راح اشرح موضوع كلللش مهم وعليه 6% المهم هذا الموضوع اسمه DDH يعني congenital hip dislocation مرات يجي dislocations ومرات subluxation ومرات اخرى فقط unstable بعدين يصير subluxation or dislocation طبعاً د.احمد كال كل الف مولود ينولدون 2 عندهم DDH ومن اهم risk factor هي :-



🚨اول شي ال breech presention اثناء delivary

🚨احتمال اثناء الحمل صار oligohydrominous

🚨ال left اكثر عرضه لانه الطفل فبطن امه مخلي رجله بوضع abduction بسبب اصطدامها بال lumber vertebrae مال الام

🚨يصير في الاناث اكثر من الذكور لانه الفيميل عندها اصلاً هواية استروجين فمن يدخل استروجين من الام عن طريق المشيمة راح يعمل على تقليل مرونه الligament فراح يصير dislocation …

🚨او ممكن genetic factor او race factor بسبب الكماط

راح اجي وألزم الطفل من جهة medial thigh واسوي flexion نوب fully abduction بحيث اخلي رجلينه يطخون بسرير الفحص فاذا حسيت اكو resistance or limited movement فهنا dislocation ومرات اسمع صار طكة كللليك هنا يعني صار reduction للخلع

Barlow test :

راح اجلي اسوي للطفل adduction نوب axial compression. فراح يصير subluxation 🦧🐣

بعد ما تنسوها مو 😂😂

طبعاً اكو علامات همات تظهر على الطفل فاذا كانت امه مثقفة ونبهة راح تنتبه عليهم مثل :

🚨 shortening leg this called Galeazzi

الطفل اول اربع اشهر جيمه مابي عظام فقط cartilage فراح أشخصه بواسطة US ……… لكن بعد 6اشهر راح يكون التشخيص بال X-ray فراح اشوف 3line :-

♻️ ال vertical line اسمه Perkin’line (هذا حفظته بكر 😂) هذا مرسوم بال Lateral aspect of Acetabulum …


♻️ال shenton line (حفظته شجون 🤫) هذا يبين medial border of femoral neck & superior border of abturator forâmen …

‼️‼️في الحالة النورمل ال femoral head موجود medially below vertical line مع وجود Shenton line لكن في حالة الخلع راح يكون laterally ,above vertical line وال shenton راح يختفي …

🦋 إذا شخصت الحالة باول أسبوعين من حياة الطفل اكول للام لبسيه 2-3 حفاظات حتى ابقي الthigh مفتحوه يعني بوضع abduction …


🦋 إذا شخّصتها وره الأسبوعين او 6 اشهر إلى عمر السنتين راح اللبسه مسند اسمه pavlike …… طبعاً هذا نسبة نجاحه 95% في subluxation و80% بال dislocation يعني خرافي …… لكن إذا ما فاد راح اسوي close reduction واحط hip spica (DDH spica ) لمدة 12weeks وابقى أراقب بال MRI,CT … إذا ما فاد او عمر الطفل عبر السنتين راح افتح open reduction ومرات يوصل انه اسوي femoral osteótomy مع او بدون pelvic osteótomy ……فبليز لا توصلون الأطفال لهيجي عمر اول ما تكتشفه دزه على دكتور اختصاص وانتهى

3- stress fracture (fracture pubic rami ,fracture around sacroiliac joint)

1-anteriorposterior compression

شوكت يكون stable?
اذا الfracture less than2.5cm
Or more than2.5cm and dont occour tear of anterior and posterior sacroiliac ligament

شوكت يكون unstable?
If fracture more than 2.5cm
And tear of both anterior and posterior sacroiliac ligament

2-lateral compression

1-transverse fracture of pubic rami
2-anteirt pubic fracture + fracture of iliac wing on same side

شوكت يصير unstable?
Fracture of pubic rami
2-fracture of iliac wing
3-fracture of contralateral illium

العلاج؟

اول شيء ABC
ثاني شيء نوكف الدم
نستخدم الطرق نعرفها
واذا جان نريد نوكفه بسرعة
Pelvic bleeding will be reduce rapidly by external fixator or pelvic binder

ثالث شيء نعالج urethral and bladder injury?
Not use folycathetar here
لان يحول partial to complete tear

فشنستخدم؟
Pratial tear? Suprapubic catheter
For complete tear?
Suprapubic cystostomy

هسه نجي نصلح الكسر واخيرا🤣
If stable open book (AP compression)?
Bed rest and hammock sling traction
اذا جان open book unstable?
Skeletal longitudenal traction and external fixation then? Open. Reduction and internal fixation

Complication

1-siatic nerve injury
Usually neuropraxia(شلل مؤقت)

2-urogenital injury
3-perisitance sacroiliac pain
4-DVT and pulmonary thrombosis
