Asymptomatic +normal xray+ positive mri

نعالجة observation every 3-6 month

Pain + normal xray+abnormal mri(early stage)

نعالجة فتحة بلgreater trochanter ونطلعfar marrow بحيث يكدر يسوي new vascular
ومرات نحتاج bone graft

Pain+ abnormal xray+abnormal mri(late stage)

العلاج total hip replacement

فراح تصير حالة من 3 حالات
1-upper root injury(erb’s palsy)
وين الinjury?

C5,C6,C7

Diagnosis is: usually obvious at birth
شلون حيجي؟

Difficult delivery and floopy or flail arm of baby

شنو الحركات الي ميكدر يسويها الطفل؟

Abduction and external rotation and supination

وشحيكوم يسوي ايدة يا حركة؟

Adduction ,internal rotation,pronation and arm held to side

شحيخسر الطفل؟

Loss of finger extension

شنو مراح نكدر نفحصة بالطفل؟

Sensation

2-lower root injuries (Klumpke palsy)

C8,T1

Mechansim?

After breech delivery of smaller baby with arm supination and elbow flextion

شنو الي راح يخسرة الطفل؟

-Loss of interinsic muscle power in hand
-reflexes are abscent
-unilateral horner syndrome

3-total plexus injury

Arm sign?

Arm is flail and pale,all finger muscles paralysis
Vasomotor impairment
Unilateral horner syndrome

العلاج نقسمة 3 مستويات
1-اذا اجه مريض upper root paralysis ورجع الbiceps تشتغل ورة 3 اشهر
معناها

Paralysis may recover completely

بس مو معناها اذا مارجعت ورة ٣ اشهر معناها مترحع بعد ابد!


2- اذا صاير total lesion ممكن ترجع جزء منها!

Paralysis may improve

3- اذا جان صاير complete lesion و خاصة اذا صايرله
Horners syndrome
اهنا:

Paralysis may remain unaltered

يعني احتمالية اقل
بس لازم نكوللهم يحركون مفاصلة!

3- inability to elevated thumb
4-sensory loss of anatomical snuffbox(مراح يكدر يحشش للاسف)

Very high lesion

1-Truma or operation around shoulder
2-chronic compression in axilla

حتصير حالتين

1-Saturday night palsy
2-crutch palsy

شنو الي حنشوفه؟

1-wrist drop
2-triceps paralysed
3-triceps reflex absent

العلاج؟
اذا open injury

Exploration+nerve repair or grafted

اذا closed اهنا شوية دوخة

بالبداية نشوفه اذا

1-first or second degree(elbow injury)

رجع بحدة شوية شوية
بس لازم نجبس (manipulation) ويحاول يحرك مفصل small joint of hand
ليش؟ حتى نبتعد من stiffness

2-fracture in humerus

a-اول شيء نسوي examination of radial nerve


b-وراها نجبسة manipulation حتى ميصير injury اكبر ويتضاعف او نسويله internal fixation اذا الfracture displaced


c-ورا ما نفتح الcast نسويله هم radial examination
