اتذكرو أنو ال final lymphatic drained of the tongue by > jugulo _omohyoid 🫡
🍁 the palat :-
دا المكون الفاصل بين ال oral cavity and nasal cavity 🤥وهو جزئين

(1)- 🦋hard palate :-
ودي بتتكون من:
1) palatine process of maxilla > form the anterior 3/4 of hard palate

2) horizontal plate of platin bone > form the posterior 1/4 of hard palate

◇there's incisive formen giving passage for nasopalatine nerve and grater palatine vessels😶

🪢blood supply of hard palate :-
Grater palatine artery from 3rd part of maxillary artery.

🪢venous drainage:
Pterygoid venous plexus
بكل بساطة 😌
⚡️nerve supply:-
-Anterior part > naso palatine nerve
- posterior part > grater palatine nerve
والإتنين ديل يعتبرو branches from [ pterygopalatine ganglia ]

(2)- 🦋soft palate:
○ودي بتعتبر متحركة
movable muscular part of palat
○بتمتد من posterior border of the hard palate.
○ وكمان بتفصل لي nasopharynx منoropharynx

💪Muscles of soft palate:-
(صغيرة وعندها عضلات 😂 )
〰1- tensor palati >

Origin :From scaphoid fossa
Insertion : tend wind around pteyrgoid hamulus
Action : tension of soft palate
يعني بتعمل على شدها
〰2-levator palati>

From> lower surface of petrous temporal bone
To > upper surface of palatine aponeurosis
It elevate soft palate upward and backwards during deglutition
ودي مهمة في عملية البلع شدييييد

〰3- palato pharyngus:

-Originate from upper surface of palatine aponeurosis
-inserted into posterior porder of thyroid cartilage
- elevate pharynx and larynx to close laryngeal inlet during deglutition
برضو مهمة في البلع🙄

〰4- palatoglossus:

-From Lower surface of palatine aponeurosis
- to side of the tongue
- elevat posterior part of the tongue

⚡️Nerve supply of muscles of soft palate:
-motor > all muscles by vagus through pharyngeal plexus except [tensor veli palatini by the mandibular branch of the trigeminal nerve]
- sensory > grater , lesser nasopalatine branches of maxillary
- test > pass through pterygopalatine ganglia to grater petrosal nerve

الليلة حنتكلم عن organ مهم شديييد ومعروف عند بعض "بعض" الناس بالثقب الأسود😂😂/أو التلاجة 😭😂
أي أي الليلة حنتكلم عنها شخصيا "Stomach "🔥🔥🔥

أرح نتعرف عليها شوية🤓
○ال Stomach تعتبر هي ال dilated portion من القناة الهضمية.

وحنلاقى عندها 3وظائف رئيسية واللي بتشمل:

(1).بتعمل لينا تخزين للأكل Stores food..
في ال adult عندها سعة [capacity] حول ال 1500 ml يعني لتر ونص كدا😳
(غايتو البحصل في السحور والإفطار حق رمضان بفوت الرقم دا بمراحل😭😂).

(2).بتعمل لينا mix للأكل مع ال gastric secretions ودا عشان تكون لينا semifluid chyme.(ودا عشان الأكل يكمل رحلتو البتضمن بقية عمليات الهضم والامتصاص)🫠

(3).بتعمل لينا control لمعدل نقل ال chyme القبل شوية دي لل small intestine .🏂

تعال معاي نشوف موقعها 📍
🔶Location
بنلقى ال stomach قاعدة في الجزء العلوي من ال abdomen. وبتمتد من ال left costal margin region لحد ال epigasteric region .

والجزء الأكبر منها بكون مغطى تحت ال lower ribs.🧑‍🦯 ودا بهد الحماية طبعا😏

🔶Description
شكلها بشبه حرف ال J_ shape .
○وبنلقى فيها فتحتين [two opening]

١.واحدة بتربطها مع ال esophagus وإسمها cardiac orifice(سموهو كدا لأنو قريب من القلب🫀ودي حتفيدنا في التشخيص المرضي بعدين)

٢.والتانية بتربطها بال small intestine وإسمها pyloric orifice

○وطبعا بما أنها في شكل J بكون عندها منحيين [two curvatures] واحد بكون صغير والتاني كبير [lesser &greater]👶👨‍🦳

○وعندها two surfaces واحد anterior والتاني posterior.

