🐣طبعاً ممكن يكون primary بدون سبب يعني idiopathic وممكن يكون secondary يعني اكو disease يسبب hyperuricemia ……… الacute يصير بسبب inflammation الناتج من الphagocytosis to crystals in joint مما يسبب synovitis يقود إلى sever pain with swelling & redness …(علاجه موجود بالصورة ) …………
🐣ال chronic راح يكون نوبات متكرر من acute attack خصوصاً بالاشخاص الي أوزانهم مرتفعة وكدلك alcoholism او الاشخاص الي ياكلون لحوم هواية … وهذا ممكن يقود إلى renal failure ايضاً وتجمع crystals of uric acid راح اسمي tophi هذه ممكن تتقرح وتكوم تنضح milky discharge 🥲
🐣طبعاً علاج الacute بين attack (يعني في نهاية الacute attack انتبه لازم تنتهي يلا انطي لانه هو اصلاً يزيد الattack ) راح انطي allopurinol (طبعا. وانطي وياه ادوية acute الي بالصورة )

Primary bone tumors
1️⃣Osteosarcoma
🌿highly malignant tumor in bone
🌿mainly in children
🌿long bones metaphysis especially around the knee and the proximal end of humerus.
🌿first symptom is pain, increased at night ,lump,tendrness ,redness
🌿increase in alkaline phosphatase enzyme.
🌿Codman's triangle, Sunburst appearance.
🌿 definite diagnosis, biopsy should be taken.
🌿 Differential diagnosis: stress fractures with callus, chronic osteomyelitis, and larg gouty tophus

🌿 malignant B-cell disorder of the bone marrow with predominant plasma cell
🌿 45-65 yrs old,
🌿Hypercalcaemia
🌿 multiple punched-out defects in the
skull, pelvis, and proximal femur, or compressed vertebrae.
🌿 Bone biopsy shows
plasmacytosis with typical myeloma
cells.
🌿 Chemotherapy like melphalan as specific treatment and corticosteroids for
bone pain are used.

Secondary metastatic bone tumors

🌿 the sudden appearance of backache or thigh pain in an elderly person is always suspicious.
