ال macrophage بعد تبلع الcausative a بتحاول تقتلو...نظرا لانو هو قوي شديد وهرب من الacute هناك م حيموت

Granuloma؟
لان شكلها بكون كده ...تتجمع كل الخلايا البالعه حول الnecrotic tissue وبيحصل ليها تضخم نتيجه للكلام القلناه فوق دا..
برضو بنلقي انواع كتيره من الخلايا متجمهره هناك..من بينهم خلايا الgaint cells
ودي قلنا نفسها الMacrophage كبرت وحصل ليها fusion

يعني لمن يحصل فقد او تلف للخلايا هنعمل الريبير الي هو انو نستبدل الخلايا الي خربت بخلايا جديدة زي لمن جزو من عجلتك يخرب تمشي للمكانيكي (او الزول لي بيصلح العجلات والله م تخصصي فما عارف🫣) فلو لقى مثلا او الفرامل حقت العجلات عندك السلك حقها للنهاية انت خربتو يقوم يشيل الفرامل ويجيب واحد بدالها )
خليك مع مثالي ده هو مثال شبه م مهم بس هنحتاجو كلو من اول كلمة للأخيرة

هنا نفترض انك كنت شقي وانت صغير ولو م شقي نرجع بالزمن نبقيك شقي عشان نفهم النقطة دي بس 😃

وجيت بتلعب  الغميضة وجو اصحابك جاريين وراك عشان يمسكوك وانت م عايز كدا والشارع كان فيه زجاج مكسر
وجيت جاري وفرامل رجولك مافي😂

وفجأة للأسف درست الزجاج ده بكل وزننننك الي كان تقيل عشان انت شوية كنت زول سمييين كدا وعمل ليك جرح لكن م عميق شديييد يعني لكن م كان بقيف لمن ضمتو براك كدا فاضطريت مشي للدكتور وهنا بدأ الدكتور يعمل  العجب وكأنه يستخدم عصاية سحرية وخيط ليك الجرح يعني ضم ال skin edges لبعض كدا قلل مساحة المنطقة الي هتنزف بالتالي عدد الخلايا الي هيحصل ليها necrosis and inflammation والعملية السحرية دي بيسموها 🪄.  healing by first intention.

طيب يا صاحبي ماذا لو أنك أبو الشقاوة كلها

قلتلك م هخليك الا اكمل الدرس شرحا وممكن تتعب معليش يعني🌚

💫طيب أول شيء لازم نعرفه هو المقصود بالdigestive system شنو... بإختصار هو الجهاز الهضمي و
its functions are the ingestion, digestion, absorption and defecation of food.

💦عندنا طريقتين لتقسيم الdigestive system

1) Oral cavity, pharynx and alimentary canal
و نعرف إنه الalimentary canal ده من الesophagus لحد الanal opening

2) GastroIntestinal Tract ; GIT
و ده من الmouth لحد ال anal opening

💫طيب عندنا main organs
و اللي هي
oral cavity/mouth, pharynx, esophagus, stomach, small and large intestines, rectum and anus

💫ال accessory organs هي ال
teeth, tongue, salivary glands, liver, gallbladder and pancreas

💢خلو بالكم ... الspleen ما organ في ال digestive system
هو organ من الlymphatic system

💦طيب نبدأ من الoral cavity

💫الoral cavity بتكون lined ب
stratified squamous epithelium
(الما قروا هستو لحدي هسي 🌚 و شغالين يقروا ملخصات بس 🌚🌚🌚 شايفنكم بالمناسبة.. قوموا اقروا و أنفعوا نفسكم.. محتاجين الجهود كلها يا دكاترة 💪🏿💪🏽💪🏻)

الoral cavity فيها :

الupper lip و الlower lip كل وحدة مثبتة بي frenulum
⬅️ من جوا بنلقى الteeth
و اللي هي deciduous (لبنية) و عددها 20 ، بتبدأ تظهر من عمر 6 شهور.

أي زول عنده شافع في بيتهم يتكرم يمسكه و يعد أسنانه للعلم 😂
*و permanent (دائمة) و عددها 32 ، و بتبدأ تظهر من عمر 6 سنين
الأسنان قاعدة في sockets و الgingiva بتحيط الbase بتاع الأسنان

💫وظيفتها :
mechanical breaking, churning and crushing of food

💫عندنا   six salivary glands
بتكون pairs
*ديل بيكونو في الجهتين وحدة في اليمين و التانية في الشمال، شفتوا مكان أذنكم... لقدام و لتحت شوية منها عند الخد parotid
*ديل تحت الفك.. برضو يمين و شمال submandibular
*ديل تحت اللسان sublingual

الsaliva أو اللعاب الناتج بيصل عن طريقducts و بيحتوي على mucus, amylase and lysozome

💦وظيفته : يبلل الأكل و يعمل ليهو lubrication و dissolving ، و يحافظ على رطوبة الmucous membranes