ومع كلللل الكلام دا إلا أنو شكلها بختلف من زول لزول (قصير طويل .. سمين ضعيف.. صغير كبير) ياخ بتخلف في الزول زاتو إتخيلو 😅 (لما تكون مليانة ما زي لما تكون فاضية) .. وحتى أثناء عملية ال respiration وحتى مع وضع الجسم.

وضحت؟😉يلا نشوف بقسموها كيف👀

○ال Stomach بتتقسم إلى الاجزاء الآتية:

•Fundus:
ودا بكون dome shape زي شكل القبة كدا 🕌
وبكون في الجزء العلوي وبكون قريب من ال cardiac orifice وعادة بكون مليان gas🌫

•Body:
ودا بمتد لينا من ال cardiac orifice لحد ما نصل لمستوى حاجة إسمها incisura angularis (ودي بتكون زي طفجة في الجزء السفلي من ال lesser curvature .. اتذكرو ال J shape)😄

•Antrum:
ودا زول بسيييط بمتد لينا الincisura angularis إلى الpylorus.

•Pylorus:
ودا الجزء الأخير وممكن نقول أنو أكتر جزو tubular وبنلقى فيهو عضلات كثيفة [thick muscular wall] عشان تعمل لينا ال pyloric sphincter. 🧣

○ ال pyloric sphincter دا بلعب دور مهم شديد في ال control لمرور ال gastric contents لل duodenum.🫸

_وبنلقى motor fiber من ال sympathetic system وال inhibitory fibers من ال vagi (عشان كدا لما تكون مروق ومستكين بتقدر تهضم كويس)🦭


○وبما أنها Stomach وعندها عصارات وإفرازات فبكون فيها mucous membrane عشان يحميها🛡

وبما أنو بكون فيها إمتصاص للمواد الغذائية فبكون فيها vascular

وكمان حنلقى في طفجات كتييرة [numerous folds] وبنسميها rugae ( أي روقي عدييل كدا 😂😜) ودي بتساعد ال stomach في التمدد وهي في الغالب longitudinal in direction.

○ حنلقى أنو ال stomach عندها 3 muscular walls ودا أكتر من باقي أجزاء ال GIT ودا بسبب وظيفتها👩‍🔧.. واللي هم
_ longitudinal _circular_ oblique

○وطبعا بحيط بيها كلها ال visceral peritoneum

مستمتعين🤩؟ ..يلا نكمل🏃
🔴🪢 Arteries:
يلا ديل كللهم بنقسمو لينا من ال celiac artery واللي هو جايينا من ال aorta. يلا نشوف تفاصيلهم 👀
1.left gastric artery:

ودا بطلع لينا من ال celiac artery مباشرة ودا بطلع فوق [upward] وبلف يسار لحد ما يصل لل esophagus وتاني بنزل تحت عل إمتداد ال lesser curvature
ودا بغذي لينا ال lower third of esophagus ومعاهو ال right upper part من ال stomach.↗️

2.right gastric artery:

ودا بطلع لينا من الhepatic artery ودا بعمل supply لل الجزء اليمين تحت [lower right part] من ال stomach↘️.

3.short gastric arteries:
وديل بطلعو من ال splenic artery وبغذو لينا ال Fundus 🕌.

4.left gastroepiploic artery:
ودا بطلع من ال splenic artery برضو لكن بغذي لي ال upper part of greater curvature⬆️.

5.right gastroepiploic artery:
ودا بطلع لينا من gstroduodenal واللي هو تفرع من ال hepatic artery ودورو أنو بغذي لينا ال lower part of greater curvature⬇️.

كدا خلصت 😮‍💨.. أرح نشوف ال veins سريع سريع

🔵🪢Veins:
ودي كلهم بحصل ليهم drainage في ال portal circulation

يلا الlymph vessels بتكون ماشة مع ال arteries لحد ما يصلو لينا لل right gastric nodes و ال right & left gastroepiploic nodes وال short gastric nodes ديل كلللهم بصبو لينا في ال celiac nodes.🫥

⚡️Nerve supply:

🔸 الstomach عبارة عن عضو على شكل J في الجهاز الهضمي بيكون reservoir للfood 🍔 و عندو وظائف في الهضم

●بيتقسم ل٤ اقسام
١\ cardia
وده المكان العبروا تتفرغ محتويات المرئ إلي المعده.

