💢Salivary Glands 💢

نبدأ بتعريفها:-
💢What are salivary glands?
The salivary glands are exocrine glands found in mouth produces saliva,
أو
It site between ramius of mandible and sternomastoid muscle below External auditory meatus .
💢boundaries of Salivary Glands
من فوق وتحت بتكون عبارة عن عضلات
Upward 👉zygomatic arch
Down wards 👉 angle of the mandible.
أما من الامام والخلف بتكون عبارة عن عظام
Anterior 👉
Overlap masseter muscle
Posterior 👉 overlap sternomastoid muscle
أما شكل ال salivary glands فبختلف حسب نظري جاي من أي جانب كما سنرى بعد قليل 👇👇

ودي أكبر أنواع ال salivary glands.
💫submandibular Glands
ديل تحت الفك.. برضو يمين و شمال
💫sublingual
تحت اللسان عشان كده سموه كده

الsaliva أو اللعاب الناتج بيصل عن طريقducts و بيحتوي على mucus, amylase and lysozome

✴️وظيفته : يبلل الأكل و يعمل ليهو lubrication و dissolving ، و يحافظ على رطوبة الmucous membranes

💫ال saliva زاتها بتنقسم إلي نوعين
1\ serous
2/mucous
1-parotid glands (serous).
2-Submandibular glands (mixed)
3-Sublingual (mucous)
💢طيب إحنا بنعرف ال salivary glands عن باقي ال glands كيف؟؟؟؟?
1-each gland is surrounded by connective tissue capsule.
2-devided into lobules.
3-each gland has secretary piece and duct system.
4-the secretary end piece has serous or mucous cells in addition to myoepithelial cells.

💢What are charcters of serous cells?
1-pyramidal.
2-rounded nuclei.
3-narrow leumen with basophilic staining.
4-serous cells form acini.
💢What are charcters of mucous cells?
1-pale.
2-cuboidal to columnar.
3-nuclei flattened.
4-wide lumen.
5-mucous cells form tubules.
💢What is serous demilune?
Is mucous tubule capped with serous acinus.

💢parotid Glands
طبعاً قلنا إنها أكبر salivary glands لكن كمية الsaliva البتفرزها 25% بس😥🥺
فصلنا فستان ال parotid Glands في نقاط سمحه ،حظها حششت للعيد 🤣😅😂
1-serous glands.
2-covered with capsule which is part of deep facia.
3-at the side of mandible.
💢تتذكروا قبيل قلنا إنو الgland بتجي من duct ؟؟؟طيب ال parotid duct موقعه وين من الإعراب؟؟
1-the duct penetrates through buccinator muscles.
2-it's palpable over masseter muscle.
3-it oppens into vestibule against upper second molar teeth.
-it extends from zygmotic arch to the upper part of the neck.
-overlap the masseter muscle anteriorly.
-in transverse section it occupies the gab between mandible, mastoid and styloid processes.
4-the capsule is supplied by great auricular nerve.
زي ما عارفين إنو أي glands عندها سطوح 🏢🏣🌃🌆 فحانتناول السطوح بتاعت ال parotid Glands

1-superficial surface :-
🔸related to the skin and facia.
2-anteriomedial surface :-
🔸grooved by posterior border of mandible.
🔸related to the masseter and medial ptegroid muscles.
🔸The parotid duct and branches of facial nerve emerge from anteriomedial surface also terminal branches of external carotid artery(ECA).
3-Posteriomedial surfaces :-
🔸contacts to styloid and mastoid processes.
🔸ECA enters the gland through this surface.

Innervation of parotid Glands

🔸Arteries:- external carotid artery and its branches within gland.
🔸Veins :- retromandibular vein.