💫  الtongue عبارة عن skeletal muscle covered with mucosa, and it has papillae on its surface
يلا الpapillae دي ببساطة الحاجات الطالعة نازلة في لسانكم دي... بالله كل واحد يشوف أقرب مراية و يعاين لي لسانه 😛😂
طيب و أنتو عند المراية لسه، أرفعوا ألسنتكم... مش في حاجة ماسكة من تحت اللسان للأرضية بتاعت الmouth؟ دي اسمها lingual frenulum

💦وظيفته :
يحرك / maneuver الأكل ، التذوق / taste ، الكلام / talking

💢وظيفة الoral cavity ككل
Take in food and prepare it for digestion

💦الpharynx و الesophagus

الأكل الممضوغ بيمر عبر الpharynx في الأول
نقطة مهمة تعرفوها عن الpharynx هي إنه ممر مشترك لثلاثة حاجات
الأول nasopharyngial
التاني oropharyngial
الثالث laryngopharyngial

الesophagus هو tube مكانه وراء ال trachea
جزء منه في الcervical region، الجزء الأكبر في الthoracic region و أصغر جزء في الabdominal region ... طول الesophagus تقريبا 25cm

و زي الpharynx، هو مكون من عضلات بتحرك الأكل بmotion بنسميها peristalisis

💦ال stomach
بين نهاية الesophagus و بداية الstomach
بنلقى sphincter muscle بنسميها cardiac sphincter muscle... هي عبارة عن بوابة بين الإثنين بس لا أكثر 🤷🏽‍♀️
الجزء العلوي من المعدة بنسميه cardia لأنه جمب القلب، و البعده fundus، و الفي النص body و الفي النهاية pylorus

طيب ال stomach هي the most dilated part of the GIT and it acts as a temporary storage place for food

هي برضو بتهضم لي الأكل عن طريق إفراز gastric juice و اللي هو مكون من HCl ، pepsinogen and mucus

و غير كده الstomach عندها muscles (longitudinal, circular and oblique) بتساعد في الmechanical digestion of food

💢نقطة مهمة :
الoblique muscles خاصة بالstomach بس في الdigestive system
المعدة ما بتمتص أي أكل.. لكن ممكن تمتص لي water, electrolytes, drugs like aspirin and alcohol
المعدة بتهضم الproteins عن طريق تحويل الpepsinogen لي pepsin باستخدام الHCl ، و الأكل في المعدة بيخفف لشكل سائل بنسميه chyme
الmucous البيتفرز بيحمي جدار المعدة من الaction حق الpepsin... في حالة نقص الmucous ده (لأسباب زي الاستفراغ الكثير) بيقل الmucous و جدار المعدة بيبقى معرض لإنزيم الpepsin و بالتالي بتنتج قرحة أوpeptic ulcer.

The salivary glands are exocrine glands found in mouth produces saliva,

It site between ramius of mandible and sternomastoid muscle below External auditory meatus .
💢boundaries of Salivary Glands
من فوق وتحت بتكون عبارة عن عضلات
Upward 👉zygomatic arch
Down wards 👉 angle of the mandible.
أما من الامام والخلف بتكون عبارة عن عظام
Anterior 👉
Overlap masseter muscle
Posterior 👉 overlap sternomastoid muscle
أما شكل ال salivary glands فبختلف حسب نظري جاي من أي جانب كما سنرى بعد قليل 👇👇

1-pyramidal.
2-rounded nuclei.
3-narrow leumen with basophilic staining.

2-cuboidal to columnar.

5-mucous cells form tubules.
💢What is serous demilune?
Is mucous tubule capped with serous acinus.

💢parotid Glands
طبعاً قلنا إنها أكبر salivary glands لكن كمية الsaliva البتفرزها 25% بس😥🥺
فصلنا فستان ال parotid Glands في نقاط سمحه ،حظها حششت للعيد 🤣😅😂
1-serous glands.
2-covered with capsule which is part of deep facia.
3-at the side of mandible.
💢تتذكروا قبيل قلنا إنو الgland بتجي من duct ؟؟؟طيب ال parotid duct موقعه وين من الإعراب؟؟
1-the duct penetrates through buccinator muscles.
2-it's palpable over masseter muscle.
3-it oppens into vestibule against upper second molar teeth.
-it extends from zygmotic arch to the upper part of the neck.
-overlap the masseter muscle anteriorly.
-in transverse section it occupies the gab between mandible, mastoid and styloid processes.
4-the capsule is supplied by great auricular nerve.

1-superficial surface :-
🔸related to the skin and facia.
2-anteriomedial surface :-
🔸grooved by posterior border of mandible.
🔸related to the masseter and medial ptegroid muscles.
🔸The parotid duct and branches of facial nerve emerge from anteriomedial surface also terminal branches of external carotid artery(ECA).
3-Posteriomedial surfaces :-
🔸contacts to styloid and mastoid processes.
🔸ECA enters the gland through this surface.

Innervation of parotid Glands

🔸Arteries:- external carotid artery and its branches within gland.
🔸Veins :- retromandibular vein.