منطقة الانحناء العلوي للمعده
٣\ body
المنطقة الرئيسية للمعده
٤\pylorus or pyloric antrum
الجزء الأسفل وده البسهل إفراغ محتويات المعده

●يلا زي باقي الgit , الwall حق الstomach بيتكون من mucosa و submucosa و muscular layer و serosa

والmuscular layer ذاتها بتنقسم ل

●الmucosa حقت الstomach بتحتوي على exocrine glands كثيييرة اللي هي gastric glands يعني غدد
بتفتح في فتحات معينة بيسموها gastric pit🤏

💫Types of cells in a gastric gland

1\ mucus cells
بتكون في عنق الغدد في ال
cardiac and pyloric regions of the stomach )
طبعاً من إسمها بتفرز لينا الmucus

2\parietal cells (oxyntic cells)
At the isthums and body of the gastric gland
ودي بتوجد في ال body and fundus of the stomach
ووظيفتها تفرز
HCL and intrinsic factor

3\chief cells (peptic zymogen )
دي موجودين مع ال parietal بنفس المكان، عرفتوا مكانها صااااح،ما تكونوا ماشين سااااي😒بتفرز ال pepsinogens

●بالاضافة لديل عندنا خلايا ال endocrine cells بتكون جنب الgastric glands
امثلة ليها
G cells
بتفرز لينا ال gastrin
ECL cells (enterochromaffin like cells ) :
تفرز الهستامين.

طيب الوظائف حقت الstomach شنووو 🤔🤔🤔
1\storage of food 🍔
بتخزن الاكل يمكن لحد ٤ ساعات وبتتحكم في خروجه للduodenum .
2\digestion of proteins and lipids
عن طريق انزيمات وهي ال gastric lipase و الpepsin
3\protection
بتحمينا عن طريق الHCL اللي بيقتل البكتريا
و الvomiting البيطلع المواد الضارة

4\synthesis of intrinsic factor
اللي هو glycoprotein بيرتبط بفيتامين b12 وبيساعد في امتصاصه
في الterminal ileum

5\facilitates absorption of iron in the intestine
بالحفاظ على الحديد في ال ferrous state
6\absorption
مين اللي قال انه الintestine وحدها اللي بتمتص 😅
حتى الstomach عندها وظائف زي كدا وبتمتص كميات صغيرة من الwater و الions و الalcohol وبعض الdrugs

7\endocrine functions
بتساعد في افراز هرمونات كثيرة زي الgastrin و الvip و glucagon و somatostatin و ghrelin

💡معلومة 💡
الghrelin دا هو الgrowth hormone releasing hormone
يعني الهرمون البيخلي هرمون النمو يتفرز 👶👦👨👴

والghrelin دا بيتفرز في الhypothalamus
وكمان من fundus of the stomach وبيدي الشعور بالجوع hunger😾
عكس الleptin. البيدي الشعور بالشبع satiety😸

طيب المرئ دا مش لمن دخل القفص الصدري قبييل كان ماشي بي جهه الشمال ...يلا لمن يصل T5حايرجع يمشي بالنص تاني(midline)وتاني لمن يصل T7تاني حاينحرف ناحيه الشمال وحايمر لينا خلال الحجاب الحاجز (at the level of the 7 left costal cartilage......ما thoracic vertebra...ماتلخبطو 😊)..

طبعا دا بسبب انو ال ribsبتكون في مستوى ادني من الvertebrae 😊👌
Blood supply

from descending thoracic aortaشخصيا🤩 and bronchial arteries.

🔵🪢venous return
to the azygos system of veins .

🟢🪢 lymph drainage
tracheobronchial and superior and posterior mediastinal nodes..

بتختلف في الطول حسب امتداد المعده🤨

بسم الله نبدأ :
○أولا ال GIT او gastrointestinal tract من oral cavity ولحدي ال anus كل عضو عنده هستولوجي ممكن نتكلم عنه لوحده بس في general plan مشتركة بينهم كلهم

طيب نتكلم عن ال General plan

- تبدا من وين هي ؟
بتدأ من ال esophagus المرئ لحد ال anal canal وفيها اختلافات طبعا بحسب ال mucosa coat .
- كلام جميل اها وتاني 🌯
- لالا شكلكم جوعانين 😆
- تمو ما عليكم بينا 😑
المهم :
لمن اشوف ال GIT بلقاها مكونة من Four distinct coats و دا ترتيبهم من الداخل الى الخارج🦺 :-

🔸1. MUCOSA

: وهي كمان تحتوي على ثلاثة طبقات :
(A) Epithelium .
(B) Lamina propria – made of connective tissue containing glands and lymphoid accumulations.
(C) Muscularis mucosa – made of smooth muscle fi bres; arranged in two layers, the inner circular and the outer
longitudinal. This layer is responsible for movement and folding of mucosa.

