و neurological change

العلاج: removal tumour

Pitutary gland
a- hyperprolactenmia
الاسباب
-medication drug
-pituitary adenoma
-chest wall injury( mild elevated)
-hypothyroidism

Lab investig?
-Prolactine? If elevated 2-10 times more than normal usually indicate pitutary adenoma
-MRI for pitutary adenoma
العلاج: removal of space occupying pituitary or brain tumor
-correction of hypothyroidism by thyroxine
-conservative by dopamine agonist( if it not pitutary adenoma cause)

Case4:
Sheehan syndrome
( pituitary infarction)
سببها؟
بنية حامل وجابت وراها صار عدها post partum haemorrhage or sever hypotension
ف راح يضرب pituitary ويفلشها ف طبيعة الحال كل الغدد الي تفرزها حيصير بيها نقص لان مدتصنع
ف يصير بيها
-hypogonadotropin hypogonadisim( Decrease in LH and FSH)
-2ry hypothyroidism (loss of TSH)
-2ry adrenal insufficiency
(Loss of ACTH)
-vasospasim of pituitary arteries cause sever hypotension leading to hypo perfusion of gland
*راح تجينه المريضة جايبة طفل وراها صار عدها نزيف وانخفاض شديد بالضغط و ماتكدر ترضع ( inability to lactate in postpartum women)
فراح يصير amenorrhea immediately postpartum or year following delivery
ال diagnosis من الاسباب نسوي تحاليل وهمين clincally نكدر نعرفه
وهم بلMRI هم نشوف abnormal pituitary gland
العلاج مالته : ننطي hormonal therapy
.low estroidal and not respond to progesterone only

ملاحظة مهمة: infiltrative process can occur in pituitary
1- hemochromatosis
2-lymphocytic hypophysitis
شحيسوون decrease gonadotropin secretion

Case 5:
spontaneous primary ovarian insufficiency (POI) or spontaneous loss of overian function or premature ovarian failure

تجيك وحدة menpause او قريبة من عمر ال40(قبلة)
تكلك اخر فترة اخر كم شهر مداصير عدي. Ovulation ولا mensturation
تجي تسالها تكللها مسوية عملية من قبل؟ تكلك اي واذا لا؟ ممكن autoimmune disease!
وتشوفها عدها اعراض مال قلة ال estrogen
٠hot flushes
• vaginal dryness
• night sweat

فيعني السبب
-idiopathic during surgery or chemotherapy (most common)
- autoimmune disease
-Turner syndrome(Lack of second X chromosome)
-fragile X syndrome
-history of mumps and cytomegalovirus

نسويلها
-FSH = elevated( greater than 30 to 40 IU/L)
-Estroodal= Low (below 30pg/mL)
-antimullerian hormone :decrease(very low)
العلاج :HRT (combination)حتى نمنع الbone fractureو نمنع حصول gential atrophy
+ calcium

ملاحظة كلش كلش مهمة:
If estridiol low in
-POI
-menopause
-sheehan syndrome
-hypothalamic amenorrhea

The women will not respond to progesterone with vaginal bleeding

*Asherman syndrome not respond to pct because uterine lining cannot regenerate

Case 6: PCOS
تجينا البنية تكول

-hirstuism

ملاحظة:
Pcos most common endocrine disorder in women of reproductive age 7-10% of all women

تصير بالنساء الobese اكثر من الضعاف بس علاجه انه تضعف نفسها
المخاطر:
-prediabetes
-T2DM
-metabolic syndrome
-fatty liver disease
-OSA
ال investigates
-testosterone (free and total) = elevated
-dehydroepiandrosteron = elevated
- androgen = greater than normal 2 times
-ultrasound in pelvis show pcos
-anti mullerian hormone = increase 2-4 times more than normal

-ogtt +fasting glucose +ha1c= Screen for T2dm of pcos
العلاج:
Oral contraceptives: treat hirsutism (especially if don’t want pregnant) use progesterone course if woman is screating adequate level of estradiol بحيث يستجاب خلال يومين الى اسبوع ورة اخر جرعة من ال progesterone
شلون ينطوا؟ 10-13 يوم خلال كل شهر او بين شهر وشهر
بس لازم نركز انه اذا صار development of endometrial hyperplasia نسويله withdrawal
اما اذا مستجاب للprogesterone بحدة؟ ننطي combination with estrogen