Facial nerve passes through it

1\superficial part وهو الجزء الرئيسي شكلها بشبه اللوزة🥔🥔و

💢superficial surface:-
بتتكون من:-
_Cervical Fascia
_Cervical branches of facial nerve
_anterior facial vein
💢medial surface:_
بتضم لينا عضلات واوعية دموية ومفاصل 👇👇👇
Mylohyiod muscle
Hyoglossus muscle
Intermediate tendon of digastric

💢Lateral surface
لو نظرنا لل lateral surface of Submandibular glands بنلقى فيها Mylohyiod line وطالما هو خط معناها بكون فاصل بين حاجتين Submandibular fossa and sublingual fossa
وبرضو ال facial artery بجي ماري في النص بين ال mandible and Submandibular glands
بالإضافة لاحتواها على ال Mylohyiod vessels and nerve.
2/deep part
موجوده بين 2muscles2 nerve
Muscles:-Mylohyiod and hyoglossus Muscles
Nerves:-lingual nerve above and hypoglossus below.
موقع ال submandibular بالضبط 🌏في ال digastric triangle وشكلها Almond.

It's parasympathatic gangilion🧠
بتقع في ال lateral surface of hyoglossus muscle.
💫ال root حقتها بتكون preganglionic fiber أما برانشاته🌿 بتكون postganglionic fiber .
💫مزايا ال Submandibular glands
-it's duct oppens into oral cavity proper.
-the duct passes forward to oppen on small sublingual papilla beside the base of frenulum,

Nerve supply
Artery 👉submental branches of facial artery.
Vein 👉anterior facial vein
Lymph node 👉facial artery node,deep cervical and jugular node.

💢sublingual glands

-mucous glands located below the tongue.
وهي أصغر قلاند في الساليفري قلاند.
-each is almond shaped and is immediately lateral to Submandibular duct.
-associated to lingual nerve in floor of oral cavity.
💫Sublingual and Submandibular are supplied by branches of facial and lingual arteries.
-lies against medial surface of the mandible and forms shallow groove(Sublingual fossa) supperior to the anterior 1/3 of mylohyoid line.

Blood supply 💦

Artery 👉sublingual branches of lingual artery and submental branches of facial artery

Nerve supply 👉sensory supply from lingual nerve
Sympathetic and parasympathatic supply from Submandibular gangilion.

ومن اسمها معناها مهمه في الطقه🍔🍕🤤 🥨

              يلا  بسم الله نبدا 🫡🔥

🛑اولا هم muscles 4⃣موجدين علي جانبي ال 💀skull :
1-masseter
2- temporalis
3- medial pterygoid
4-latral pterygoid

🛑ثانياً:-
عندهم حاجات مشتركه😻🤝 هي:

1-they take origin from skull.
2-inserted into mandible .
3-they act on temporomandibular joint .
○كسره
-ثواني  ثواني 🤌ماشين وين🚶🤚 نتكلم شويه عن ال joint ده:

-اولا ☝️هو قاعد بين articular tubercle و anterior portion of the
mandibular fossa
-ثانيا✌️ نوعه synovial joint

4-they supply by mandibular nerve

       كده خلصنا الحاجات المشتركه.

🛑ثالثاً:-
Movement of mandible🌪:

_elevation ⬆️and depression⬇️.
_protraction ➡️and retraction⬅️ .
_side by side movements⏭⏮.

✨بعد كده هنمسكهم muscle muscle هنعرف ال originوال insertionو ال action ي إلهي يعني ذي locomotor😳 لا لا ياخ ده حلو شويه😍😂

💢1/Masseter muscle :-

-✨ origin : lower border and inner aspect of zygomatic .

- ✨insertion: outer surface of ramus arch of mandible .

- ✨action:
1- elevation of mandible.                                                 (.    anti-gravity muscle
هي عباره عن عضلات معينه ذي الموجوده في الرقبه بتكون مشدوده طول م الشخص مستيقظ😎 و تترتخي أثناء النوم😴)
2- protrusion ➡️of the mandible.

- ✨insertion : medial surface of ramus above the angle.

- ✨action:
1- elevation ⬆️of mandible.
2- protrusion ➡️of mandible.
3- deviations to on sides ↔️


💢4/lateral pterygoid :-

⭐️دي اخت 👭ال  medial pterygoid
برضو عندهاupper and lower2 head

- ✨origin:
A- upper head ( infratemporal crest
B- lower head ( lateral surface of lateral pterygoid plate)

-✨ insertion: pterygoid fova.

-✨action:
1- depression⬇️ of mandible.
2- protrusion ➡️of mandible.
3- deviations ↔️to one side .