🔸2.Submucosa :

○ Consists of fibroelastic connective tissue.
○ Contains Meissner’s nerve plexus.
(ودا مهم في الفيسيولوجي🤓)
○ May contain glands (oesophagus and duodenum) .

🔸3. Muscularis externa :

عضلات عضلات💪💪
○ Composed of two layers of smooth muscle, the inner circular and the outer longitudinal.
○ Muscularis externa is responsible for peristaltic contractions.
○ In the oesophagus skeletal muscle is present in the upper part.
○Contains Auerbach’s nerve plexus (myenteric) and parasympathetic ganglia between the two layers of muscle.

🔸4. Adventitia/Serosa :
وهنا دا بعتمد على موقع العضو من ال protenuim 🫥

○ Adventitia consists of only loose connective tissue without peritoneum.
○ Serosa consists of peritoneum (mesothelial lining) over a layer of loose connective tissue.

بالعربي كدا "الأمعاء الدقيقة"
أرح نتعرف عليها أكتر 🏃🧐

●هي تعتبر أطول جزء في الGIT وبتمتد لينا من ال pylorus of the stomach لحد ما نصل لل ileocecal junction (ودا بربطها بالأمعاء الغليظة🥸)

●و الجزو الأكبر من ال digestion & absorption بكون فيها 🥶

●ال small intestine مقسم ل ٣ أجزاء واللي هم :
(1). Doudenum
(2). Jejunum
(3). Ileum

يلا نخش عليهم بالتفصيل:🔥

🔶(1). Doudenum:
الدودو جااا😭😂(دا كائن خرافي بشبه الأسد موجود في الثقافة السودانية) ...
طبعا إسمو المتعارف عليهو بالعربي هو "الإثنا عشر" واتسمى كدا لأنو لما قاسو طولو بالأصابع سوا ١٢ أصبع👆
_وهو بيعتبر أكتر جزئية fixed و widest  مقارنة بأخوانو

○هو بياخد لينا شكل حرف ال C وطولو حوالي (inch10 (25 cm .. وهو بصل لينا بين ال stomach و ال jejunum .

○ فيهو حاجة مهمة شديييد😲واللي هي أنو بتفتح لينا فيهو كل من ال pencreatic & bile ducts.

○يلا ال mucous membrane حقو بكون thick .

○في بدايتو بكون smooth لكن تاني ببقى فيهو circular folds بنسميها plicae circulares

وبتقسم 4 parts واللأجزاء دي:

▫️1st part

○أول 1inch (2.5cm) بتكون مغطية بال protenuim والجزئية دي بنسميها ال duodenal cap🧢 اما الباقي يعتبر retroperitoneal
○ببدا لينا بال pylorus ومشي upward &backward ودا على مستوى ال L1
○يلا نشوف علاقتو مع باقي الstructures👀
•Anteriorly:
_في قدامو ال quadrate lobe of the liver وال gallbladder
• Posteriorly:
_وراهو في ال lesser sac _gstroduodenal artery _bile duct_portal vein_inferior vena cava

• Superiorly:
Epiploic foramen

• Inferiorly:
تحتو حنلقى ال head of pencreas

(عارف الكلام دا بتحسو بيهو كتير كنص لكن لو شفتو الصورة حتوضح ليكم كتيييير والموضوع ببقى أسهل😉)

▫️2nd part

○ودا نازل بشكل عمودي [vertically ] لحد ما يصل للhelium of right kedney . كمان حنلقى أنو جاي عل مستوى ال L2&L3

○ يلا هنا في الجزء دا بتحصل حاجة مهمة شديييد(ذكرناها قبيل🙄)
_ انو ال bile duct ومعاها ال pencreatic duct بتلمو وبعملو حاجة إسمها ampulla ودي بتفتح في ال duodenum بحاجة إسمها major doudenal papilla👨‍🦰.
_وبما أنو في major فمعناها في minor doudenal papilla 🧒 ودي بتعملها لينا ال accessory pancreatic duct.

وضح ؟🌚🔥... يلا نشوف الrelations حقتو

• Anteriorly:
بكون عندنا ال Fundus حق ال gallbladder + ال right lob of the liver
ومعاهم ال transverse colon +ال coils حقت الsmall intestines😐

• Posteriorly:
بكون فيها ال hilum of the right kedney وأكيد معاها ال righ ureter.

