العلاج بكل سهولة راح اسوي close reduction بحيث اسوي abduction ,extension ,external rotation وأكول للمريض يمشي على عكازة ……

هنا المريض راح يكون متعرض إلى fit or electrical shock فراح يجي بوضعية 👇🏻👇🏻

Extension ,abduction,external rotation with shorting leg

هذا النوع يترافق مع :-


🚨acetabulum fracture
🚨femoral head fracture
🚨posterior hip dislocation
🚨anterior hip dislocation
🚨knee injury (rare )

Fracture of femoral neck :

🚨هذا الكسر كلللللللشششششش خطر لانه معرض إلى non union + a vascular necrosis لانه interacapsular يعني موجود synovial fluid يمنع تكوين hematoma وكذلك لا يحتوي على periosteum والتغذية مالته تكون reterograde supply … الدكاترة يرتعبون منه المريض راح يجيك ياما :-

👉🏻young with history of renal cell carcinoma, steriod uses

👉🏻elderly with history of minor trauma (this most common presention )

المريض راح يجي بوضع flexion ,abduction ,external rotation with shorting leg ……فراح ابلش العلاج :

💡elderly =orthoplasty

♻️Fracture of intratrochantric :


🚨هذا نوعين النوع الاول stable والنوع الثاني unstable ( وهو الأكثر شيوعاً ) ويكون على عدة أنواع :-


🚨reverse oblique
🚨subtrochantric extension
🚨fracture with large perimedial segment


🦋حسب كلام دكتور محمد الكيسات الي شايفهم اغلبها displacement unstable فعلى الغالب العلاج راح يكون ORIF ( واكو طريقة مال nail حجالنا عليها بالسشن كال هو دائماً يشتغل بيها 😅🦧)



♻️Subtrochantic fracture :

🚨هذا جوه lesser trochanter بمقدار 5cm هذا في الغالب يترافق مع interatrochantric fracture وهنا دائماً المريض راح يجي عنده unstable displace r fracture بسبب فعل العضلات بحيث انه gluteus muscle تسحب proximal segment بشكل abduction وrotator muscle تسولي external rotation اما iliopsoas راح تسوي flexion …اما distal part راح تسحبه adductor muscle فراح يصير له addiction …… بسببب كل هذا هنا ضروري تسوي له ORIF 🥹

Femoral shaft fracture :


هذا اول ما تسمعي بي حط فبالك shock قد يكون open or close fracture زين open راح اشوف الدم المفقود زين إذا close شلون اعرفه اول شي راح تشوف swelling بالمنطقة بلس من تجي تفحص pulse راح تقارن bilateral إذا بالجهتين ماكو pulse يعني المريض مشوك 🥺… في الاطفال يصير بسبب العنف الأسري فانتبه لهده الحالة بحياتك المهنية 🐥🦧

فستبل المريض ABCD وره ما تستقر حالته سوي X-ray وبلش الاپروج المناسب للعلاج حسب الفئة العمرية :-

Child 👉🏻traction for 2-3w after that hip spica

Adult 👉🏻external fixation fallow ORIF
