treatment of acute PACG
1-admission to hospital
2-medical treatment👍🏻
•carbonic anhydrase inhibtors=decrease aqueous that decrease IOP
بالبداية ننطيها acetozolmide 500mg iv
وراها انطيها يوميه 4 مرات 250mg orally

•pilocarpine اربع مرات topcially
تزيد drianage فيقلل IOP
•beta blocker(timolol)
مرتين باليوم topical
•steroid to avoid permanent adhestion between iris and cornea


استخدمنا هل ادوية وورا ساعة فحصنا وشفنا بعده صاعدIOP
شننطي؟
20%mannitol 1-2gm/Kg iv over 45mintue
ورا ميصير زين؟ وينزل شنسوي؟

بس بشرط مصاير بي perminant anterior synechia(ولهذا نستخدم steroid)
بس اذا جانا متاخر وصاير perminant anterior synechia

شيسوي هذا حيكون vessel جديد للretina بالاماكن الي بيها ischemia الي صارت بسبب hypoxia👍🏻
بس هاي veesel حتكون هشة فحيصير rupture وتسوي عمى(sudden blindness)
بسبب؟ retinal or vetrious haemorrhage
فمرات تعبر الVPGF
الى anterior chamber ويروح يسد trabeculam
زين واحد يسال شلون صار hypoxia وسوت ischemia?

-long standing retinal detachment

نجي نحجي العلاج
1-medical treatment
•timolol(beta blocker) topical
•acetozolamide( carbon anhydraze inhibtor) topical
•topical atropin
•topical steroid
اخر ثنين يقللن inflammation and gonested even IOP high👍🏻
2- panretinal photcoagulation with laser
ندمر الtissue احسن من ما يبقى hypoxic ويطلعلنا VPGF اكثر🤣
بس نبقي macula حتى يشوف خوما تعمي وانت تعالجة حتى يفصلك👍🏻
3-trabeculectomy👍🏻
4-cyclodestruction
ندمر ال cilliary body حتى ماتفرز اكيوس فلود اكثر
5- retrobulbar alcohol injection
حتى نكتل الاعصاب sensoryحتى يروح الالم
6-enculation اخر الحلول😔