      قالو كل ال  muscles supplied by anterior division of mandibular nerve

يلا بسم الله ح نتكلم عن ال oral cavity🤗

○ال oral cavity محيطه بيه من برا ال :
-Lips (anteriorly)
-Checks( laterally)
طيب يلا نشوف👀 :

1) lips👄:-

2 fleshed fold surrounding the oral orifice they accommodate obtcularies Oris muscles

. يعني ال muscles of lips [ obtcularies Oris]😱

●طيب يلا ال nerve supply😖:
_The upper lip by infraorbital nerve
_the lower lip by mental nerve

●وال blood supply يلا دا سمح😁:
The upper lip by: superior lipial artery
Lower lip : inferior lipial artery

●يلا ال lymphatic :
The 2 lips drained in submandibular lymph node عدا the center of lower lip drained in submental lymph node🫠


2) the cheeks ( 😆الجضوم)

-cheeks are continue with lateral sid of nose and lips at [nasolapial groove]
-their muscles is [buccinator muscle ]
-They are lined internally by >mucous membrane
-And coverd externally by >skin >buccal bad of fat > buccophayngeal fascia

●Nerve supply:
-buccinator muscle supply by [facial nerve]
-Skin of cheeks supply by buccal branch of mandibular nerve
⚡️-OK then then oral cavity from inside":
بتقسم لي 2 :
🍂1)vestibule:

it is the space between teeth and cheeks
- it contains :
1-parotid duct
2- superior lapial frenulum (الخط الفي النص بين ال upper gums and upper lip)
3- inferior lapial frenulum
4- fornix ( reflection of mucus membrane from gums to lip)

🍂2) oral cavity proper:-

It is the space laying inside teeth and gums
يلا بتكون من :
1-Tongue
2-Palate)> form the roof of oral cavity
3- teeth
نبدأ بال
🍁Tongue 👅
○يلا هو عبارة عن muscular organ بنلقاهو مغطى ب mucous membrane .

○وظايفه هي: test ,deglutition,speech

○حنلقى أنو بتقسم ل:
apex ,root, body, 2 surfaces ( ventral , dorsal ) 😳

1) the apex; free and directed anteriorly
2) the root; attached to mandible and hyoid bone
3) ventral surface;-
-It smooth it show the following:-
1- frenulum lingue :"
Formed by mucosa in median plane بين ال tongue وال floor of the month
2- deep lingual vein :"
(يلا دا عندو أهمية في ال Sublingual administration "الأدوية البتتخاد تحت اللسان) 😋
On either side of frenulum lingue
3- fimbriated fold :
Lateral to deep lingual vein اسمها [plica fimbriate] 😱
4) dorsal surface :-
عندنا ال V- shaped sulcus terminals
أي بتشبه حرف الVعدييل 😝
بتقسمو لينا علي اتنين :
○- '"anterior 2/3( oral part ):-
It's thick and rough بسبب وجود ال papilae وعندنا ٤ أنواع من ال papilae
- valiate papilae , faliform, foliate, fungiform .🍄

كلهم فيهم test puds عدا ال faliform papilae
○- posterior 1/3 ( pharyngeal part )
Smooth and rich in mucous gland
لحدي هنا كويسين ؟☻️ ارح قربنا انتو لها ❤️‍🔥


💪-Muscles of the tongue:-
1- extrinsic muscles
2- intrinsic muscles

🦋1) extrinsic muscles:-
يلا ديل ٤ عضلات بغيرو موقع اللسان يعني بحركوهو طيب هم شنو 🤔
1- genioglossus :-
- من ال superior genial tubricle عامله insertion in whole under surface of the tongue
-بتعمل protrusion of the tongue👉

2- hyoglossus :-
-From grater horn of hyoid bone to the posterior 1/2 of side of the tongue
- do depression of the tongue 👇

3- styloglossus :
-From styloid process to whole side of the tongue
-do elevation and retraction of the tongue 👆

4- palatoglossus :-
من ال soft palate لحدي ال side of the tongue
- it elevate back of the tongue and depress the soft palate

🦋2) intrinsic muscles:-
ديل يلا ٤ عضلات بغيرو شكل اللسان الي هم
1- superior longitudinal
2-inferior longitudinal
3- transverse ( narrow tongue)🤲
4- vertical ( flattened the tongue)👐

