🦋Maybe patient came with history of recurrent miscarriage in 2nd trimester
🦋history of painless cervical dilated with miscarriage or preterm labour /delivary
🦋hystosalpingograph show fundal cervix
🦋US show length of cervix less than 2cm
🦋time of insertion cerclage 13w (12-14w)
🦋time of remove cerclage 37w (but if was elective SC ,more than this interval )
💡انتبه لكلامي فيميل صار عندها PROM بالاسبوع 24-32 وهي حاطة cerclage وماكو infection انتظر 48 ساعة يلا
تشيله يجوز تكدر تصحح ال PROM 🤝🏻
💡انتبه لكلام مرة لخ أجتك بيشنت عندها threatend miscarriage ( يعني حملها مهدد باي طريقة كانت ) وهي بالاسبوع 20-17 و مضطرة تسوي cerclage هنا الوحيدة الي اكدر اسوي لها rescue cerclage ( اما غير حالات ممنوع منعاً باتاً خاف يصير عندها contraction تقود إلى preterm labour )

إذا أجتك فيميل عندها prolong uterine bleeding وما تريد تسوي hysterectomy شنو راح تسوي لها ؟
راح تكوي بطانة الرحم endometrial ablation طبعاً هذه كومة يسونها حسب كلام دكتورة آلاء بس هم لازم يكونون مدركين لمضاعفات العملية ومن اهم المضاعفات احتمال يصير عند البيشنت amenorrhea واذا ما صار راح يصير placenta acarreta ……واذا الدكتورة فاشلة احتمال تحرك الرحم يعني توصل لحد myometrium فهنا سلملي على الuterus والإيميل لانه هنا راح يصير pulmonary embolism (تسبب الموت ) او اضطرّ اشيل الرحم
