By investigation found klebsiella & US enlarge kidney & loss of corticomedullary differentiation & IVP delay excretion &CT enlargement area & decreased perfusion
& he have Abscess & scarring dx?

Pt come with Renocolic fistula by imaging found hypodense area well defined & thick enhancing wall necrotic non-enhanced Center & thickening of gerotas fascia


شنو السبب

Patient have psoas abscess by imaging found hypodense area surrounded the kidney & kidney dilated with mild hydronephrosis
What the most common cause

Pt known case of DM sufferings strong, persistent urge to urinate. & Pain or a burning feeling when urinating. For less than 2wk

Imaging reduced bladder volume & emphysematous
DX & cause

احس حلة غلط والجواب فقط A بعد مادري

40 yrs Have blood in the urine or passing urine frequently or during the night, change in urine colour – dark, rusty or brown & a lump in the abdomen & rapid, unexplained weight loss by imaging
Us=hyperechoic / isoechoic with central necrosis
Colour Doppler = IV thrombus
CT= large distort kidney & necrosis or calcification
DX?

Renal cell carcinoma
TCC
Angiomyolopoma
Oncocytoma
Lymphoma

Adult have Blood in the urine
& Pain or burning with urination
& Back pain
& Unexplained weight loss
By imaging

IVP /CT urography = irregular filling defect in renal pelvis

US= isoechoic solid mass with obliteration of renal pelvis

CT=central location
-preserved renal contour
Poor contrast enhancement

1-Renal cell carcinoma
2-TCC =Transitional cell cancer of the renal pelvis and ureter



3-Angiomyolopoma
Oncocytoma
Lymphoma

Angiomyolipoma
-benign
Commonest benign in adult
Lipoma=fat
US/circumscribed highly reflective mass بيضاء
CT= fat density +No calcification
راح تبين سوداء هنا
لان بس haemorrhage & calcification يطلع ابيض بل CT

MRI = high signal on T2 loss signal in STIR

هاي بنص الكلى تضرب Renal pelvis
اكثر شيء يبدي بل bladder وبعدين لل kidneys سو حيروح لل renal pelvis وبل CT حتطلع preserved renal contour محافظة ع حواف ما بها شيء

Solid hyperechoic mass (+/- cystic areas)
الmass راح تكون بيضاء وممكن تحتوي أو لا ع cyst
CT
Large heterogeneous enhancing mass
لا هي سوداء ولا هي بيضاء
-pseudocapsule
Calcification <10 % قليل
بالمقارنه بلي بعدها

Neuroblastoma

هي تشبه ال Wilms Tumor
بس راح نميزها من خلال العمر هي اقل السنتين سنة ونص

Hyperechoic solid mass
Hypo= hge /necrosis highly vascular in Doppler


CT
=heterogeneous solid mass
-calcification 90%
-encase vessels ويضغط عليها
65% metastases at presentation (bone /LN)