🤓⚡️The nerve supply of the tongue :-
□Motor supply > all muscles of the tongue supply by [hypglossal nerve] except palatoglossus muscle supply by[ vago accessory complex ]
اتفهم🤓👍؟!...يلا نمشي للبعدو

□posterior 1/3 :- General sensation and test sensation by [ glossopharngeal nerve]

□anterior 2/3 :-
general sensation by [ lingual nerve]
Test sensation by [ chorda tympani]


🩸Blood supply of the tongue
By lingual artery ( from common carotid artery) through 2 branches

ال deep cervical lymph node هم ال
- jugulo _omohyoid
- jugulo _digestric
اتذكرو أنو ال final lymphatic drained of the tongue by > jugulo _omohyoid 🫡
🍁 the palat :-
دا المكون الفاصل بين ال oral cavity and nasal cavity 🤥وهو جزئين

(1)- 🦋hard palate :-
ودي بتتكون من:
1) palatine process of maxilla > form the anterior 3/4 of hard palate

2) horizontal plate of platin bone > form the posterior 1/4 of hard palate

◇there's incisive formen giving passage for nasopalatine nerve and grater palatine vessels😶

🪢blood supply of hard palate :-
Grater palatine artery from 3rd part of maxillary artery.

🪢venous drainage:
Pterygoid venous plexus
بكل بساطة 😌
⚡️nerve supply:-
-Anterior part > naso palatine nerve
- posterior part > grater palatine nerve
والإتنين ديل يعتبرو branches from [ pterygopalatine ganglia ]

(2)- 🦋soft palate:
○ودي بتعتبر متحركة
movable muscular part of palat
○بتمتد من posterior border of the hard palate.
○ وكمان بتفصل لي nasopharynx منoropharynx

💪Muscles of soft palate:-
(صغيرة وعندها عضلات 😂 )
〰1- tensor palati >

Origin :From scaphoid fossa
Insertion : tend wind around pteyrgoid hamulus
Action : tension of soft palate
يعني بتعمل على شدها
〰2-levator palati>

From> lower surface of petrous temporal bone
To > upper surface of palatine aponeurosis
It elevate soft palate upward and backwards during deglutition
ودي مهمة في عملية البلع شدييييد

〰3- palato pharyngus:

-Originate from upper surface of palatine aponeurosis
-inserted into posterior porder of thyroid cartilage
- elevate pharynx and larynx to close laryngeal inlet during deglutition
برضو مهمة في البلع🙄

〰4- palatoglossus:

-From Lower surface of palatine aponeurosis
- to side of the tongue
- elevat posterior part of the tongue

⚡️Nerve supply of muscles of soft palate:
-motor > all muscles by vagus through pharyngeal plexus except [tensor veli palatini by the mandibular branch of the trigeminal nerve]
- sensory > grater , lesser nasopalatine branches of maxillary
- test > pass through pterygopalatine ganglia to grater petrosal nerve

كيفكم يا شباب عساكم بخير 😄✨

الليلة حنتكلم عن organ مهم شديييد ومعروف عند بعض "بعض" الناس بالثقب الأسود😂😂/أو التلاجة 😭😂
أي أي الليلة حنتكلم عنها شخصيا "Stomach "🔥🔥🔥

أرح نتعرف عليها شوية🤓
○ال Stomach تعتبر هي ال dilated portion من القناة الهضمية.

وحنلاقى عندها 3وظائف رئيسية واللي بتشمل:

(1).بتعمل لينا تخزين للأكل Stores food..
في ال adult عندها سعة [capacity] حول ال 1500 ml يعني لتر ونص كدا😳
(غايتو البحصل في السحور والإفطار حق رمضان بفوت الرقم دا بمراحل😭😂).

(2).بتعمل لينا mix للأكل مع ال gastric secretions ودا عشان تكون لينا semifluid chyme.(ودا عشان الأكل يكمل رحلتو البتضمن بقية عمليات الهضم والامتصاص)🫠

(3).بتعمل لينا control لمعدل نقل ال chyme القبل شوية دي لل small intestine .🏂

تعال معاي نشوف موقعها 📍
🔶Location
بنلقى ال stomach قاعدة في الجزء العلوي من ال abdomen. وبتمتد من ال left costal margin region لحد ال epigasteric region .

