نسويهن ثنينهم upper and lower motor neuron sign

Upper sing?
Spasticity
Upgoing planters معناها يخسر planter reflex

Lower sign?
Fasciculation
Muscle atrophy

ملاحظة مهمة!
No sensory sign
No involvement of extra ocular muscles or sphincter
اذا اي وحدة من فوكهن اكو معناها تشخيصك غلط وغير مرض هذا

اذاً اذا عمرة اقل من 30 او ماعده احساس او عضلات عينة متأذية ف اهنا تشخيص غلط

DD:
-cervical spondylosis
-cervical cord compression by tumor
اذا جان C5 معناها
Give LMNL upper limb + UMNL for lower limb

اذا اعلى من 5? يعني4
Give UMNL for upper limb
-foremen magnum tumor
-multifocal motor neuropathy
-chronic lead poisoning (urinalysis نسوي)

نسوي MRI حتى نستثني الmagnum tumor
نسوي EMG ختى نشوف اكو denervatice لان اكو atrophy


نسوي CPK enzyme لازم يكون normal او mild elevated لان اذا كلش عالي حيدل على انه lead toxicity
نسوي muscle biopsy حتى نشوف اكو denervative

100% definitive diagnosis?
Combined sign of upper and lower motor neuron in bulbar reign+ at least two spinal regions (cervical ,thoracic or lumbosacroal)
Or three spinal region

نستخدم
Riluzole
حتى ناخر المود 2-3 لشهر ويشتغل على توقيف glutamate release


نستخدم edaravone(radicava)
هم حتى ياخرة شهررين ثلاثة
ونستهدم symptomatic treatment
If dysphagia=NG tube
Respiratory difficulties = tracheostomy and mechanical ventilation

استراحة لل دعش وعشرة ونكمل

In adult=LMNL=weakenss more in distal

In paediatric =LMNL=weakness more in proximal

•Weakness proximal more than distal
•fasciculation

Infantile spinal muscular atrophy (werding Hoffmann disease of SMA-I)