• Laterally:
هنا بكون في ال Ascending colon +colic flexure وحيلاقينا تاني ال right lobe of the liver (الزول دا مالو كتير كدا😂😭)

• Medially:
وديل تلاتة head of pencreas..bile duct ..main pencreatic duct

■ 3rd part

○ودا بمشي بشكل أفقي[horizontally] في جهة ال left .

وال relations حقتو هي😃
• Anteriorly:
حنلقى فيهو ال root of mesentery وتاني ال superior mesenteric vessels بالإضافة لل coils of jejunum

• Posteriorly:
حنلقى هنا ال right ureter + psoas muscle بالإضافة للعظيمين😎ال aorta &IVC
• Superiorly:
head of pencreas

• Inferiorly:
coils of jejunum.

باقي لينا آخر جزو من ال Duodenum👏🔥

■ 4th part

وال relations حقتو هي:
• Anteriorly:
حنلقى فيها بداية ال root of mesentery +ال coils of jejunum

• Posteriorly:
حنلقى ال left margin of aorta بالاضافة ل ال left psoas muscle

تمت🥳😭..(بالنسبة لل relations زي ما قلت ليك لما تشوفها من الصورة وتقراها حتفظها بسهولة أكبر 🫡)

أرح نشوف ال Blood supply 🩸

🔴🪢Arterial:
ودا بكون بإتنين
_ superior pancreaticduodenal artery
ودا جاينا كتفرع من ال gstroduodenal artery ودا بعمل supply للنص الفوق⬆️

- inferior pancreaticoduodenal artery
ودا جاي كتفرع من ال superior mesenteric artery وبعمل supply للنص التحت ⬇️

🔵🪢Venous:
فيهو superior pancreaticduodenal vein ودا بصب لينا في ال portal vein

أما بالنسبة لل inferior فدا بتحد مع ال superior mesenteric vein

بإختصااار كدا الفوق كلهم بصبو في ال celiac nodes أما التحت بصبو في ال superior mesenteric nodes

⚡️Nerve supply:
ال sympathetic&parasympathetic(vagus)ديل بجو من ال celiac &superior mesenteric plexuses

🔶(2). Jejunum

○ودا لما نجمعو مع أخوهو ال ileum طولهم بعادل 6أمتار😱 و صاحبنا ال jejunum بعادل الخُمسين 2/5 الفوق [upper two fifths].

○وطبعا ببدا لينا من ال duodenojejunal flexure

○يلا ال coils حقتو بتكون freely mobile وبتكون متصلة بال posterior abdomenal wall بي حاجة بنسميها mesentery of small intestine😴

○يلا دا طولو بعد تلاتة أخماس ال 6 متر القبيل ديك وبنلقاهو قاعد في الجزئية التحت [lower 3 fifths ].

○نهايتو بتكون في الileocecal junction

_اال jejunum بكون قاعد في الupper right.
بينما ال ileum بكون قاعد في ال lower left .

_ال jejunum بكون wider and thicker مقارنة بال ileum ودا عشان ال plicae circulares فيهو كبيرة ومتقاربة🙌

_في ال jejunal mesenteric vessels بعملو لينا only one or two arcades
(ال arcade يعتبر زي anastomosis في متتالي كدا وحنوضحو في الصور التحت👇)وبطلع منها long branches بنسميها vasa recta .. بخلاف ال ileum البتكون عندو ال arcades كتيرة وال vasa rectaحقتها short .

بالنسبة للfat البتكون مترسبة بتكون بعيدة من ال walls في حالة ال jejunum وملصقة مع ال wall في حالة ال ileum.

الiluem بتميز بوجود lymphoid tissue إسمها Peyer patches في الwalls حقتو.

ودا بكون بال superior mesenteric artery ودا بعمل intestinal branches بتعمل حركة anastomosis(ما حاسين أنكم عرفتو دا منو🥸) ودا عشان تعمل لينا الarcades 😜ودي بتعمل لينا ال vasa recta.
_وطبعا ما ننسى ال ileocolic artery البعمل لينا supply للجزء التحت من ال ileum.

وديل بتجمعو عشان يصبو لينا في ال superior mesenteric vein

🟢🪢 Lymph drainage:
وديل كلهم بصبو لينا في ال superior mesenteric nodes