والجزء الأكبر منها بكون مغطى تحت ال lower ribs.🧑‍🦯 ودا بهد الحماية طبعا😏

🔶Description
شكلها بشبه حرف ال J_ shape .
○وبنلقى فيها فتحتين [two opening]

١.واحدة بتربطها مع ال esophagus وإسمها cardiac orifice(سموهو كدا لأنو قريب من القلب🫀ودي حتفيدنا في التشخيص المرضي بعدين)

٢.والتانية بتربطها بال small intestine وإسمها pyloric orifice

○وطبعا بما أنها في شكل J بكون عندها منحيين [two curvatures] واحد بكون صغير والتاني كبير [lesser &greater]👶👨‍🦳

○وعندها two surfaces واحد anterior والتاني posterior.

ومع كلللل الكلام دا إلا أنو شكلها بختلف من زول لزول (قصير طويل .. سمين ضعيف.. صغير كبير) ياخ بتخلف في الزول زاتو إتخيلو 😅 (لما تكون مليانة ما زي لما تكون فاضية) .. وحتى أثناء عملية ال respiration وحتى مع وضع الجسم.

وضحت؟😉يلا نشوف بقسموها كيف👀

○ال Stomach بتتقسم إلى الاجزاء الآتية:

•Fundus:
ودا بكون dome shape زي شكل القبة كدا 🕌
وبكون في الجزء العلوي وبكون قريب من ال cardiac orifice وعادة بكون مليان gas🌫

•Body:
ودا بمتد لينا من ال cardiac orifice لحد ما نصل لمستوى حاجة إسمها incisura angularis (ودي بتكون زي طفجة في الجزء السفلي من ال lesser curvature .. اتذكرو ال J shape)😄

•Antrum:
ودا زول بسيييط بمتد لينا الincisura angularis إلى الpylorus.

•Pylorus:
ودا الجزء الأخير وممكن نقول أنو أكتر جزو tubular وبنلقى فيهو عضلات كثيفة [thick muscular wall] عشان تعمل لينا ال pyloric sphincter. 🧣

○ ال pyloric sphincter دا بلعب دور مهم شديد في ال control لمرور ال gastric contents لل duodenum.🫸

_وبنلقى motor fiber من ال sympathetic system وال inhibitory fibers من ال vagi (عشان كدا لما تكون مروق ومستكين بتقدر تهضم كويس)🦭


○وبما أنها Stomach وعندها عصارات وإفرازات فبكون فيها mucous membrane عشان يحميها🛡

وبما أنو بكون فيها إمتصاص للمواد الغذائية فبكون فيها vascular

وكمان حنلقى في طفجات كتييرة [numerous folds] وبنسميها rugae ( أي روقي عدييل كدا 😂😜) ودي بتساعد ال stomach في التمدد وهي في الغالب longitudinal in direction.

يلا ديل كللهم بنقسمو لينا من ال celiac artery واللي هو جايينا من ال aorta. يلا نشوف تفاصيلهم 👀
1.left gastric artery:

ودا بطلع لينا من ال celiac artery مباشرة ودا بطلع فوق [upward] وبلف يسار لحد ما يصل لل esophagus وتاني بنزل تحت عل إمتداد ال lesser curvature
ودا بغذي لينا ال lower third of esophagus ومعاهو ال right upper part من ال stomach.↗️

2.right gastric artery:

ودا بطلع لينا من الhepatic artery ودا بعمل supply لل الجزء اليمين تحت [lower right part] من ال stomach↘️.

3.short gastric arteries:
وديل بطلعو من ال splenic artery وبغذو لينا ال Fundus 🕌.

4.left gastroepiploic artery:
ودا بطلع من ال splenic artery برضو لكن بغذي لي ال upper part of greater curvature⬆️.

5.right gastroepiploic artery:
ودا بطلع لينا من gstroduodenal واللي هو تفرع من ال hepatic artery ودورو أنو بغذي لينا ال lower part of greater curvature⬇️.

🟢 Lemph drainage:
يلا الlymph vessels بتكون ماشة مع ال arteries لحد ما يصلو لينا لل right gastric nodes و ال right & left gastroepiploic nodes وال short gastric nodes ديل كلللهم بصبو لينا في ال celiac nodes.🫥