Facial nerve passes through it

1\superficial part وهو الجزء الرئيسي شكلها بشبه اللوزة🥔🥔و
بكون كبير🐃

💢superficial surface:-
بتتكون من:-
_Cervical Fascia
_Cervical branches of facial nerve
_anterior facial vein
💢medial surface:_
بتضم لينا عضلات واوعية دموية ومفاصل 👇👇👇
Mylohyiod muscle
Hyoglossus muscle
Intermediate tendon of digastric

💢Lateral surface
لو نظرنا لل lateral surface of Submandibular glands بنلقى فيها Mylohyiod line وطالما هو خط معناها بكون فاصل بين حاجتين Submandibular fossa and sublingual fossa
وبرضو ال facial artery بجي ماري في النص بين ال mandible and Submandibular glands
بالإضافة لاحتواها على ال Mylohyiod vessels and nerve.
2/deep part
موجوده بين 2muscles2 nerve
Muscles:-Mylohyiod and hyoglossus Muscles
Nerves:-lingual nerve above and hypoglossus below.
موقع ال submandibular بالضبط 🌏في ال digastric triangle وشكلها Almond.

It's parasympathatic gangilion🧠
بتقع في ال lateral surface of hyoglossus muscle.
💫ال root حقتها بتكون preganglionic fiber أما برانشاته🌿 بتكون postganglionic fiber .
💫مزايا ال Submandibular glands
-it's duct oppens into oral cavity proper.
-the duct passes forward to oppen on small sublingual papilla beside the base of frenulum,

Nerve supply
Artery 👉submental branches of facial artery.
Vein 👉anterior facial vein
Lymph node 👉facial artery node,deep cervical and jugular node.

💢sublingual glands

-mucous glands located below the tongue.
وهي أصغر قلاند في الساليفري قلاند.
-each is almond shaped and is immediately lateral to Submandibular duct.
-associated to lingual nerve in floor of oral cavity.
💫Sublingual and Submandibular are supplied by branches of facial and lingual arteries.
-lies against medial surface of the mandible and forms shallow groove(Sublingual fossa) supperior to the anterior 1/3 of mylohyoid line.

Blood supply 💦

Artery 👉sublingual branches of lingual artery and submental branches of facial artery

Nerve supply 👉sensory supply from lingual nerve
Sympathetic and parasympathatic supply from Submandibular gangilion.

ومن اسمها معناها مهمه في الطقه🍔🍕🤤 🥨

              يلا  بسم الله نبدا 🫡🔥

🛑اولا هم muscles 4⃣موجدين علي جانبي ال 💀skull :
1-masseter
2- temporalis
3- medial pterygoid
4-latral pterygoid

🛑ثانياً:-
عندهم حاجات مشتركه😻🤝 هي:

1-they take origin from skull.
2-inserted into mandible .
3-they act on temporomandibular joint .
○كسره
-ثواني  ثواني 🤌ماشين وين🚶🤚 نتكلم شويه عن ال joint ده:

-اولا ☝️هو قاعد بين articular tubercle و anterior portion of the
mandibular fossa
-ثانيا✌️ نوعه synovial joint

4-they supply by mandibular nerve

       كده خلصنا الحاجات المشتركه.

🛑ثالثاً:-
Movement of mandible🌪:

_elevation ⬆️and depression⬇️.
_protraction ➡️and retraction⬅️ .
_side by side movements⏭⏮.

✨بعد كده هنمسكهم muscle muscle هنعرف ال originوال insertionو ال action ي إلهي يعني ذي locomotor😳 لا لا ياخ ده حلو شويه😍😂

💢1/Masseter muscle :-

-✨ origin : lower border and inner aspect of zygomatic .

- ✨insertion: outer surface of ramus arch of mandible .

- ✨action:
1- elevation of mandible.                                                 (.    anti-gravity muscle
هي عباره عن عضلات معينه ذي الموجوده في الرقبه بتكون مشدوده طول م الشخص مستيقظ😎 و تترتخي أثناء النوم😴)
2- protrusion ➡️of the mandible.

2-⬅️retraction of mandible by posterior fiber .

شباب في  pterygoid هنا ال silent  P

⭐️-كأنها  mirror image of Masseter muscle

A- superficial head (tuberosity of maxilla )
B- deep head (medial surface of medial pterygoid plate )

- ✨insertion : medial surface of ramus above the angle.

- ✨action:
1- elevation ⬆️of mandible.
2- protrusion ➡️of mandible.
3- deviations to on sides ↔️


💢4/lateral pterygoid :-

⭐️دي اخت 👭ال  medial pterygoid
برضو عندهاupper and lower2 head

- ✨origin:
A- upper head ( infratemporal crest
B- lower head ( lateral surface of lateral pterygoid plate)

-✨ insertion: pterygoid fova.

-✨action:
1- depression⬇️ of mandible.
2- protrusion ➡️of mandible.
3- deviations ↔️to one side .
