هو عبارة عن شنو اعراض نفسية وجسدية و حركية بجسم البنية تصير بفترة luteal(premenstural)
هو يصير بكل النساء الي يكونون بلreproductive age بنسبة 60%
الاسباب مالتها وهي ٣ اسباب بس محد مثبتها لحد الان:
1-estrogen progesterone imbalance (decrease estro and increase prog)
Increase aldosterone ,hypoglycaemia, hyperprolactenmia
2- physiological ovarian function trigger (الاكثر شيوعا (
3-serotonin decrease and gaba level decrease

Diagnosis
بالبداية من خلال الhistory الناخذه من المريض الاعراض حتخلينا نستبعد
1-thyroid disease
2-anemia

الاعراض الي يكولها المريض symptoms
1-Mood symptoms (irritability,mood swings,depression,anxiety)
2-physical symptoms (bloating,breast tenderness,insomnia,fatigue,hot flushes,increased appetite)
3-cognitive changes (confusion,poor concentration)
شوكت تظهر الاعراض؟ بالنصف الثاني من الmens تسمى luteal phase
شوكت تكون البنت خالية من الاعراض؟at least 7 days in first half of cycle
شوكت نثبت وجود المرض؟ بالقليل يتكرر مرة بالشهر خلال شهرين متواصلين
شوكت نعرف انه هاي الاعراض هي مال هذا المرض؟ لدرجة هلكد ماقوي ماتتحمل وتطلب علاج ومسكن

Examination
-Migraine like headache
-visual scotomas( اكو غواش بالنص او نقطة ميشوف بيها بنص الرؤية)
-vomiting
-mastodynia or mastalagia (breast pain)
-depression

Treatment
1- cocp
Yasmin
Daily for 3 month
Bicycling or tricycling pill packet

2-transdermal estrogen
3-SSRI like fluxetine
4-GnRH analuge
يستخدمون وياه HRT حتى يقلل من Osteoporosis
وهو يشتغل ك switch off of ovarian activity
5-hysterectomy and bilateral salpingo-oopheractomy
نجرب قبله GnRH حتى نطفي الovary اذا فاد نشيلة
بس نسوي هل عملية اذا فشل كل العلاجات ومديفيد والبنية مدتتحمل
6-vitamin like magnesium,calcium and isoflavones
7-evening primrose oil
8-cognitive behavioural therapy

الم اثناء الmens cycle
البنية لازم تفرق بين الم الدورة الاعتيادي الmild وبين هذا الالم الي مايخليها تسوي امورها اليومية وتضطر تاخذ مسكنات
بيه نوعين
Primary dysmenorrhea non organic cause

Secondary dysmenorrhea
Organic cause
-endometriosis
-adenomyosis
-PIV
-cervical stenosis
-fibroid
-endometrial polyp
ال pathogensis
1-prostaglandin activity
•abnormal increase of prostanoid and eicosanoid lead to increase uterine contraction that reduce uterine blood flow lead to utrine hypoxia

2-vasopressin
نعالجة بatosiban

3-psychology factor

Clincal findings
History is most important question is to diagnosis:
-when dos the pain occur?
-what dose the paitent do about pain?
-are there other symptoms (vomiting,bleeding from other side =endometriosis bleeding per rectum)
-do oral contraceptives relieve pain?
-dose the pain become more severe over time?endometriosis

ال dysmenorrhea مراح يصير بلmenarche
وانما بل adolescence
شوكت يصير الالم؟
On first day of mense وممكن يروح بثاني يوم
ويصاحبة؟ nausea vomiting diarrhoea and headache

If pain in pubis= primary
And if in right or left
لازم نسوي ultrasonography و laparoscopy حتى نستبعد الsecondary او نعرف شنو سبب

العلاج
First choice of treatment
Is Nsaid antiprostoglandin
Like naproxen and ibuprofen
But can get it cox 2 inhibitor
Like
-Rofecoxibe
-valdecoxibe
-lumiracoxibe
هو نفس الكفاءة بس مو احسن من الnaproxen
شوكت نستخدمه؟
قبل يوم او يومين من Bleeding or cramping اذا جانت الmense regular اذا لا شوكت ما يجيني الالم استخدمه!
2- oral contraceptives
Indicate
a- pain wasn’t relived by nsaid
b- cannot tolerate them( have ulcer due to nasid use)
بحيث حيوكف الovulation و decrease prostoglandin production

3-Surgical
.laproscopic uterisacral ligament division
.cervical dilitation
.presacral neuroectomy (most extreme choice in primary)


اذا جان secondary ومستجاب للعلاجات نسويله hysterectomy
4-Adjuvante treatment
.continue low level topical heat therapy
. Excersis
