1-avulsion fracture
2- direct fractures (ischeal fracture and fracture of iliac blade
3- stress fracture (fracture pubic rami ,fracture around sacroiliac joint)

1-anteriorposterior compression

يوصفوا ب؟
Open book injury

شوكت يكون stable?
اذا الfracture less than2.5cm
Or more than2.5cm and dont occour tear of anterior and posterior sacroiliac ligament

شوكت يكون unstable?
If fracture more than 2.5cm
And tear of both anterior and posterior sacroiliac ligament

2-lateral compression

شلون يصير؟
RTA
شوكت يكون stable?
1-transverse fracture of pubic rami
2-anteirt pubic fracture + fracture of iliac wing on same side

شوكت يصير unstable?
Fracture of pubic rami
2-fracture of iliac wing
3-fracture of contralateral illium

3-vertical shear

العلاج؟

اول شيء ABC
ثاني شيء نوكف الدم
نستخدم الطرق نعرفها
واذا جان نريد نوكفه بسرعة
Pelvic bleeding will be reduce rapidly by external fixator or pelvic binder

ثالث شيء نعالج urethral and bladder injury?
Not use folycathetar here
لان يحول partial to complete tear

فشنستخدم؟
Pratial tear? Suprapubic catheter
For complete tear?
Suprapubic cystostomy

هسه نجي نصلح الكسر واخيرا🤣
If stable open book (AP compression)?
Bed rest and hammock sling traction
اذا جان open book unstable?
Skeletal longitudenal traction and external fixation then? Open. Reduction and internal fixation

Complication

1-siatic nerve injury
Usually neuropraxia(شلل مؤقت)

2-urogenital injury
3-perisitance sacroiliac pain
4-DVT and pulmonary thrombosis

‼️‼️‼️عندي مرض اسمه perther’s disease هنا راح يجيك طفل كتكوت عمر 4-10 سنوات ويعرج (طبعاً الاهل راح يتجاهلون الامر عبالهم شبة طول ) فهنا صاير a vascular necrosis بال femoral head لحد الان سببه unknown لكن البعض يعتقدون انه له علاقة بالblood disease مثل antithromin factor deficiency or hypofibrolysis,thrombophilia …… وأكثر شيوعاً بالميل 🥲🥲 طبعاً هذا المرض بي stages مثل:


🚨stage 1 =ischmia

هنا ال X-ray يبين العظم نورمل لكن من تركز اكثر راح تشوف انه العظم المصاب متوقف نموه فصاير اقصر من اخوه …فحتى تأكد التشخيص دزه على MRI طبعاً هذه خلال 8h وتبلش necrosis الي يكون 4grade :-

Grade1=necrosis less than 50% of nucleus (head)
Grade2=necrosis more than 50% of nucleus (head)
Grade3=necrosis more than 75% of nucleus (head)
Grade2=necrosis all bone

🚨stage 2 = revaculartion:

هنا راح يصير cá deposition فراح يصير high density وال epiphysis fregmention (يعني راح ينكسر ) وكذلك metaphysis راح يصيرله osteoporosis ……


🚨stage3 = remodelling:

هنا راح يبنى عظم مره لخ ويكون شكله يشبه المشروم 🍄🍄‍🟫

💡sickle cell anemia
💡hypothyroidism
💡slipping epiphysis
💡epiphyseal dysplasia
💡morquio’s disease (حفظتها مرجان )

🦧🦧هسه راح اعالج المريض إذا اكتشفته early راح احطله skin traction لمدة 6week ونوب اخلي يمشي ……… اما إذا اجه late راح انطي مسكنات الم نوب احدد الحالة واسوي له hip spica & brace …… وفي حالة sever راح اسوي osteótomy …

Slipping epiphysis :

هنا راح يصير displacement to proximal femoral تحديداً epiphysis of femoral neck فراح يسبب dislocation or subluxation فراح يجي البيشنت بوضع flexion & external rotation …… وعلى هذا الاساس راح يكون :-

🚨Mild

العلاج راح يكون fixation 2screw or pin ……

🚨Moderate

راح اثبت ب internal fixation (2scrow ,pin) وبعد مدة راح اسوي osteótomy …

🚨Sever

راح يكون open reduction ونوب راح اسوي fixation ……لكن في بعض الحالات اسوي fixation بدون reduction وdeformity راح يحتاج اسوي osteótomy ..
