شلون حيجي يوصفة المريض؟
Begin as a burning sensation over lateral aspect of nose or as a pressure behind the eye
وياها شنو مرات؟
Ipsilateral conjunctival injection,lacrimation , nasal stiffness or Horner syndrome(ipsilateral ptosis and myosis)

واغلب الحالات بالليل بحيث يكعدة من النوم والرجال اكثر من نساء ب٣ مرات ومعدل العمر 25 سنة وعده family history

Treatment
1-acute phase?
a-oxygen high concentration 7-12 litres for 15-20 mint(100%)
B-subcutaneous sumatriptan injected
2-sever cluster?
Lithium

3-other drug?
IM or subcutaneous or oral ergot(بس انكطع لان يسوي ادمان)
او
Subcutaneous octeriotide
او
Intranasal lidocine

الprophylaxis
1-verapmile 80mg ثلث مرات باليوم
2-valproid acid
3- no role of botox
4- gabapentine
5-melatonine
6-occiptal nerve stimuli
7-topirmate
8-short case of oral corticosteroids

Tension type headache

Its most common type of headache 🤓
Is chronic disorder appears after age 20 years
وكلعادة يكون الميكانزم unknowing
ومرات يكولون عليه هو milder version of migraine

•nonthrobbing
•bilateral occipital head pain not associated with vomiting or nausea 🤮❌
Not associated with visual disturbances (photophobia)❌

العلاج؟
1-acute attack
•aspirin
•nsaid
•acetomenophin

ملاحظة مهمة: اذا صار عدها tension tye headache in maigranius فنستخدم sumatriptan
Prophylactic:
•amitriptalyine or nortriptyline or impirmine
•no role of botox in tension
•massage,physica therapy

بس انه يبقى متزامن هذا شيء جدا نادر
وتسالها يكعدج من النوم؟ تكلك مو دائماً لو ما تصير اصلاً

يكون مثل multiple sclerosis او brainstem tumours
وتلكه بيه خلل مثل عيونة توجعة وهذا يضرب الشباب😔
وخاصة يصير bilateral
زين بصورة عامة من نسوي CT,convential MRI,arteriography حيطلع normal
بس من نسوي high resolution MRI

Microvascular nerve compression

نادرا مرات يصير
Trigeminal neurolgia +cluster headach

العلاج؟
غالباً يستجاب للcarbamazpine
ويستحب انه نبدي بلlow dose وراها نزيد شوية شوية
400-1200mg orally
اذا مفاد نبدي نستخدم
Oxyxarbamazipine
Gabapentine
Pregabaline
Amitriptaline
Glucocorticoid
Lamotrigen
Baclofen
للacute attak
Iv phenytoin 250mg followed by orall 100-300mg
اذا مفادت الادوية نسوي عملية
Decompression of trigeminal nerve from superior cerebella artery
ومرات ننطي حقن alcohol or phenol لل periphral branch of nerve

Younger age than in trigeminal neuralgia

يبدي بالبداية swallowing يتبلعم ويبين اثناء الكلام

Carabamazpine or phenytoin
