Primary bone tumors

1️⃣Osteosarcoma

🌿highly malignant tumor in bone
🌿mainly in children
🌿long bones metaphysis especially around the knee and the proximal end of humerus.
🌿first symptom is pain, increased at night ,lump,tendrness ,redness
🌿increase in alkaline phosphatase enzyme.
🌿Codman's triangle, Sunburst appearance.
🌿 definite diagnosis, biopsy should be taken.
🌿 Differential diagnosis: stress fractures with callus, chronic osteomyelitis, and larg gouty tophus

🌿Poor prognosis
🌿fast growing

2️⃣Chondrosarcoma

🌿primary or secondary to pre-existing benign chondroma or osteochondroma.
🌿 4th and 5th decades,
🌿men more
🌿slow growing
🌿 dull ache and enlarging lump and pathological fractures
🌿cartilage
🌿 radiolucent area with central flecks of calcifications
🌿 not respond to either radiotherapy or chemotherapy

3️⃣Ewing's sarcoma


🌿 endothelial cells in bone marrow
🌿 10-20 yrs old
🌿 tubular bones especially the tibia in
fibula, and clavicle. In mid-diaphysis
🌿 Onion-peel appearance.
🌿prognosis is poor.

4️⃣Multiple myeloma

🌿 malignant B-cell disorder of the bone marrow with predominant plasma cell
🌿 45-65 yrs old,
🌿Hypercalcaemia
🌿 multiple punched-out defects in the
skull, pelvis, and proximal femur, or compressed vertebrae.
🌿 Bone biopsy shows
plasmacytosis with typical myeloma
cells.
🌿 Chemotherapy like melphalan as specific treatment and corticosteroids for
bone pain are used.

Secondary metastatic bone tumors

🌿 more than 50 years old
🌿 commonest source is carcinoma of the breast, prostate, kidney, lung, thyroid, bladder, and gastrointestinal tract
🌿 The commonest sites for bone metastases are the vertebrae, pelvis,the proximal half of the femur and humerus.
🌿 usually osteolytic except prostatic carcinoma is Osteoblastic
🌿 the sudden appearance of backache or thigh pain in an elderly person is always suspicious.
