•no response to sertoli
•papilits and altudinal visual feild defect =permanent damage

About ocular involvement
وتكلك المريضة كانه تنزل بردة سودة على عيوني ثوواني وتروح

تصير بسبب
Rapid increase in most case

باعوا بما انه دنحجي على ocular
اكو معلومة بداية الكيس كلنا انه كمت ماشوف

From abducent VI nerve palsy
وهاي الحالة يسموها؟
False localising sign
ليش هيج سموها؟
لان مجرد ما يرجع ضغط الرأس للطبيعي حترجع الرؤية وتروح diplopia
وياأكذون من هلشيء انه ماكو brain tumour عن طريق image
وهو يصير من تصعد ضغط الرأس حيصير brain edema congestion that compress abducent nerve

زين احنا كلنا انه حيصير brain edema عن طريق csf زين شلون؟
لان هو اصلا صاير venous outflow obstruction

الvisual loss احنا مو كلنا انه بسبب كومبرشن صاير بل ابديوسنت؟
ف الvisual loss حيأدي الى secondary optic(II) nerve atrophy

زين حنحجي على visual accuity تكون نورمال باغلب المرضة بس اكو عدهم
Moderate to sever papilledema
بحيث يصير عدهم
Constriction of visual feild and enlargement of blindspot by opthalmoscop
بحيث يسموي bilateral optic disc swelling
وبقية الاكزام تكون طبيعية

اذا سوينا follow up للمريض شراح يصير؟
•normal good vision
•headache subside
•optic disk swelling not found
•visual feild is normal

بس مانكدر نعتمد على csf manometry
لان ممكن csf pressure يبقى صاعد لسنوات عديدة

ركز على كلمة false local sign وحاول تفهمها لان تجي mcq

وهي من خلالها تفرق انه عنده تيومر لو اديوباثك 🥶

من نشوف حالاته الفوگ Clincal
وراها نقرر نسوي CT and MRI
حتى نشوف اكو تيومر؟
فنشوف؟

وعن.طريق الmri حنشوف
Optic nerve sheath is characteristic by
•dilated
•back of globe is flattened

اما اذا شفنا اكو تيومر
فشنسوي؟

CSF PRESSURE

نشوفه صاعد اكثر من 250mm
فاحنا نقلل 20-40ml of csf
بحيث شوية يعالج الم الرأس
بقية شغلات الcsf تكون نورمال

اذا جان مموجود لا اكو slit like ventricle
ولا اكو تيومر
فنفكر بل
Psedutumor cerbri
فنسوي تحاليل
مثل؟
•endorcrine and metabolic screen
•Connective tissue screen
•neoplastic metastatic screen
•MRV for cerebral venous thrombosis

اذا طلع نگتڤ معناها IIH
اذا positive
فممكن بسبب TB meningitis

العلاج
اذا جان

Pseduo tumor cerbri

نعالج السبب
وهمين ننطي دوة يقلل من ICP

اما اذا جان

Idiopathic intracranial hypertension

فننطي medical
1-acetozolamide
1000-1250 mg/d
Side effect
•frequent dialysis
•electrolytes disturbance
•pain and tingling sensation in finger

2-topiramate 50-100mg/day
S.e: pain and tingling sensation in finger

اذا جانت حامل لازم نراقبICP عن طريق تكرار lumbar puncture
ونسحب اذا جانت اكو زيادة ب20CC

ومرات نتدخل جراحياً
اذا جانت عده optic nerve sheath fenestration and
Ventriculoperitoneal shunt and lumbo peritoneal shunt
اذا مفادت الادوية

يضعف نفسه وكلش مهم هل شبء
ونستخدم corticosteroids وهو كلش مفيد
بس بي unwanted side effect
هو يعالجة بس يكطعة يرجعلة
فيضرة اكثر من ما يفيدة

Intracranial hypotension

احنا مو كلنا انه علاج الهايبر انه نسوي lumbar puncture
ونسحب بين 20-40cc
اذا سحبنا اكثر شحيسوي؟

Post lumbar puncture headache

وصفاته انه يكون
•postural headache
•increase in upright position
•relife in recumbancy بالاستلقاء
عكس الhypertension

•in occiptal area
•comes on within 48-72 hours after lumbar puncture
وتروح ورة يوم او يومين ومرات توصل اسبوع

Nausea and vomiting may occur
ومرات اكو
Positive meningial sign
•neck stiffness
بس ماكو اعراض الmeningitis
وهمين اكو history of spinal anesthesia
ومرات يصير بسبب
Leak of csf from spinal subarachnoid spacr
شلون امنع الموضوع؟
By using small gauge needle 22 or smaller for puncture
ويكون lateral position and
Proper manoeuvre
ويكون lying flat afterward

Bed rest
Good hydration
Anti emitc
Iv caffine sodium benzoate 500mg
ونكررة ورة 45دقيقة اذا الالم بقة
واذا مفاد والالم بقة؟
نضربة ابرة
Autologous blood into epidural space at the site of puncture
يعني ناخذ دم من عده ونضربة بهل مكان
