaller branch )
📍Course of the artery
A_From Neck of the radius

B_ Enters anatomical snuff box posteriorly ( deep to tendons of abductor pollicis longus , extensor pollicis brevis , extensor pollicis longus & superficial to lateral ligament wrist joint ) >>> Reach proximal end of 1st interosseous space

C_Passes between 2 heads of fisrt dorsal interosseous muscle & 2 heads of adductor pollicis longus >>> Forms deep palmar arch

📍Branches
A_ Radial recurrent artery

B_muscular branches

C_palmar carpal branch

D_dorsal carpal branch

E_superficial palmar branch

F_dorsum of hand : Lateral branch to thumb , 1st dorsal metacarpal artery

H-palm of hand :
Princeps pollicis artery
radialis indicis artery
