العلاج:

1-Oligomenorrhea/amenorrhea 75%
. Lifestyle modification
.metformin( oral biguanide)
Increase insuline sensitivity and improve menstural cycling and ovulation.

.cyclical progesterone
(Medroxyprogesterone acetate)10 mg daily for 10 day in luteal phase from day16 to 26

.alternative oral contraceptives
ننطي للمرأة متريد تصير حامل

2-hirsutism
-cyperoterone acetate
يشتغل ك
.anti androgen that inhibit androgen receptors
وما يجي بحدة وانما يجي وية مركب ثاني
Dianette/ diane35(2mg of cyperosterone and 35 microgram of ethylene estrdiol )
-yasmin :contain drosperinon has anti androgen effect
.inhibit ovarian androgen production
.block androgen receptors

-metformin act as insuline sensitizing agent
To improve
.insulin resistance
.hyperandrogensim
.anovulation and acne

-local treatment:
a.mechanical depilation
b.elctyrolysis
c.eflornitihine hydrochloride cream

3- subfertility 70%
- letrozole (aromatase inhibtior) first line🔥

-clomphine citrate (increase fsh and act at anti estrogen)
المرأة الي تصير عدها ovulation ومديصير عدها حمل لمدة ٦ اشهر ننطيها هل

-tamoxifien بس غير محبب لان يسوي bleeding

-Metformin +clomphine citrate better than metformin monotherapy

-exogenous Gonadotropin
But risk of multiple pregnancy and OHSS(2nd line)

-Lod(laproscopic ovarian drilling) third line
التاثير نفسه الي يسوي gonadotropin بس مابي risk الي سواه الgonadotropin

4-obesity 40%
-weight loss
-metformin
-orlistat
-bariatric surgery
