Femoral Supracondylar fracture :

طبعاً مولر كلاسفكيشن ما راده الدكتور وبسرعة طفر للmengmemt فالعلاج راح يكون ORIF بسبب وجود عضلة (نسيت اسمها يمكن gastrocnemius 😅) ………ومن اهم مضاعفات :-

💡neurovascular injury = popliteal.A ,tibial.n
💡stiffness ,osteoarthritis,non union

Patella fracture :

👉🏻direct trauma = comminuted fracture (stellate) 👉🏻treatment by patelactomy


👉🏻indirect trauma =tranverse fracture 👉🏻treatment by ORIF (K-wire )+ barce or backslap

سواء كان displace or non displacement بسبب pulling by extensor .m

Supracondylar fracture= non union

Proximal tibia= compartment

Distal tibia= non union

عندي fracture of tibia plateau هذه له classifications على اساسه راح احدد العلاج مال المريض (طبعاً لدكتور رائد رأي ثاني بعدين تعرفونه 😅؛) المهم هذا الكلاسفگيشن :

♻️type2 =verticle fracture +depressed


♻️type3=fracture depression in articular surface

♻️type4=medial condyle

♻️type5=both condyle

♻️type6 =both condyle +subcondyle

مرة اخذنا دكتور سعد جمعة فكال كل الكسور الي تؤثر على الknee راح يصير وياها hemo-arthrosis ويكون مصحوب بswelling & tenderness فلازم تسوي له aspiration نوب بلش عالج :-

Fracture of proximal tibia & fibula :

طبعاً اني الي يهمني دائماً ال tíbia لكن إذا ال fibula ما مكسوره واكو كسر بtíbia (راح اكسر الfibula حتى من اسوي fixation يثبت بدون ما يصير non union 😊 شكد مظلومة الtíbia لعد مثل امك من اخوك الصغير يسوي مصيبة وتجي ترزلك انت 😂😂 ) المهم هنا في الغالب راح تكون الكسور هي open fracture لكن ممكن تكون close fracture وقد تكون spiral or oblique المهم خلي نحجي على تصنيف غليتسون للجروح المفتوحة :-


Type 1 👉🏻soft injury (>1cm ) with minimal comminuation ..


Type 2👉🏻soft tissue injury (2-5cm )with moderate comminuation with minimal strip periosteum



Type 3A 👉🏻soft tissue injury (5-10cm) with displacement fracture and loss fragments

Type3B 👉🏻more than 10cm ..need to skin flap +associated with nerve damage

Type3c 👉🏻need to skin flap +associated with arterial damage ,bone exposure need to skin graft

Close fracture ;type1 open fracture (non displacement) =conservative by cast

Close fracture ;type1 open fracture (displacement) =close reduction follow by cast if failure this use ORIF

Open fracture (الخطوات الي تعرفها ) + external fixation

حط فبالك من اهم اهم اهم المضاعفات الي تصير ويه proximal tibial fracture هي compartment syndrome (من تعالج ضروري تحطها فبالك لانه 90% من المرضى الي عندهم tibial fracture راح يتعرضون لها ) ومن اهم الاعراض الخ :-

👉🏻stiffness
👉🏻infection
👉🏻osteoporosis
👉🏻osteoarthritis
👉🏻neurovascular injury (Popliteal.A)
👉🏻reflex sympathetic syndrome
👉🏻non union ,delay union ,mal union
